Per oral endoscopic myotomy(POEM) is a novel endoscopic procedure for achalasia treatment. Due to its novelty and high success rates, a repeat procedure is usually not warranted, making the feasibility and safety of such approach unknown. We report the first case of a successful repeat POEM done at the same site of a previously uncompleted POEM. An 84-year-old female with type 2 achalasia presented for a POEM procedure. The procedure was aborted at the end of tunneling and before myotomy due to hypotension, which later resolved spontaneously. POEM was re-attempted at the same site of the original tunnel 1 year afterward, and surprisingly we didn't encounter any submucosal fibrosis. The procedure felt similar to a native POEM and a myotomy was performed uneventfully. Our case is the first to suggest that submucosal tunneling during a repeat POEM can be done at the same site. Hypotension during POEM is a rare complication that should be recognized as a potential result of tension capnothorax, it can however, be managed with close supportive care.

We report on large work function shifts induced by the coverage of several organic semiconducting (OSC) films commonly used in organic light emitting diodes (OLEDs) and organic photovoltaics (OPVs) with a porphyrin aggregated layer. The insertion between the organic film and the aluminum cathode of an aggregated layer based on the meso-tetrakis(1-methylpyridinium-4-yl) porphyrin chloride (porphyrin 1), with its molecules adopting a face-to-face orientation parallel to the organic substrate, results in a significant shift of the OSC work function towards lower values due to the formation of a large interfacial dipole and induces large enhancement of either the OLED or OPV device efficiency. OLEDs based on poly[(9,9-dioctylfluorenyl-2,7-diyl)-co-(1,4-benzo-2,1′,3-thiadiazole)] (F8BT) and incorporating the porphyrin 1 at the cathode interface exhibited current efficiency values up to 13.8 cd/A, an almost three-fold improvement over the efficiency of 4.5 cd/A of the reference device. Accordingly, OPVs based on poly(3-hexylthiophene) (P3HT), [6,6]-phenyl-C61 butyric acid methyl ester (PC61BM) and porphyrin 1 increased their external quantum efficiencies to 4.4% relative to 2.7% for the reference device without the porphyrin layer. The incorporation of a layer based on the zinc meso-tetrakis (1-methylpyridinium-4-yl)porphyrin chloride (porphyrin 2), with its molecules adopting an edge-to-edge orientation, also introduced improvements, albeit more modest in all cases, highlighting the impact of molecular orientation.

The purpose of the research is to theoretically establish the simple characteristic diagrams of the rotating machine and to verify them by experiments. In order to reach this goal, the scheme of an experimental setup was elaborated, as well as its constructive solution. The final part of the paper presents the method of the measurements to be performed. The experimental data are processed and compared to the theoretical ones.

Cuboid fractures due to the particular bone anatomy and its protected location in the midfoot are rare, and they are usually associated with complex injuries of the foot. Clinical examination to diagnose these fractures should be detailed and the differential diagnosis, especially in the case of vague symptoms, should include the exclusion of all lateral foot pain causes. Conventional radiographs do not always reveal occult fractures, which can be under diagnosed especially in children. In this case, further investigation including magnetic resonance imaging or scintigraphy may be required. The treatment of these injuries depends on the particular fracture characteristics. Non-displaced isolated fractures of the cuboid bone can be effectively treated conservatively by immobilization and by avoiding weight bearing on the injured leg. In the case of shortening of the lateral column> 3 mm or articular displacement > 1 mm, surgical management of the fracture is mandatory in order to avoid negative biomechanical and functional consequences for the foot and adverse effects such as arthritis and stiffness as well as painful gait. In this review, an update on diagnosis and management of cuboid fractures is presented.

Osteitis pubis(OP) is a debilitating overuse syndrome characterizing by pelvic pain and local tenderness over the pubic symphysis commonly encountered in athletes often involved in kicking, twisting and cutting activities in sports such as soccer and rugby and to a lesser degree distance running. It is a common source of groin pain in elite athletes attributable to pubis sympysis instability as the result of microtrauma caused by repetitive muscle strains on pubic bones. Diagnosis is based mainly on detailed sports history and a meticulous clinical examination, although occasionally is difficult to distinguish this nosological entity from other pathologies affecting the involved area which may occur concomitantly in the same patient. Radiologic examinations such as plain radiographs, magnetic resonance imaging and 3 phase bone isotope scanning may be helpful to differentiate from other clinical entities with similar clinical presentation. Most cases respond well to conservative treatment which includes several physical modalities and especially a progressive rehabilitation programmed individualized to each one of patients diagnosed with OP. Local injection therapies have been also been proposed as a non-operative therapeutic option for the efficient management of these patients. In refractory cases, surgical therapeutic strategies are warranted. These include several open or minimally invasive surgical interventions such as arthroscopic or open symphysis curettage, wedge or total resection of pubic sympysis, polypropylene mesh placement and pubic fusion. In this review a critical analysis of OP in elite athletes is performed with special focus on current concepts of diagnosis and management of this source of athletic groin pain.

The Agios Athanasios ore deposit is located within the wider area of Ieropigi in Kastoria, Greece. The specific ore deposit is developed in form of layers between ophiolites and Tertiary molassic conglomerates. The main mineralogical components are hematite, goethite, quartz, and secondarily, garnierite, lizardite, saponite, willemzeite and sepiolite, while scarcers are chromite, calcite and nepouite. Nickel is mainly found in garnierite, willemzeite and nepouite, which in coexistence with quartz are the main components in the binder material of the ore. For the mineral processing gravimetric and magnetic separations are used in the size of fractions -8 + 4 mm, -4 + 1 mm, -1 + 0.250 mm and -0.250 + 0.063 mm. The chemical and mineralogical analysis in combination with microscopic examination showed that mineral processing by gravimetric separation gave the most satisfactory results for the size fraction -1 + 0.250 mm.

Transforaminal Percutaneous Endoscopic Discectomy(TPED) is a minimally invasive technique mainly used for the treatment of lumbar disc herniation from a lateral approach. Performed under local anesthesia, TPED has been proven to be a safe and effective technique which has been also associated with shorter rehabilitation period, reduced blood loss, trauma, and scar tissue compared to conventional procedures. However, the procedure should be performed by a spine surgeon experienced in the specific technique and capable of recognizing or avoiding various challenging conditions. In this review, pitfalls that a novice surgeon has to be mindful of, are reported and analyzed.

Honey bee colonies in Peloponnesos, Greece, were sampled in spring 2014 to evaluate efficacy of Provigoro?, a natural water acidifier, against Nosema spp. Colonies treated with Provigoro? showed reduced Nosema spore load than did colonies that received only sugar syrup treatment, which finally collapsed. Preliminary results suggest that Provigoro? was successful at reducing nosemosis in Apis mellifera colonies.
