Bacillus Calmette-Guérin (BCG) is a live attenuated form of Mycobacterium bovis, initially used in medicine as a vaccination agent only. The discovery of its antineoplastic effects in bladder cancer has led to the widespread recognition of BCG intravesical instillation as a therapeutic option. Although sepsis following BCG intravesical instillation is rare, it is nonetheless a dreadful and potentially fatal complication. Therapy usually relies on antituberculous therapy and steroids, alongside with intensive care unit admission. The authors report a case of a 67-year-old male patient who developed septic shock with multiple organ dysfunction after intravesical BCG instillation and review the currently available knowledge concerning the risk factors, diagnosis, management and prevention of BCG sepsis.

Evaluation of Different Parasitological Techniques for Diagnosing Intestinal Parasites in Dogs

Intestinal nematodes are among the most common parasites of dogs, being Toxocara spp. and Ancylostoma spp. threats to public health due to their zoonotic potential. In this study, three parasitological diagnostic techniques for detecting the eggs/oocysts of canine intestinal parasites were evaluated. Fecal samples (n = 285) were collected from the environment (i.e. public streets, squares and homes) in Garanhuns, Pernambuco, Brazil. Samples were analyzed using the Willis-Mollay simple flotation technique, spontaneous sedimentation (Hoffman, Pons and Janer) and modified centrifugal flotation. Among the samples analyzed, 56.49% (161/285) showed eggs and/or oocysts of intestinal parasites. In particular, 44.21% (126/285) were detected by the Willis technique, 45.14% (121/268) by centrifugal flotation and 31.57% (90/285) by the Hoffmann technique. The kappa index indicated good concordance between the centrifugal flotation and Willis techniques (κ = 0.625), whereas fair concordance was observed between Hoffmann and Willis (κ = 0.480), and between Hoffman and centrifugal flotation (κ = 0.433). In conclusion, the centrifugal flotation technique showed high sensitivity towards detecting Ancylostoma spp. and Toxocara spp. (the most common parasites in this study) and could be used in association with the Willis technique to obtain safe diagnoses within routine laboratory analysis.

Adiponectin as a protective factor in hepatic steatosis

AIM: Obesity and insulin resistance (IR) are closely related to hepatic steatosis (HS), and adiponectin is a hepatic insulin sensitizer that has important effects in liver function. This study aims at investigating the relationship between serum adiponectin concentration and the presence of HS. METHODS: We carried out a cross-sectional study in a check-up unit of a University Hospital in Mexico City. We enrolled 196 subjects, comprising 98 subjects with HS (27 women, 71 men) and 98 controls (37 women and 61 men). Anthropometric, metabolic and biochemical variables were measured in the two groups. Serum adiponectin and leptin concentrations were determined, their association with grade of HS tested, and concentrations, according to quartiles, compared between cases and controls. X2 analysis for linear trends was used to test for a dose-response relationship and logistic regression analysis was conducted to test for a protective effect of adiponectin. RESULTS: The HS subjects were older and more obese than controls, with a central obesity pattern. In the fourth quartile of adiponectin concentrations, HS was less common and severe. In a multivariate model of the fourth quartile of the adiponectin concentrations, we observed a protective effect (OR = 0.17, 95%CI: 0.04-0.67, P= 0.01). In subjects with more severe HS, we observed higher leptin concentrations, and caloric intakes, total fat and iron consumption were higher than in controls. CONCLUSION: The results of the present study suggest that a high serum concentration of adiponectin is associated with a protective effect against HS.

Leaf Essential Oil from Three Exotic Mytaceae Species Growing in the Botanical Garden of Rio de Janeiro, Brazil

The leaf essential oils of three Myrtaceae species: Melaleuca leucadendra, Lophostemon confertus and Ugni molinae, non-native to Brazil and growing in the Rio de Janeiro Botanical Garden, were obtained by hydrodistillation and analyzed by GC and GC-MS. Oil yields from fresh leaves were 0.76%, 0.08%, and 0.04%, respectively. Sixty-seven percent of M. leucadendra oil was made up of monoterpenes, wherein 1,8-cineole was largely predominant (49%), followed by α-terpineol (7.6%) and terpinen-4-ol (4.3%). The sesquiterpene profile was characterized by a significant amount of viridiflorol (29%). Oil from L. confertus was 42% monoterpenes;the most predominant constituents being α-pinene (20.8%) and α-thujene (7.1%). Total sesquiterpenes (55.4%) comprised spathulenol (28%), globulol (14%), and aromadendrene derivatives (8.5%) as the main species. U. Molinae oil was exclusively composed of sesquiterpenes, with a predominance of β-elemene (44%) followed by β-caryophyllene (7.1%), and bicyclogermacrene (6.7%);guaiol-type alcohols made up 30% of its profile. To the best of our knowledge, this is the first time U. Molinae leaf oil composition has been described. The composition of M. leucadendra oil corresponded to a chemotype described for a species growing in Cuba, but was different from that produced by M. leucadendra growing in Egypt, Tunisia, and elsewhere in Brazil. The profile of the L. confertus leaf oil showed high similarity with those described for Australian native specimens, in particular in relation to the relevant presence of α-pinene, aromadendrenes, and the guaiol-type alcohols.

Low serum levels of ghrelin are associated with gallstone disease

AIM： To explore the role of ghrelin in gallstone disease. METHODS： We carried out a cross-sectional study in 150 subjects, 38 with gallstones （cases） and 112 controls. We also did a real-time PCR-RT study in twenty gallbladder samples each. Body mass index （BNI）, serum insulin, ghrelin, and serum lipids were measured. Logistic regression analyses （univariate and multivariate） were conducted to estimate the probability of gallstone disease associated with serum ghrelin concentrations. RESULTS： Cases were statistically different from controls in gender distribution （P = 0.01）, age （53 vs 44 yr, P = 0.002）, BNI （28 vs 25; P = 0.004）, and glucose （5.26 vs 4.98 mmol/L; P = 0.05）. The prevalence of ghrelin serum levels above the third tercile was lower in subjects without metabolic syndrome （P 〈 0.05）. In a multivariate model, we found a protective effect, when ghrelin values were higher than the median value （OR = 0.27, 95%CI 0.09-0.82, P = 0.02）. Twenty （20%） gallbladder specimens expressed ghrelin mRNA. CONCLUSION： Serum ghrelin concentrations are associated with a protective effect of GD.

The rational design of emulsions requires study of the main factors that influence their formation, physicochemical properties and, consequently, stability and performance. The use of vegetable oils in the pharmaceutical and cosmetic industries has recently become attractive. Dipteryx alata Vogel(D. alata) is an oleaginous species native to Brazil. The seeds of this species contain highly unsaturated oil with significant amounts of tocopherols and phytosterols, representing an important source of agents capable of combatting oxidative processes. In this work, a lamellar gel phase emulsion using oil extracted from the seeds of D. alata(baru) was developed. The steps involved in the development of this research were as follows: 1) development of formulations and 2) in vitro assays by simulating the evaporation of the final product after application to the skin and Electron paramagnetic resonance spectroscopy(EPR) of fatty acid spin labels was used to investigate the profile of interaction of the dispersed systems with stratum corneum(SC) lipids. The results indicate that the developed system shows no signs of instability during the storage period. Moreover, EPR studies indicated that D. alata oil and especially the developed formulation were able to increase SC lipid fluidity and extract a fatty-acid spin label from the lipid domain structures of SC, demonstrating its potential to act as a drug or skin care vehicle.
