Comparative study of the efficacy and safety of bromfenac, nepafenac and diclofenac sodium for the prevention of cystoid macular edema after phacoemulsification

AIM： To compare the efficacy, tolerability and safety of bromfenac 0.09%, nepafenac 0.1% or diclofenac 0.1% for the prophylaxis of the cystoid macular edema（CME） after phacoemulsification. METHODS： Group sequential observational comparative study. After phacoemulsification, patients received two months for topical treatment of either diclofenac sodium, bromfenac or nepafenac. All patients received concomitant topical tobramycin 0.3% and topical prednisolone 1%. We measured CME using optical coherence tomography（OCT） central foveal thickness, macular thickness and total macular volume. RESULTS： We enrolled 243 patients from January to June 2015, and 35% received diclofenac, 32.9% bromfenac and 32.1% nepafenac. When we compared pre-operative to three weeks to two months, bromfenac was more effective in reducing foveal volume（21.3 and 35.4 mm3, respectively）, compared with the diclofenac（1.3 and 11.5 mm3, respectively）, and the nepafenac group, became more edematous 6.4 and 5.3, respectively. Totally 133 patients completed the post-surgical satisfaction questionnaire. Patients complained of eye stickiness in 13.8% whom we gave nepafenac, versus 10.3% whom we gave diclofenac sodium, and in 0 whom we gave bromfenac. CONCLUSION： Bromfenac is the best tolerated and is more effective than diclofenac and nepafenac in reducing CME after phacoemulsification.

Questions relating to premenstrual asthma

The study of asthma in fertile women needs to consider its potentially recurrent exacerbation in a specific phase of the menstrual cycle. Premenstrual asthma(PMA)refers to the deterioration of asthma in some women of fertile age during the premenstrual phase. Prevalence varies considerably according to studies(11%-47.44%) mainly because there is no standardized definition of the illness. There is a possible link between PMA and premenstrual syndrome, which is a set of physical and psychic manifestations that occur in some fertile women during the same premenstrual phase. This relation has been widely studied but there are still several unknowns. PMA etiopathogeny is not known. It involves possible causes such as hormonal variations in the premenstrual phase, the coexistence of atopy, variations during the cycle in substances related to inflammation, like LTC4 leukotrienes, catecholamines, E2 and F2α prostaglandins and certain cytokines. Also considered are psychological factors related to this phase of the menstrual cycle, a high susceptibility to infection or increased bronchial hyperreactivity prior to menstruation. Yet no factor fully explains its etiology, consequently no specific treatment exists. Researchers have investigated hormones, antileukotrienes, prostaglandin synthesis inhibitors, diuretics, phytoestrogens and alternative therapies, but none has been shown to be effective.

为了研究交流电场作用下微流体高频驱动机理,以点面电极系统为基础,进行了高于驰豫频率范围的交流电动微流体驱动实验,发现了这一范围内的明显流体流动现象.指出电热效应是点面电极系统中产生高频微流体流动的主要因素,分析了电热效应对点面电极系统中电导率分布的影响.以点面电极系统中的电荷守恒方程、传导与扩散方程、Navier-Stokes方程以及热对流和传导方程为基础,对点面电极系统高频流体驱动过程进行耦合及数值仿真分析,给出了点面电极系统中电场、温度场以及速度场的分布曲线.分析了电压、频率等因素对于高频流体驱动速度的影响,并与实验结果进行了对比分析.结果表明,点面电极系统中,交流电热效应导致的点面电极之间的电导率梯度与电场的综合作用,是该系统高频流体驱动的主要因素,机理分析与实验结果具有较好的一致性.

Fiber optic sensors have a set of properties that make them very attractive in biomechanics. However, they remain unknown to many who work in the field. Some possible causes are scarce information, few research groups using them in a routine basis, and even fewer companies offering turnkey and affordable solutions. Nevertheless, as optical fibers revolutionize the way of carrying data in telecommunications, a similar trend is detectable in the world of sensing. The present review aims to describe the most relevant contributions of fiber sensing in biomechanics since their introduction, from 1960s to the present, focusing on intensity-based configurations. An effort has been made to identify key researchers, research and development （R＆D） groups and main applications.

Adiponectin as a protective factor in hepatic steatosis

AIM: Obesity and insulin resistance (IR) are closely related to hepatic steatosis (HS), and adiponectin is a hepatic insulin sensitizer that has important effects in liver function. This study aims at investigating the relationship between serum adiponectin concentration and the presence of HS. METHODS: We carried out a cross-sectional study in a check-up unit of a University Hospital in Mexico City. We enrolled 196 subjects, comprising 98 subjects with HS (27 women, 71 men) and 98 controls (37 women and 61 men). Anthropometric, metabolic and biochemical variables were measured in the two groups. Serum adiponectin and leptin concentrations were determined, their association with grade of HS tested, and concentrations, according to quartiles, compared between cases and controls. X2 analysis for linear trends was used to test for a dose-response relationship and logistic regression analysis was conducted to test for a protective effect of adiponectin. RESULTS: The HS subjects were older and more obese than controls, with a central obesity pattern. In the fourth quartile of adiponectin concentrations, HS was less common and severe. In a multivariate model of the fourth quartile of the adiponectin concentrations, we observed a protective effect (OR = 0.17, 95%CI: 0.04-0.67, P= 0.01). In subjects with more severe HS, we observed higher leptin concentrations, and caloric intakes, total fat and iron consumption were higher than in controls. CONCLUSION: The results of the present study suggest that a high serum concentration of adiponectin is associated with a protective effect against HS.

Low serum levels of ghrelin are associated with gallstone disease

AIM： To explore the role of ghrelin in gallstone disease. METHODS： We carried out a cross-sectional study in 150 subjects, 38 with gallstones （cases） and 112 controls. We also did a real-time PCR-RT study in twenty gallbladder samples each. Body mass index （BNI）, serum insulin, ghrelin, and serum lipids were measured. Logistic regression analyses （univariate and multivariate） were conducted to estimate the probability of gallstone disease associated with serum ghrelin concentrations. RESULTS： Cases were statistically different from controls in gender distribution （P = 0.01）, age （53 vs 44 yr, P = 0.002）, BNI （28 vs 25; P = 0.004）, and glucose （5.26 vs 4.98 mmol/L; P = 0.05）. The prevalence of ghrelin serum levels above the third tercile was lower in subjects without metabolic syndrome （P 〈 0.05）. In a multivariate model, we found a protective effect, when ghrelin values were higher than the median value （OR = 0.27, 95%CI 0.09-0.82, P = 0.02）. Twenty （20%） gallbladder specimens expressed ghrelin mRNA. CONCLUSION： Serum ghrelin concentrations are associated with a protective effect of GD.

Eco-Efficiency： Environmental Performance vs Economic Performance

The second half of the 20th century was characterized by an increasing interest in the pace of economic growth and its impact on the environment. As a result, the concept of eco-efficiency as a philosophy of management has arisen, which combines the excellence of the environment and business. This paper uses the distribution-free approach to provide empirical evidence in relation to the eco-efficiency paradigm by studying the relation between environmental performance （measured by the level of emissions of air-contaminating substances） and economic performance （approximated by economic efficiency）, in 199 companies from mineral manufacturing industry in Spain between 2004 and 2007. The empirical analysis shows that the results are consistent with the paradigm of eco-efficiency and therefore it can be concluded that being responsible for the environment is positively related to economic efficiency.

Plant growth and nutrient use efficiency of two native Fabaceae species for mineland revegetation in the eastern Amazon
