Management of hepatitis C in patients with chronic kidney disease

Hepatitis C virus(HCV) infection is highly prevalent among chronic kidney disease(CKD) subjects under hemodialysis and in kidney transplantation(KT) recipients, being an important cause of morbidity and mortality in these patients. The vast majority of HCV chronic infections in the hemodialysis setting are currently attributable to nosocomial transmission. Acute and chronic hepatitis C exhibits distinct clinical and laboratorial features, which can impact on managementand treatment decisions. In hemodialysis subjects, acute infections are usually asymptomatic and anicteric; since spontaneous viral clearance is very uncommon in this context, acute infections should be treated as soon as possible. In KT recipients, the occurrence of acute hepatitis C can have a more severe course, with a rapid progression of liver fibrosis. In these patients, it is recommended to use pegylated interferon(PEG-IFN) in combination with ribavirin, with doses adjusted according to estimated glomerular filtration rate. There is no evidence suggesting that chronic hepatitis C exhibits a more aggressive course in CKD subjects under conservative management. In these subjects, indication of treatment with PEG-IFN plus ribavirin relies on the CKD stage, rate of progression of renal dysfunction and the possibility of a preemptive transplant. HCV infection has been associated with both liver disease-related deaths and cardiovascular mortality in hemodialysis patients. Among those individuals, low HCV viral loads and the phenomenon of intermittent HCV viremia are often observed, and sequential HCV RNA monitoring is needed. Despite the poor tolerability and suboptimal efficacy of antiviral therapy in CKD patients, many patients can achieve sustained virological response, which improve patient and graft outcomes. Hepatitis C eradication before KT theoretically improves survival and reduces the occurrence of chronic graft nephropathy, de novo glomerulonephritis and post-transplant diabetes mellitus.

AIM:To evaluate the overlap of autoimmune hepatitis in hepatitis C virus(HCV)-infected patients with intense interface ***:Among 1759 patients with hepatitis C submitted to liver biopsy,92(5.2%) presented intense interface *** patients were evaluated regarding the presence of antinuclear antibody(ANA),anti-smooth muscle antibody(SMA) and anti-liver/kidney microsomal antibody(LKM-1),levels of γ-globulin and histological findings related to autoimmune hepatitis(plasma cell infiltrate and presence of rosettes).RESULTS:Among patients with hepatitis C and intense interface hepatitis there was a low prevalence of autoantibodies(ANA=12%,SMA=5%,LKM-1=0%) and the median γ-globulin level was within the normal *** histological findings of autoimmune disease were observed in only two cases(2%).After applying the score for diagnosis of autoimmune hepatitis,only one patient was classified with a definitive diagnosis of autoimmune *** overlap with autoimmune hepatitis was not the explanation for the intense necroinflammatory activity in patients with chronic hepatitis C we sought to identify the variables associated with this *** presence of intense interface hepatitis was associated with more advanced age,both at the time of infection and at the time of the biopsy,and higher prevalence of blood transfusion and alcohol ***:Although possible,overlap with autoimmune hepatitis is a very rare association in HCV-infected patients with intense interface hepatitis,an unusual presentation which seems to be related to other host variables.

The aims of this study were:i)to compare no-till areas in two municipalities located in different regions of brazil,along with the influence on CO_(2)Flux and GPP,and ii)to verify the difference between environmental factors followed by the trends of these variables regarding future scenarios(ARIMA time-series model number).The study was carried out in two areas with different latitudes in the municipalities of Sinop-MT and Passo Fundo-RS,both in brazil.A time series of 19 years was performed with data acquired by remote sensing from the following satellites:i)Landsat-8(OLI and TIRS),and ii)TERRA/AQUA(MODIS).The results propound that the spectro-temporal variables are directly influenced by soil management and agricultural practices over the observation time,with a satisfactory correlation in future predictions of the variables for the next ten years,in which presented that the variation of GPP and albedo values for the two study sites would gradually increase until 2028 and the temperature remained constant between the range of its seasonality,and CO_(2)Flux tends to decrease in its seasonality,indicating a higher CO_(2) absorption.

Perovskites as host structures of cations were used in order to generate in situ active and stable catalysts for ethanol steam reforming. For this purpose,La_(1-x)Mg_xAl_(1-y)Ni_yO_3(x = 0.1; y = 0,0.1,0.2,0.3) perovskites were synthetized by the citrate *** segregation is evident for a substitution level higher than 0. 2. The segregation of Ni as NiO generated species interacts with different metal-support after the reduction step. The y = 0.1 catalyst presents the highest H_2 yield value about 85% during reaction time,with low mean values of CH_4 and CO selectivities of 3.4% and 11%,respectively and a low carbon formation. The better performance of y = 0.1 catalyst could be attributed to the minor proportion of segregated phases,thus a controlled expulsion of Ni is successfully reached.

AIM： To evaluated the therapeutic and prophylactic effect of thalidomide on 2, 4, 6-trinitrobenzene sulfonic acid （TNbS）-induced colitis. Thalidomide has been reported to downregulate the expression of tumor necrosis factor α （TNF-α）, IL-12, and vascular endothelial growth factor （VEGF）, hallmarks of intestinal inflammation in Crohn＇s disease （CD）. METHODS： Male Wistar rats were divided in five groups of ten animals each. Four groups received a rectal infusion of TNbS in ethanol. The first group was sacrificed 7 d after colitis induction. The second and third groups received either thalidomide or placebo by gavage and were sacrificed at 14 d. The fourth group received thalidomide 6 h before TNbS administration, and was sacrificed 7 d after induction. The fifth group acted as the control group and colitis was not induced. Histological inflammatory scores of the colon were performed and lamina propria CD4＋ T cells, macrophages, and VEGF＋ cells were detected by immunohistochemistry. TNF-α and IL-12 were quantified in the supernatant of organ cultures by ELISA. RESULTS： Significant reduction in the inflammatory score and in the percentage of VEGF＋ cells was observed in the group treated with thalidomide compared with animals not treated with thalidomide. both TNF-α and IL-12 levels were significantly reduced among TNbS induced colitis animals treated with thalidomide compared with animals that did not receive thalidomide. TNF-α levels were also significantly reduced among the animals receiving thalidomide prophylaxis compared with untreated animals with TNbS-induced colitis. Intestinal levels of TNF-α and IL-12 were significantly correlated with the inflammatory score and the number of VEGF＋ cells. CONCLUSION： Thalidomide significantly attenuates TNbS-induced colitis by inhibiting the intestinal production of TNF-α, IL-12, and VEGF. This effect may support the use of thalidomide as an alternate approach in selected patients with CD.

This research aimed to analyze the possibility to estimate and automatically map large areas of soybean cultivation through the use of MODIS(Moderate-Resolution Imaging Spectroradiometer)*** major techniques were used:GEOgraphic-Object-based Image Analysis(GEObIA)and Data Mining(DM).In order to obtain the images,the segmentation algorithm implemented by Definiens Developer was used.A decision tree(DT)was created from a training set previously ***-series of images from the MODIS sensor aboard the Terra satellite were acquired in order to represent the wide variation of the vegetation pattern along the soybean crop *** time-series data were used only for the CEI ***,to compare the results obtained from GEObIA,the slicing technique was used at the CEI *** the training,the DT was applied to the vegetation indices generating the thematic map of the spatial distribution of *** accordance with the error matrix and kappa parameter analysis,tests for statistical significance were *** indicate that the classification achieved by Kappa coefficients is *** short,the obtained results proved that combining vegetation indices and time-series data using GEObIA return promising results for mapping soybean plantation on a regional scale.
