Serum SCCA-IgM as a predictor of hepatocellular carcinoma in patients with liver cirrhosis

Aberrant Squamous Cell Carcinoma Antigen (SCCA) expression is an early hepatocarcinogenetic event and circulating SCCA-IgM complexes are elevated in most HCC patients. We evaluated whether serum SCCA-IgM levels can identify HCV +ve cirrhotic patients at low HCC risk. In this retrospective study we enrolled 29 cirrhotic patients in whom serum SCCA-IgM was measured 8 - 69 months (median 31) before HCC diagnosis, and 28 cirrhotic patients who remained HCC- free, with SCCA-IgM measured 15 - 68 months (median 48) before the study end. The best discriminating value of SCCA-IgM was calculated and tested in predicting HCC diagnosis within 12, 24 and 36 months. Sensitivity analysis, considering different HCC incidence, was conducted to identify the patient subgroup with an annual cancer risk below the threshold of a cost-effective semiannual surveillance with ultrasound. Cumulative HCC incidence at 12, 24 and 36 months was 7.0%, 15.7% and 26.3%, respectively. SCCA-IgM levels were higher in HCC than in cirrhotic patients [median: 381 (95% C.I.: 50 - 5289) vs. 100 (70 - 493) AU/mL, P = 0.005]. The SCCA-IgM value ≤ 200 AU/mL accurately identified patients at low risk of HCC development in the subsequent year (sensitivity 75%, specificity 62%, positive predictive value 13% and negative predictive value 97%). Considering an annual HCC incidence ≤ 3%, patients with SCCA-IgM ≤ 200 AU/mL (60% of the whole patients) had an HCC risk below the accepted threshold of a cost-effective surveillance (1.5%). In conclusion, provided that our provocative results are confirmed in larger studies, SCCA-IgM serum measurement could permit implementation of a two step (with different costs) surveillance: an initial serological surveillance, based on the annual monitoring of this biomarker, and the conventional surveillance by semiannual US when SCCA-IgM becomes >200 AU/mL. This could improve the cost/effectiveness of surveillance of HCV infected patients at risk of HCC.

Improving the Hydromechanical Deep-Drawing Process Using Aluminum Tailored Heat Treated Blanks

The present work demonstrates the effectiveness of combining the hydromechanical deep-drawing process with the Tailored Heat Treated Blank（THTB） technique. In the hydromechanical deep-drawing process, the fluid pressure is used for postponing the fracture occurrence in the blank, while the THTB technique allows to create a material property gradient through a suitable artificial aging treatment carried out prior to the forming process. Since the number of process variables is large, in the present work the authors propose an optimization loop for the determination of the parameters controlling the extension of the blank regions to be subjected to the aging treatment and the temperature levels to be set during the heat treatment. The proposed methodology couples a simple finite element model（Abaqus） with a multiobjective optimization platform（mode FRONTIER）. A preliminary experimental campaign was carried out for determining the effect of the aging treatment on the mechanical（through tensile tests） and deformative（through formability tests）behavior of the AC170 PX aluminum alloy. Optimization results prove the effectiveness of the adopted methodology and put in evidence that the adoption of properly aged blanks in the hydromechanical deep drawing allows to increase the limit drawing ratio and to simplify the process since it is conducted at room temperature.

Surgery in biliary lithiasis: from the traditional “open” approach to laparoscopy and the“rendezvous” technique

BACKGROUND:According to the current literature, biliary lithiasis is a worldwide-diffused condition that affects almost 20% of the general population. The rate of common bile duct stones(CBDS) in patients with symptomatic cholelithiasis is estimated to be 10% to 33%, depending on patient’s *** to stones in the gallbladder, the natural history of secondary CBDS is still not completely understood. It is not clear whether an asymptomatic choledocholithiasis requires treatment or not. For many years, open cholecystectomy with choledochotomy and/or surgical sphincterotomy and cleaning of the bile duct were the gold standard to treat both pathologies. Development of both endoscopic retrograde cholangiopancreatography(ERCP) and laparoscopic surgery,together with improvements in diagnostic procedures, influenced new approaches to the management of CBDS in association with *** SOURCES:We decided to systematically review the literature in order to identify all the current therapeutic options for CBDS. A systematic literature search was performed independently by two authors using Pub Med, EMBASE, Scopus and the Cochrane Library ***:The therapeutic approach nowadays varies greatly according to the availability of experience and expertise in each center, and includes open or laparoscopic common bile duct exploration, various combinations of laparoscopic cholecystectomy and ERCP and combined laparoendoscopic ***:Although ERCP followed by laparoscopic cholecystectomy is currently preferred in the majority of hospitals worldwide, the optimal treatment for concomitant gallstones and CBDS is still under debate, and greatly varies among different centers.

Transformation of leather waste into microwave-activated and N-doped carbon for Li-S batteries

The use of carbon from waste biomass has the potential to buffer the drawbacks of Li-S batteries and improve their overall performance. Chrome-Tanned-LeatherShavings(CTLS) are undesirable wastes, highly available for transformation into porous carbon. Microwave activation is an efficient state-of-the-art procedure to obtain unique porous structures and nitrogen incorporation can improve the performance of porous carbons. This study proposes the transformation of CTLS into a microwave activated porous carbon, and then was modified to introduce nitrogen. A microporous activated carbon with a specific surface area of 556 m2g-1 with honeycomb-like structure was obtained. Both urea thermal decomposition and ethanolamine impregnation improves the nitrogen and amine presence of surface groups. However,the results indicated that only urea treatment was effective in improving the initial capacity of the cell(1363 mAhg-1), which is probably linked with the sorption of long-chain polysulfides. This investigation confirms that it is possible to obtain carbon materials from CTLS, for use as sulfur-host cathodes in Li-S batteries, and improve their performance with urea thermal decomposition. Furthermore, a Radial basis function neural network was fitted to provide statistical support to the experimental results. The trained model corroborated the importance of nitrogen content of the carbons in the discharge capacity of the cells.

Unexplainable development of a hydatid cyst

Echinococcosis is a cyclozoonosis characterized by cystic lesions usually situated inside or outside the liver. We discuss the case of a 77-year-old woman with a recurrent hydatidosis with a cyst arising from the liver, growing through the lateral right abdomen wall, and reaching the subcutaneous tissue of the lumbar region. In the literature, rare subcutaneous or muscular Iocalizations of hydatid cysts are described, however, there is no mention of a cyst growing over the abdominal wall muscles, shaped like an hourglass, partially in the liver and partially in the subcutaneous tissue, as in our case. We have not found any pathogenetic explanation for this growth pattern which is not typical of the biological behaviour of a hydatid cyst.

Effect of a Hybrid Zinc Stearate-Silver System on the Properties of Polylactide and Its Abiotic and the Biotic Degradation and Antimicrobial Activity Thereof

This work investigates the degradation and properties of a thermoplastically prepared composite comprising a polylactide/ hybrid zinc stearate-silver system. The influence of the zinc stearate-silver system on the properties of the composite is investigated by electron microscopy, differential scanning calorimetry and tensile tests. Furthermore, the antimicrobial activities of the systems are examined. The degradation behaviour of the composites is studied in both abiotic and biotic （composting） environments at an elevated temperature of 58 ℃. The results reveal good dispersion of the additive in the PLA matrix, a stabilizing effect exerted by the same on the polylactide matrix during processing, and slight reduction in glass transition temperature. The zinc stearate-silver component also reduces brittleness and extends elongation of the composite. Abiotic hydrolysis is not significantly affected, which is in contrast with pure PLA, although mineralization during the early stage of biodegradation increases noticeably. The composite exhibits antimicrobial activity, even at the lowest dosage of the zinc stearate/silver component （1 wt%）. Moreover, Ag and Zn contents were found to be present in the composite during abiotic hydrolysis, which was demonstrated by minimal diffusion of Ag ions from the matrix and very extensive washing of compounds that contained Zn.

Non-polypoid colorectal neoplasms:Classification,therapy and follow-up

In the last years,an increasing interest has been raised on non-polypoid colorectal tumors(NPT) and in particular on large flat neoplastic lesions beyond 10 mm tending to grow laterally,called laterally spreading tumors(LST).LSTs and large sessile polyps have a greater frequency of high-grade dysplasia and local invasiveness as compared to pedunculated lesions of the same size and usually represent a technical challenge for the endoscopist in terms of either diagnosis and *** to the Paris classification,NPTs are distinguished in slightly elevated(0-Ⅱa,less than 2.5 mm),flat(0-Ⅱb) or slightly depressed(0-Ⅱc).NPTs are usually flat or slightly elevated and tend to spread laterally while in case of depressed lesions,cell proliferation growth progresses in depth in the colonic wall,thus leading to an increased risk of submucosal invasion(SMI) even for smaller *** may be frequently missed by inexperienced endoscopists,thus a careful training and precise assessment of all suspected mucosal areas should be *** or,if possible,narrow-band imaging technique should be considered for the estimation of SMI risk of NPTs,and the characterization of pit pattern and vascular pattern may be useful to predict the risk of SMI and,therefore,to guide the therapeutic *** suitable to endoscopic resection are those confined to the mucosa(or superficial layer of submucosa in selected cases) whereas deeper invasion makes endoscopic therapy *** mucosal resection(EMR,piecemeal for LSTs > 20 mm,en bloc for smaller neoplasms) remains the first-line therapy for NPTs,whereas endoscopic submucosal dissection in high-volume centers or surgery should be considered for large LSTs for which en bloc resection is mandatory and cannot be achieved by means of *** piecemeal EMR,follow-up colonoscopy should be performed at 3 mo to assess resection *** case of en bloc resection,surveillance colonoscopy should be sc

Development and validation of a risk score for advanced colorectal adenoma recurrence after endoscopic resection

AIM: To develop and validate a risk score for advanced colorectal adenoma(ACA) recurrence after endoscopic ***: Out of 3360 patients who underwent colon polypectomy at University of Foggia between 2004 and 2008, data of 843 patients with 1155 ACAs was retrospectively reviewed. Surveillance intervals were scheduled by guidelines at 3 years and primary endpoint was considered 3-year ACA recurrence. Baseline clinical parameters and the main features of ACAs were entered into a Cox regression analysis and variables with P didate variables into a stepwise Cox regression model(conditional backward selection). The regression coefficients of the Cox regression model were multiplied by 2 and rounded in order to obtain easy to use point numbers facilitating the calculation of the score. To avoid overoptimistic results due to model fitting and evaluation in the same dataset, we performed an internal 10-fold cross-validation by means of bootstrap sampling. RESULTS: Median lesion size was 16 mm(12-23) while median number of adenomas was 2.5(1-3), whereof the number of ACAs was 1.5(1-2). At 3 years after polypectomy, recurrence was observed in 229 ACAs(19.8%), of which 157(13.5%) were metachronous neoplasms and 72(6.2%) local recurrences. Multivariate analysis, after exclusion of the variable "type of resection" due to its collinearity with other predictive factors, confirmed lesion size, number of ACAs and grade of dysplasia as significantly associated to the primary outcome. The score was then built by multiplying the regression coefficients times 2 and the cut-off point 5 was selected by means of a Receiver Operating Characteristic curve analysis. In particular, 248 patients with 365 ACAs fell in the higher-risk group(score ≥ 5) where 3-year recurrence was detected in 174 ACAs(47.6%) whereas the remaining 595 patients with 690 ACAs were included in the low-risk group(score < 5) where 3-year recurrence rat
