Antiviral therapy in hepatitis c virus cirrhotic patients in compensated and decompensated condition

The main goals of treating cirrhotic patients with antiviral therapy are to attain sustained viral clearance(SVR),halt disease progression,and prevent re-infection of the liver ***,while the medical need is great,the use of interferon and ribavirin might expose these patients to severe treated-related side effects as a large proportion of them have pre-existing hematological *** have reviewed potential benefits and risks associated with antiviral drugs in patients with liver cirrhosis,due to hepatitis c virus(HcV) *** cases presenting with bridging fibrosis or cirrhosis,current regimens of antiviral therapy have attained a 44%-48% rate of *** cirrhotic patients with portal hypertension,the SVR rate was 22% overall,12.5% in patients with genotype 1,and 66.7% in those with genotypes 2 and 3 following therapy with low doses of either Peg-IFN alpha-2b and of *** patients with decompensated cirrhosis,full dosages of Peg-IFN alpha-2b and of ribavirin produced a SVR rate of 35% overall,16% in patients with genotype 1 and 4,and 59% in those with genotype 2 and *** of hematological cytokines will either ensure full course of treatment to be accomplished with and prevent development of treatment-associated side *** benefits after HcV eradication were partial recovery of liver metabolic activity,prevention of hepatitis c recurrence after transplantation,and removal of some patients from the waiting list for liver *** observations highlighted that therapy is inadvisable for individuals with poor hepatic reserve(child-Pugh-Turcotte score ≥ 10).Although SVR rates are low indecompensated cirrhotics due to hepatitis c,these patients have the most to gain as successful antiviral therapy is potentially lifesaving.

In china,flash floods are one of the main natural disasters causing loss of life and damage to *** threat of flash floods is exacerbated with climate change and increased human activities,such that the number of disasters has shown a clear upward trend in recent ***,due to the scarcity of instrumental data or overly short timeseries,we are still lacking critical data to understand spatio-temporal patterns and driving factors of extreme flash *** missing knowledge is however crucial for a proper management of these hazards,especially in remote mountain *** forested catchments,dendrogeomorphology allows the reconstruction of past process activity based on growth disturbances(GDs)in trees that have been affected by past flash ***,in our study,for the first time,we reconstruct past flash floods in the Qilian Mountains,northeast Tibetan Plateau,over past *** this end,we sampled 99 Qinghai spruce(Picea crassifolia)trees affected by flash floods,with a total of 194increment cores,and identified 302 GDs induced by past flash *** GDs have been caused by at least 21 flash floods that we are able to reconstruct over the last 170 *** position of GDs within tree rings and the intra-seasonal dating of past events also allowed discussion of the likely synoptic situations that may have led to the triggering of flash floods in the *** regression analysis confirms that significant correlation exists between cumulative maximum 5-day August-September precipitation and reconstructed flash floods,which is corresponding to the majority of scars and related tangential rows of traumatic resin ducts(TRDs)found in the latewood portion of growth *** results support the idea that abundant precipitation occurring at the end of the summer season and early fall is the key factor driving flash floods in our study *** research not only fills the gaps regarding historical flash flood histories in

Application of Optical Fiber in Radiation Detection Systems

In the last years, the production of optical fibers cables has made possible the development of a range of spectroscopic probes for in situ analysis performing beyond nondestructive tests, environmental monitoring, security investigation, application in radiotherapy for dose monitoring, verification and validation. In this work, a system using an optical fiber cable to electromagnetic signal transmission from a NaI（TI） radiation detector is presented. The innovative device takes advantage mainly of the optical fibers large passband, small signal attenuation and immunity to electromagnetic interference to application for radiation detection systems. The main aim was to simplify the detection system making it to reach areas where the conventional device cannot access due to its lack of mobility and external dimensions. Some tests with this innovative system are presented and the results stimulate the continuity of the researches.

We revise the notion of von Neumann regularity in JB^＊-triples by finding a new characterisation in terms of the range of the quadratic operator Q（a）. We introduce the quadratic conorm of an element a in a JB^＊-triple as the minimum reduced modulus of the mapping Q（a）. It is shown that the quadratic conorm of a coincides with the infimum of the squares of the points in the triple spectrum of a. It is established that a contractive bijection between JBW^＊-triples is a triple isomorphism if, and only if, it preserves quadratic conorms. The continuity of the quadratic conorm and the generalized inverse are discussed. Some applications to c^＊-algebras and von Neumann algebras are also studied.

The discovery and realization of topological insulators,a phase of matter which hosts metallic boundary states when the ddimension insulating bulk is confined to(d−1)-dimensions,led to several potential ***,it was shown that protected topological states can manifest in(d−2)-dimensions,such as hinge and corner states for three-and two-dimensional systems,*** nontrivial materials are named higher-order topological insulators(HOTIs).Here we show a connection between spin Hall effect and HOTIs using a combination of ab initio calculations and tight-binding *** model demonstrates how a non-zero bulk midgap spin Hall conductivity(SHc)emerges within the HOTI *** this,we performed high-throughput density functional theory calculations to find unknown HOTIs,using the SHc as a *** calculated the SHc of 693 insulators resulting in seven stable two-dimensional *** work guides novel experimental and theoretical advances towards higher-order topological insulator realization and applications.

Hepatitis c virus and metabolic disorder interactions towards liver damage and atherosclerosis

Hepatitis c virus (HcV) is one of the main causes of liver disease worldwide, and alterations of glucose metabolism have reached pandemic proportions in western countries. However, the frequent coexistence between these two conditions is more than simply coincidental, since HcV can induce insulin resistance through several mechanisms. Indeed, the virus interferes with insulin signaling both directly and indirectly, inducing the production of pro-inflammatory cytokines. Furthermore, the entire viral life cycle has strict interconnections with lipid metabolism, and HcV is responsible for a “viral” steatosis which is frequently superimposed to a “metabolic” one. Several evidences suggest that HcV-induced metabolic disorders contribute both to the evolution of liver fibrosis and, likely, to the progression of the other disorders which are typically associated with altered metabolism, in particular atherosclerosis. In the present review, we will examine in depth the links between HcV infection and insulin resistance, liver steatosis and diabetes, and analyze the impact of these interactions on the progression of liver fibrosis and atherosclerosis. Special attention will be focused on the highly debated topic of the relationship between HcV infection and cardiovascular disease. The available clinical literature on this item will be broadly reviewed and all the mechanisms possibly implied will be discussed.
