Prognostic factors for recurrence of gastrointestinal bleeding due to Dieulafoy's lesion

AIM： To analyze the effectiveness of the endoscopic therapy and to identify prognostic factors for recurrent bleeding. METHoDS： Retrospective study of patients with gas- trointestinal bleeding secondary to Dieulafoy＇s lesion （DL） from 2005 to 2011. We analyzed the demographic characteristics of the patients, risk factors for gastro- intestinal bleeding, endoscopic findings, characteristics of the endoscopic treatment, and the recurrence of bleeding. We included cases in which endoscopy de- scribed a lesion compatible with Dieulafoy. We exclud- ed patients who had potentially bleeding lesions such as angiodysplasia in other areas or had undergone other gastrointestinal endoscopic procedures. RESULTS： Twenty-nine patients with DL were identi- fied. Most of them were men with an average age of 71.5 years. Fifty-five percent of the patients received antiaggregatory or anticoagulant therapy. The most common location for DL was the stomach （51.7%）. The main type of bleeding was oozing in 65.5% of cases. In 27.6% of cases, there was arterial （spurting） bleeding, and 6.9% of the patients presented with an adherent clot. A single endoscopic treatment was ap- plied to nine patients （31%）, eight of them with adren- aline and one with argon, while 69% of the patients received combined treatment. Six patients （20.7%） presented with recurrent bleeding at a median of 4 d after endoscopy （interquartile range = 97.75）. Within these six patients, the new endoscopic treatment ob- tained a therapeutic success of 100%. The presence of arterial bleeding at endoscopy was associated with a higher recurrence rate for bleeding （50% vs 33.3% for other type of bleeding） rP = 0.024, odds ratio （oR） = 8.5, 95% CI = 1.13-63.87]. The use of combined en- doscopic treatment prevented the recurrence of bleed- ing （10% vs 44.4% of single treatment）（P = 0.034, oR = 0.14, 95% CI = 0.19-0.99）. The slope soil analysis remains a corporate concern in construction activities. Because of its significance, the evaluation of slope soil stability has called widespread attention to several researchers all over the world. Many methods have been technologically advanced to evaluate the stability of slopes soils founded on distinct expectations and circumstances. Every method has specific benefits and limits. This work makes a comparison among safety factors and slip surfaces of slopes soils based on using Limit Equilibrium and Finite Element methods. Therefore, SLIDE 6.0 and PLAXIS 8.0 software were used for Limit Equilibrium and Finite Element methods, respectively. The computations of safety factors were performed for diverse shapes of slopes including different types of soils. Failure surfaces and values of safety factors obtained were compared for both methods used. It was noticed that the safety factors obtained from Limit Equilibrium methods were larger than of which is obtained by the finite element code. Moreover, an important change is noticed between the slip surfaces obtained by using both approaches.

Isotopic Variations of oxygen (δ^(18)o) in Benthic Foraminifera under Antiestuarine Conditions in the Colorado River Delta

Benthic foraminifera are excellent environmental indicators;CaCo3 test records the isotopic concentration of their surroundings and can be used to analyze environmental changes that occur during a certain time period. Stable isotopes, particularly those of oxygen (δ18o), are useful for interpreting ancient environments, given that they are used as “proxy” environmental variables (temperature and salinity). In this study, we provide ranges of isotopic variation in benthic foraminifera communities from the Colorado River delta. Four sampling campaigns were conducted in one year (2009-2010) in the adjacent subtidal zone of Baja California. Four transects with a total of sixteen sample station were drawn perpendicular to the coastline. Here, we recorded the following in situ: sand-type, salinity, and temperature. In the laboratory, 300 individuals per sample were separated, and sub-samples were taken for isotopic analysis. Data was processed using the software’s R 2.12.2, PAST 1.81 and Arc Map 9.3. Forty species were identified in the dead assemblages (Thanatocoenosis), whereas thirteen species were found in the living assemblages (Biocoenosis). The most abundant species in both communities belong to the genera Ammonia and Cribroelphidium. In the living assemblages, isotopic variation −2.15% to 5.94% within a temperature interval of 11°C, indicated antiestuarine conditions. In the dead assemblages, isotopic composition −3.04 to −0.74 served as a sign of estuarine conditions prior to damming.
