IgA Nephropathy: Are We Doing Enough?

Immunoglobulin A nephropathy (IgAN) was first identified and described as a disease by Berger and Hinglais in 1968. It is the most common primary glomerulopathy worldwide [1], most prevalent in East Asians and Caucasians and rare in black individuals. There are four key elements that contribute to IgAN, which determine the severity, course, and prognosis of the disease: circulating IgA immunocomplexes that favor mesangial deposition, the efficiency of the reticuloendothelial system, mesangial cell affinity and reaction to mesangial accumulation of poorly glycosylated IgA1, and the renal tendency to glomerulosclerosis and interstitial fibrosis. Clinical manifestations among patients with IgAN include hematuria, approximately 40% to 50% of cases present with one or more episodes of hematuria, usually preceded by upper respiratory tract infections. Between 30% and 40% present with hematuria and non-nephrotic range proteinuria that may be associated with arterial hypertension and impaired renal function. The TESTING study reveal a significant decrease in outcomes such as the risk of a 40% decrease in glomerular filtration rate or the need for renal replacement therapy in the group treated with steroids. The decrease in renal function compared to the group treated in the previously mentioned STOP-IgAN trial was 4 times less than in the TESTING study. Are we doing enough? Obviously, more trials are required with the use of adequate nephroprotection measures. We present 3 patients with a diagnosis of IgA nephropathy who attend the follow-up consultation and voluntarily decide to take part in the review.

Introduction: Metabolic acidosis (MA) is a frequent alteration in chronic kidney disease (CKD) that is associated with numerous complications, which is why its correction is recommended. Oral sodium bicarbonate is currently the treatment of choice. Objective: The objective is to determine if venous bicarbonate is equal to arterial bicarbonate in the follow-up of a patient with chronic kidney disease. Materials Methods: Single-center Cross-sectional studies in a cohort of adult patients with stage 4 - 5 CKD. Samples were taken between January 2022 and January 2023, in a Clinic in the city of Ibague/ Colombia obtained from the radial artery. The inclusion criteria were: not being treated with alkaline at the time of inclusion. Results: A total of 71 patients were included, 73.2% male (52) and 26.8% female (19), with different stages: stage 3 with 5.6% (4), stage 4 with 60.6% (43), stage 5 with 33.8% (23). 66.2% were diabetic, 88.7% had arterial hypertension, and 15.5% of the patients presented hematoma as a complication and pain associated with arterial puncture. The result of mean venous bicarbonate was 18.8 with a standard deviation of 2.3, arterial bicarbonate a mean of 19.4 with a standard deviation of 2.1 with a value of P 0.46, venous pH with a mean of 7.37 with a standard deviation of 0.48 and a mean arterial pH of 7.38 with a standard deviation of 0.48 with a P value of 0.01. Values of venous bicarbonate compared to arterial bicarbonate showed no statistically significant difference in patients with chronic kidney disease, but there were more complications such as hematoma and pain in patients in the arterial puncture cohort, because of this result venous bicarbonate corresponds to arterial bicarbonate, but has less risk of complications associated with the procedure. Graphene/Ni（110） has been studied by time-resolved X-ray photoelectron spectroscopy and near-edge X-ray absorption fine structure. The C ls core level shows a splitting typical of periodically rippled interfaces. The analysis of the C K-edge reveals that the interface states previously observed for graphene/Ni（111） are suppressed in graphene/Ni（110）. This suppression is due to the reduced hybridization of the Dirac-cone electrons in graphene with the d-bands of the （110）-oriented nickel contacts. Our results show that, contrary to commensurate growth of graphene on Ni（111）, epitaxially grown graphene on Ni（110） behaves as a quasi-freestanding sheet, as the lattice mismatch gives rise to a moir6 reconstruction.

The impact of forest microhabitats on physiochemical properties of the soil and that of microbial communities on tropical soils remain poorly *** elucidate the effect of tropical forest stand on leaf litter and soil microbial communities,we studied enzyme activities,microbial biomass,and diversity in three distinct microhabitats in terms of plant richness,diameter at breast height(DBH),and physiochemical properties of soil and litter,each associated with a different Vanilla *** the soil,positive correlations were found between electrical conductivity(EC)and total organic carbon(TOC)with phosphatase activity,and between nitrogen(N)and water-soluble carbon(WSC)content with urease activity(UA).In the litter,the water content was positively correlated with bacterial and fungal biomass,and N and WSC contents were positively correlated with fungal *** correlations were found between plant richness and UA in the soil,plant richness and fungal biomass in the soil and litter,and DBH and fungal biomass in the *** sequencing revealed differences between microhabitats in the relative abundance of some fungal and bacterial taxa and in the bacterial community composition of both litter and *** richness and diversity were different between microhabitats,and,in litter samples,they were negatively correlated with DBH and plant richness,*** contrast,none of the soil and litter physiochemical properties were significantly correlated with microbial *** results show that significant shifts in enzyme activity,microbial biomass,and diversity in the microhabitats were driven by key abiotic and biotic factors depending on the soil or litter sample type.

By changing the control parameters, many physical systems reach a slow dynamics regime followed by an arrested or a quasiarrested state. Examples, among others, are gels and glasses. In this paper, we discuss some experimental and theoretical results in polymer and colloidal systems, where gel and glass transitions interfere, and use models from Mode Coupling Theory（MCT）to illustrate the rich phenomenology observed. The continuous and the discontinuous transition lines, found in the MCT models,are considered suitable to describe respectively the gel and the glass transitions, so we suggest that the interplay between gel and glass may be interpreted in terms of the F13 MCT model, clarifying also the origin of logarithmic decays often observed in such systems. In particular, the theoretical predictions of the MCT in the F13 model are compared with Molecular Dynamics simulations in model systems for chemical gels and charged colloids.

Radiative cooling has proven to be a useful tool to address the problems of lack of comfort and excessive energy consumption in situations of high temperatures,overheating and heat ***,incorporating courtyards in warm climate zones has been found to be highly beneficial in addressing similar ***,there is interest in analyzing the combined effects of both:radiative cooling and *** paper presents an analysis of the impact of the application of radiative cooling on a courtyard using a comprehensive simulation approach that includes a CFD model for the thermodynamic airflow in the adjacent roofs and inside the courtyard,equations for the transient heat conduction through roofs,walls and courtyard slabs,and a hybrid raytracing-radiosity model for the evaluation of the solar radiation reaching the building surfaces and its reflections,both of specular and diffuse origin,and for the calculation of the thermal radiation exchange,especially with the *** results show that in the hot season,the courtyard with radiative cooling always provides lower temperatures than the initial courtyard does,with a temperature range of 18.33℃to 33.78℃,compared to a range of 19.32℃to 38.00℃in the initial courtyard,and producing a greater difference with outdoor temperatures that can reach 12℃versus 8℃for the reference *** addition,it was found that the courtyard with radiative cooling is able to significantly reduce the observed nighttime overheating by providing lower temperatures than the outdoor temperatures in the 50%of the nights *** was also found that the thermal loads to achieve indoor thermal comfort in the spaces adjacent to the courtyard were reduced by 63.46%to 69.85%.
