Heart rate variability to assess ventilatory thresholds in professional basketball players

Purpose: The aim of this study was to determine if heart rate variability(HRV) during incremental test could be used to estimate ventilatory threshold(VT) in professional basketball players, with sufficient precision to be used in their training. Furthermore, the second aim was to analyse the association between HRV and 3 methods of VT determination by gas analysis.Methods: Twenty-four professional basketball players(age: 23.4 ± 4.9 years; height: 195.4 ± 9.8 cm; body mass: 92.2 ± 11.9 kg) performed an incremental running test to exhaustion. First ventilatory threshold(VT1) was determined by ventilatory equivalent(VE) and HRV and second ventilatory threshold(VT2) was determined by 3 methods of gases analysis(V-slope, VE and gas exchange ratio(R), and HRV). Pearson's coefficient(r) was used to detect differences between data and the strength of each relationship. The mean of absolute differences and Bland–Altman analysis were used to evaluate whether there was agreement.Results: The results showed no significant differences in HR and oxygen consumption(VO2) at VT1 between the 2 methods. Furthermore, no significant differences among the methods of gases analysis and HRV were observed in speed, HR, and VO2 at VT2. Moreover, VTs estimated using HRV and gas methods were significantly correlated. Correlation in HR values was higher between R and HRV(r = 0.96) and VE and HRV(r = 0.96) than V-slope and HRV(r = 0.90).Conclusion: These findings provide a practical, inexpensive approach for evaluating specific training loads when determining VT2 in basketball players. Therefore, HRV is an alternative method to determine VT2 without the application of expensive technology that limits its use to laboratories.

Efficacy of neoadjuvant therapy and surgical rescue for locally advanced hepatoblastomas:10 year single-center experience and literature review

AIM:To report our experience with long-term outcomes after multimodal management therapy.METHODS:An observational retrospective study was performed containing seven patients with hepatoblastoma(Hbl)treated in our institution,a tertiary referral center,from 2003 to 2011.Demographic,preoperative,surgical,and outcome variables were collected.A survival analysis and a review of the current literature related to combination neoadjuvant chemotherapy and surgical resection on Hbl were performed.RESULTS:The median age at surgery was 14.4 mo,with a male to female ratio of 4:3.Pretext staging at diagnosis was as follows:stageⅠ,4 cases;stageⅡ,2 patients;and stageⅢ,1 case.Mean pretreatment tumor volume was 735 cm3.Five out of seven patients received neoadjuvant chemotherapy according to SIOPEL-3 or SIOPEL-6 protocols.Tumor volume and alphafetoprotein levels significantly dropped after neoadjuvant therapy.Surgical procedures performed included hemihepatectomies,segmentectomies and atypical resection.All patients received chemotherapy after surgery.Median postoperative hospital stay was 8 d.All patients were alive and disease-free after a median follow-up period of 23 mo.With regards to the literature review,seventeen articles were found that were related to our search.CONCLUSION:Our series shows how multimodal management of Hbl,exhaustive control and a meticulous surgical approach leads to almost 100%complete resection with optimal postoperative results.

Duodenal diverticula are a relatively common condition. They are asymptomatic, unless they become complicated, with perforation being the rarest but most severe complication. Surgical treatment is the most frequently performed approach. We report the case of a patient with a perforated duodenal diverticulum, which was diagnosed early and treated conservatively with antibiotics and percutaneous drainage of secondary retroperitoneal abscesses. We suggest this method could be an acceptable option for the management of similar cases, provided that the patient is in good general condition and without septic signs.

Tars and alkali ashes from biomass gasification processes currently constitute one of the major problems in biomass valorisation, generating clogging of filters and issues related with the purity of syngas production. To date, these waste residues find no useful applications and they are generally disposed upon generation in the gasification process. A detailed analysis of these residues pointed out the presence of high quantities of Ca （〉30 wt%）. TG experiments indicated that a treatment under air at moderate temperatures （400-800 ~C） decomposed the majority of carbon species, while XRD indicated the presence of a crystalline CaO phase. CaO enriched valorized materials turned out to be good heterogeneous catalysts for biodiesel production from vegetable oils, providing moderate to good activities （50%-70% after 12 h） to fatty acid methyl esters in the transesterification of sunflower oil with methanol.

Stable carbon isotope geochemistry provides important information for the recognition of funda- mental isotope exchange processes related to the movement of carbon in the lithosphere and permits the elab- oration of models for the global carbon cycle. Carbon isotope ratios in fluid-deposited graphite are powerful tools for unravelling the ultimate origin of carbon （organic matter, mantle, or carbonates） and help to constrain the fluid history and the mechanisms involved in graphite deposition. Graphite precipitation in fluid-deposited occurrences results from C02- and/or CH4-bearing aqueous fluids. Fluid flow can be considered as both a closed （without replenishment of the fluid） or an open system （with renewal of the fluid by successive fluid batches）. In closed systems, carbon isotope systematics in graphite is mainly governed by Rayleigh precipi- tation and/or by changes in temperature affecting the fractionation factor between fluid and graphite. Such processes result in zoned graphite crystals or in successive graphite generations showing, in both cases, isotopic variation towards progressive 13C or 12C enrichment （depending upon the dominant carbon phase in the fluid, C02 or CH4, respectively）. In open systems, in which carbon is episodically introduced along the fracture systems, the carbon systematics is more complex and individual graphite crystals may display oscillatory zoning because of Rayleigh precipitation or heterogeneous variations of 613C values when mixing of fluids or changes in the composition of the fluids are the mechanisms responsible for graphite precipitation.

Severe Acute Respiratory Syndrome Coronavirus(SARS-CoV)-2 infection induces an exacerbated inflammation driven by innate immunity components.Dendritic cells(DCs)play a key role in the defense against viral infections,for instance plasmacytoid DCs(pDCs),have the capacity to produce vast amounts of interferon-alpha(IFN-α).In COVID-19 there is a deficit in DC numbers and IFN-αproduction,which has been associated with disease severity.In this work,we described that in addition to the DC deficiency,several DC activation and homing markers were altered in acute COVID-19 patients,which were associated with multiple inflammatory markers.Remarkably,previously hospitalized and nonhospitalized patients remained with decreased numbers of CD1c+myeloid DCs and pDCs seven months after SARS-CoV-2 infection.Moreover,the expression of DC markers such as CD86 and CD4 were only restored in previously nonhospitalized patients,while no restoration of integrinβ7 and indoleamine 2,3-dyoxigenase(IDO)levels were observed.These findings contribute to a better understanding of the immunological sequelae of COVID-19.
