Endoscopic ultrasound-guided drainage of pancreatic fluid collections

Pancreatic fluid collections (PFCs) develop secondary to either fluid leakage or liquefaction of pancreatic necrosis following acute pancreatitis, chronic pancreatitis, surgery or abdominal trauma. Pancreatic fluid collections include acute fluid collections, acute and chronic pancreatic pseudocysts, pancreatic abscesses and pancreatic necrosis. Before the introduction of linear endoscopic ultrasound (EUS) in the 1990s and the subsequent development of endoscopic ultrasound-guided drainage (EUS-GD) procedures, the available options for drainage in symptomatic PFCs included surgical drainage, percutaneous drainage using radiological guidance and conventional endoscopic transmural drainage. In recent years, it has gradually been recog-nized that, due to its lower morbidity rate compared to the surgical and percutaneous approaches, endoscopic treatment may be the preferred first-line approach for managing symptomatic PFCs. Endoscopic ultrasound-guided drainage has the following advantages, when compared to other alternatives such as surgical, per-cutaneous and non-EUS-guided endoscopic ***-GD is less invasive than surgery and therefore does not require general anesthesia. The morbidity rate is lower, recovery is faster and the costs are lower. EUS-GD can avoid local complications related to per-cutaneous drainage. Because the endoscope is placed adjacent to the fluid collection, it can have direct ac-cess to the fluid cavity, unlike percutaneous drainage which traverses the abdominal wall. Complications such as bleeding, inadvertent puncture of adjacent viscera, secondary infection and prolonged periods of drainage with resultant pancreatico-cutaneous fistulae may be avoided. The only difference between EUS and non-EUS drainage is the initial step, namely, gaining access to the pancreatic fluid collection. Metastatic Leiomyosarcoma of the Spine Updates in Management and Surgical Strategy

Introduction: Leiomyosarcoma (LMS) is a rare malignant tumor arising from the smooth-muscle cells which rarely metastasizes to the bone and even less is found in the spine as a primary localization. In this article the authors describe a case of a peculiar localization of this neoplasm in the upper thoracic spine which required a tailored surgical treatment. Case Report: A 52-year-old female presented with an 8 weeks' history of gradually worsening myelopathy and sensory level in the upper thoracic spine. Pre-operation MRI scans showed an invasive mass neoplasm at T3 and T4 levels involving the vertebral bodies and pedicles with a significant cord compression. A 360° two-step approach was performed. The first operation consisted in a T1-T4 decompressive laminectomy with a sub-total tumor resection and pedicle fixation extending from C7 to T5. A second surgical step was performed through an anterior approach in order to improve the amount of tumor removal. Instrument fixation assured the stabilization of the spinal segment. Post-operative scans demonstrated an optimal decompression. Neurological symptoms gradually improved, and the patient benefitted from a gradual reversion of previous symptomatology and could go back to her previous life. Conclusion: LMS represents a challenging tumor due to an extremely aggressive behavior and a considerably high rate of recurrence which requires a tailored approach in terms of surgical strategy and follow-up. A exhaustive review of literature was performed in the attempt to rationalize a surgical strategy and correct management of this extremely rare neoplastic lesion.

Mammographic Features of Hidradenitis Suppurativa

Hidradenitis suppurativa is a chronic suppurative inflammation of the apocrine sweat glands. Axillary, inframammary, intermammary, and peri-areolar apocrine gland involvement may be visualized mammographically. Characteristic lesions tend to be round or oval, lucent, and smooth-bordered, with or without central densities. While diagnosis made purely via mammography is unlikely, having an awareness of this entity may prove useful in patient care in regards to proper utilization of both imaging and consultative resources.

The coupling between microwave generators and ECR ion sources(ECRIS)is a key point for the design of the new generation ECRIS as well as for the optimization of the existing *** electromagnetic characterization of the plasma chamber where the ionization phenomena take place is a fundamental starting point to understand and model such *** such effort the complex structures of the injection and extraction flanges together with the large dimensions of the chamber and the high frequencies that are typically used make impossible an analytical solution and also create great difficulties in the modelling even with state-of- art electromagnetic simulators(CST,HFSS),In the following paper the results of some numerical calculations for the optimum plasma chamber excitation will be presented along with the experimental measurements carried out with the SERSE ion source at INFN-LNS.A campaign of measurements is also planned to further investigate the microwave coupling and the mode excitation,which determines the efficiency of the ECR plasma heating.

PURPOSE: To investigate the use of high-resolution ultrasonography for detect ing choroidal layer abnormalities in eyes with central serous chorioretinopathy (CSC). DESIGN: Prospective observational case series. METHOD: Optical coherence tomography (OCT) and high-resolution ultrasonography with a 20-MHz immersion p robe were performed bilaterally in 10 patients, of whom 5 were affected by unila teral active CSC and 5 by unilateral cystoid macular edema (CME). Ten age-match ed control subjects were also investigated. RESULTS: Both OCT and high-resoluti on ultrasonography showed a macular elevation in eyes with CSC and CME. High-re solution ultrasonography has shown evidence of a nonechogenic linear band under the retinal pigment epithelium in patients affected by CSC. This could be found in neither patients with CME or in control subjects. CONCLUSION: Highresolution ultrasonography can detect a nonechogenic space consistent with hyperpermeabilit y of choroidal capillaries in eyes with active CSC. Shadowing by an altered reti nal pigment epithelium cannot be ruled out, however.
