Antenatal anxiety in the first trimester: Risk factors and effects on anxiety and depression in the third trimester and 6-week postpartum

Objective: Anxiety is common among pregnant women. However, research attention in the area of reproductive mental health has mainly focused on postpartum depression in past decades. Given adverse outcomes of antenatal anxiety, there is an urgent need to fill the research gaps. The objectives of the present study were to determine the prevalence of antenatal anxiety symptoms and examine the risk factors and effects of anxiety symptoms in early pregnancy on anxiety and depressive symptoms in later pregnancy and early postpartum period. Methodology: A prospective longitudinal design with quantitative approach was adopted. A consecutive sample of 1470 Chinese pregnant women from hospitals in Hong Kong was invited to participate in the study and was assessed using standardized instruments on 3 time points including first and third trimesters of pregnancy and 6-week postpartum. Results: The results showed that 17.7% of pregnant women manifested anxiety symptoms in the first trimester of pregnancy. Single mothers, younger mothers, mothers who smoked before pregnancy and mothers who received low education level reported significantly higher levels of anxiety symptoms in the first trimester. Unwanted pregnancy, low self esteem, low marital satisfaction and perceived low social support were significant psychosocial risk factors for anxiety symptoms in the first trimester. Anxiety symptoms in the first trimester were independent predictors for anxiety symptoms in the third trimester ( QUOTE β = 0.26, t = 5.74, p < 0.001), however anxiety symptoms in the first trimester no longer significantly predicted anxiety and depressive symptoms in 6 weeks postpartum after adjusting for the effects of potential confounders. Discussions: The present study points to the need for greater research and clinical attention to antenatal anxiety given that antenatal anxiety is a prevalent problem and has serious impacts on maternal well-being. Analysis of the Screening of Contact Subjects of People Living with HIV at the Antivenereal Health Center of the National Institute of Public Hygiene of Treichville, Abidjan/Côte d’Ivoire from January 2009 to December 2018

Introduction: In C?te d’Ivoire, people living with HIV are estimated to 460,000 with about 290,000 who know their HIV status. Index testing is a strategy for screening unidentified HIV carriers. This study aimed at analyzing the screening of contact subjects of people living with HIV at the anti-venereal health center of Treichville in Abidjan. Methods: We carried out a cross-sectional study from January 2009 to December 2018. The target population was index patients and their sexual partners. After giving their consent, index patients and their sexual partners were interviewed during their medical visit. Data were analyzed using Epi info 3.5.4. The khi2 test was performed at 5% significance level. Results: A total of 114 index patients have been included in the study. The majority of sexual partners were men (sex ratio M/F = 1.5) with an average age of 43 years (±9.77). 87.7% of index patients lived in pairs. The analysis showed that 87.7% of index patients had disclosed their HIV status to their sexual partners and 94.3% of index patients invited their sexual partners to be tested. 85.8% of sexual partners had been tested. 19.8% of sexual partners who have been tested were HIV positive. All the 8 children of female index patients were HIV negative. HIV test acceptability was statistically associated with living in pairs (P Conclusion: Screening sexual partners from index patients has been an effective way to detect HIV carriers who did not know their status.

Comparison between the SAPIEN S3 and the SAPIEN XT transcatheter heart valves:A single-center experience

AIM To investigate the clinical outcomes of transcatheter aortic valve implantation(TAVI) with the SAPIEN 3 transcatheter heart valve(S3-THV) vs the SAPIEN XT valve(XT-THV).METHODS We retrospectively analyzed 507 patients that underwent TAVI with the XT-THV and 283 patients that received the S3-THV at our institution between March 2010 and December *** Thirty-day mortality(3.5% vs 8.7%:OR=0.44,P=0.21) and 1-year mortality(25.7% vs 20.1%,P=0.55) were similar in the S3-THV and the XT-THV *** rates of both major vascular complication and paravalvular regurgitation(PVR)>1 were almost 4 times lower in the S3-THV group than the XT-THV group(major vascular complication: 2.8% vs 9.9%,P1: 2.4% vs 9.7%,P<0.0001).However,the rate of new pacemaker implantation was almost twice as high in the S3-THV group(17.3% vs 9.8%,P=0.03).In the S3 group,independent predictors of new permanent pacemaker were pre-procedural RBBB(OR=4.9:P=0.001),pre-procedural PR duration(OR=1.14,P=0.05) and device lack of coaxiality(OR=1.13:P=0.05) during *** The S3-THV is associated to lower rates of major vascular complications and PVR but higher rates of new pacemaker compared to the ***-optimal visualization of the S3-THV in relation to the aortic valvular complex during deployment is a predictor of new permanent pacemaker.
