Assessing of the Therapeutic Efficacy of These 2 Treatment Regimens in the Management of Helicobacter pylori Infection at Kinshasa

Introduction: Helicobacter pylori (Hp) infection is a worldwide public health problem. Unfortunately, its management poses a problem because of resistance to antibiotics. However, there are codified treatment protocols covering sequential and concomitant quadritherapy with regard to first-line probabilistic treatment. The objective of this study was to assess the therapeutic efficacy of these 2 treatment regimens in the management of Hp infection at Kinshasa. Methods: This was a mixed study, with documentary, descriptive and interventional approaches, carried out between September 1, 2018 and April 30, 2020. Results: Sixty-four patients were collected, including 36 men against 28 women with a sex ratio of 1H:1F;the mean age was 54 ± 16.5 years. There was an over-representation of senior patients (n = 29);an intermediate number of adult patients (n = 22) and a lower number of young patients (n = 13). 34 and 30 were respectively treated according to the concomitant and sequential regimens. Concomitant quadruple therapy offered an eradication rate of 91.2% compared to 56.7% for sequential quadruple therapy;concomitant treatment, advancing age and absence of risky behavior more quickly predicted the occurrence of eradication success. Conclusion: The present study showed superiority of concomitant quadruple therapy over sequential quadruple therapy in first-line treatment. Alcohol with active smoking had a negative influence, while concomitant quadruple therapy, advancement in age had a positive influence on the success of the eradication of Hp infection.

Context and Objective: The practice of outpatient anaesthesia has many advantages over anesthesia with conventional hospitalisation, particularly in the reduction of post-operative complications and the faster resumption of activity, etc. No study has been carried out on this subject in our community;this study was carried out in order to come up with a concrete state of play when it comes to outpatient anesthesia for medical and surgical procedures in the city of Kinshasa. Methodology: This was a cross-sectional study, which took place in seven hospitals in the city of Kinshasa from 1 November 2020 to 31 January 2021. The population consisted of all patients who received outpatient anesthesia and the included patients had signed an informed consent. The variables of the study were the general profile of patients, surgical procedures and indications, anesthetic data and patient evolution. Statistical analyses were performed with SPPS 21.0 with p Results: We collected data from 971 patients who had undergone anesthesia in the seven medical facilities in the city of Kinshasa. Among these patients, 394 had benefited from outpatient anesthesia, i.e. a frequency of outpatient anesthesia estimated at 40.5%. Three hundred and ninety-four (394) patients were selected. They were 58.6% women and 41.4% men i.e. a sex ratio M/F of 0.7. The mean age was 39.3 ± 18.7 years with the extremes of 1 and 82 years. Gastroscopy was the most performed (operative) procedure (21.6%). Patients were classified as ASA 1 in 75.1%, narcosis (80.7%) using propofol (80.2%) was the most used anesthetic technique and performed by a specialist doctor in Anesthesia in 65.5%. Suxamethonium was the most used curare in 13.2%. Fentanyl was the most used opioid in 14.5%. Bupivacaine (10.9%) was the most widely used local anesthetic. Mg-air batteries have attracted tremendous attention as a potential next-generation power source for portable electronics and e-transportation due to their remarkable high theoretical volumetric energy density,environmental sustainability,and ***,the fast hydrogen evolution reaction(HER)in NaCl-based aqueous electrolytes impairs the performance of Mg-air batteries and leads to poor specific capacity,low energy density,and low ***,the conventionally used NaCl solute was proposed to be replaced by NaNO_(3)and acetic acid additive as a corrosion inhibitor,therefore an electrolyte engineering for long-life time Mg-air batteries is *** resulting Mg-air batteries based on this optimized electrolyte demonstrate an improved discharge voltage reaching~1.8 V for initial 5 h at a current density of 0.5 mA/cm^(2) and significantly prolonged cells'operational lifetime to over 360 h,in contrast to only~17 h observed in NaCl electrolyte.X-ray photoelectron spectroscopy and time-of-flight secondary ion mass spectrometry were employed to analyse the composition of surface film and scanning electron microscopy combined with transmission electron microscopy to clarify the morphology changes of the surface layer as a function of acetic acid *** thorough studies of chemical composition and morphology of corrosion products have allowed us to elucidate the working mechanism of Mg anode in this optimized electrolyte for Mg-air batteries.

Background and Objective: HIV infection is often associated with HBV and HCV infection, together leading to high morbidity and mortality in developing countries. The objective of this study is to describe the clinical, biological, immunological and therapeutic profile of patients co-infected with HIV-HBV and/or HCV. Methods: A cross-sectional and descriptive study including 180 people living with HIV (PLWHIV) in the city of Kinshasa province was conducted. Socio-demographic, clinical, biological and serological characteristics were analyzed. Results: The frequency of HIV-HBV/HCV co-infection was 23.9%. The distribution of age and sex of patients did not differ significantly according to co-infection status. The notion of pedicure and manicure was significantly more observed in patients free from viral hepatitis (51.1% versus 32.6%, p = 0.034). The median duration of knowledge of the HIV status which was longer in the co-infected (4 years versus 2 years, p = 0.022). A lower median level of GPT was observed in co-infected compared to other patients (14 IU/L versus 20 IU/L, p = 0.041). Serum albumin (3.1 g/L versus 3.3 g/L, p = 0.034) and prothrombin (58.3% versus 65.6%, p = 0.045) were lower in HIV co-infected-VHB and/or VHC. The median INR was higher in co-infected than in other patients (1.6 versus 1.4;p = 0.009). Patients without therapy Antiretroviral (TARV) medication were more numerous in co-infected (20.9% versus 8.0%, p = 0.025). Conclusions: The profile of PLWHIV was dominated by the presence of pedicures and manicures with high transaminases and without anti-viral treatment.

The Maxwell-Heaviside Equations Explained by the Theory of Informatons

In the articles “Newtons Law of Universal Gravitation Explained by the Theory of Informatons” and “The Gravitational Interaction between Moving Mass Particles Explained by the Theory of Informatons” the gravitational interaction has been explained by the hypothesis that information carried by informatons is the substance of gravitational fields, i.e. the medium that the interaction in question makes possible. From the idea that “information carried by informatons” is its substance, it has been deduced that—on the macroscopic level—a gravitational field manifests itself as a dual entity, always having a field- and an induction component (Egand Bg) simultaneously created by their common sources. In this article we will mathematically deduce the Maxwell-Heaviside equations from the kinematics of the informatons. These relations describe on the macroscopic level how a gravitational field (Eg, Bg) is generated by whether or not moving masses and how spatial and temporal changes of Egand Bgare related. We show that there is no causal link between Egand Bg.

In the article “Newtons Law of Universal Gravitation Explained by the Theory of Informatons” the gravitational interaction between mass particles at rest has been explained by the hypothesis that g-information carried by informatons is the substance of the medium that the interaction in question makes possible. It has been showed that, on the macroscopic level, that medium—the “gravitational field”—manifests itself as the vector field Eg. In this article we will deduce from the postulate of the emission of informatons, that the informatons emitted by a moving mass particle carry not only information about the position (g-information) but also about the velocity (“β-information”) of their emitter. It follows that the gravitational field of a moving mass particle is a dual entity always having a field- and an induction-component (Egand Bg) simultaneously created by their common sources: time-variable masses and mass flows and that the gravitational interaction is the effect of the fact that an object in a gravitational field always tends to become “blind” for that field by accelerating according to a Lorentz-like law.

Newton’s Law of Universal Gravitation Explained by the Theory of Informatons

In the context of classical physics, Newton’s law of universal gravitation describes the attraction between two mass particles separated in space. In the same context a vector field Eg, that is not associated with anything substantial, has been introduced as the entity that mediates in the gravitational interactions. In this article, we will show that Egis the mathematical quantity that—at the macroscopic level—fully characterizes the medium that makes the interaction between particles at rest possible. We identify that medium as “the gravitational field”. To define the nature of the gravitational field, we will start from the hypothesis that a material object manifests itself in space by the emission—at a rate proportional to its rest mass—of mass and energy less granular entities that—relative to an inertial reference frame—are rushing away with the speed of light and that are carriers of information referring to the position of their emitter (“g-information”). Because they transport nothing else than information, we call these entities “informatons”. We will show that the expanding cloud of g-information created by the continuous emission of informatons by a mass particle at rest can be fully characterized by the vector field Eg, which implies that that cloud can be identified as the gravitational field of the particle. We will also show that the gravitational interaction between mass particles can be explained as the response of a particle to the disturbance of the symmetry of its “proper” gravitational field by the field that, in its direct vicinity, is created and maintained by other mass particles.

Ultra-wideband(UWB)microwave sources driven by specialised pulsed power generators have experienced a considerable development in the last decade due to their wide domain of new applications such as defence or counter-terrorism *** authors present the main findings of a research dedicated to the development of a pulsed power-driven electromagnetic field source for disabling improvised explosive devices(IED).The pulsed power generator driving the source is a 13-stage compact Marx producing voltage pulses reaching an amplitude of 0.5 MV,with a pulse repetition frequency(PRF)of up to 100 *** generator is coupled to a bipolar pulse forming line,providing bipolar pulses with a dV/dt of around 1.6 MV/*** pulsed power system feeds an array composed of 16 Koshelev-type UWB antennas through an impedance matching *** resulting electromagnetic source is capable to produce pulsed electric fields(PEFs)having a figure-of-merit(FOM)of 1 ***,practical experiments were carried out to study the effects of the PEFs on *** targets used in the present study are M2B type flashbulbs,known to have the same susceptibility as the US army M6 *** configurations of wires(shielded,twisted,etc)with different lengths were used in connecting items inside these *** tests were performed by placing the flashbulbs at different distances to determine the essential parameters(i.e.,amplitude,duration,and frequency range)of the PEFs required to trigger *** overview of the experimental campaign and the main findings are also presented followed by conclusions.

Background and Purpose: The hepatitis C prevalence rate is among the highest the world. Thus, in the context of transfusion safety, WHO has made it compulsory to screen blood bags by rapid diagnostic orientation tests (TROD) validated in regions that do not have the capacity to use more sophisticated technologies. The purpose of this study is to assess the performance of the TRODs commonly used in Kinshasa. Methods: This was a cross-sectional and analytical study carried out from August 26, 2019 to January 20, 2020 in 200 voluntary blood donors at the University Clinics of Kinshasa (CUK). The detection of anti-HCV antibodies was carried out by Mindray (gold standard) and the SD Bioline, Encode, Fortress and Accurate tests. The diagnostic performance of TRODs was evaluated compared to the Mindray test. Results: Of a total of 200 donors, 112 (56.0%) were male versus 88 (44.0%) female with a sex ratio of 1M: 1F. HCV seroprevalences by Mindray, and other tests were 29.5%, 28.0%, 27.0%, 27.0% and 31.5%, respectively;the Se, Sp, VPP, VPN, SD Bioline, Accurate, Fortress and Encode were 74.6%, 92.9%, 81.5%, 89.7%, respectively;74.6%, 92.9%, 81.5%, 89.7%;74.6%, 91.4%, 78.6%, 89.6%;69.5%, 70.0%, 65.1%, 84.4%. Conclusion: The diagnostic performance of TRODs evaluated in this study does not meet European Union standards (Se = 100%;Sp > 99.5%). The Ministry of Health should promote large-scale validation of TRODs based on World Health Organization guidelines.

Obtaining clean energy is of prime importance for planetary health and sustainable *** aimed to assess the association between residential energy transition and the risk of chronic respiratory *** data from the Global Health Observatory and Global Burden of Diseases,Injuries,and Risk Factors Study,we delineated the spatial distribution and temporal trends of the population using clean fuels for cooking at a global *** the China Health and Retirement Longitudinal Study,we performed rigorous and well-structured multistage analyses incorporating both cross-sectional and prospective data analyses to examine the associations between solid fuel use,residential energy transition,duration of solid fuel use,and the risk of chronic respiratory *** great progress,huge disparities in access to clean energy persist *** energy transition was associated with a lower risk of chronic respiratory *** the period of 2011–2013,compared with persistent solid fuel users,both participants who switched from solid to clean fuels(adjusted risk ratio[RR]0.78,95%confidence interval[CI]0.62–0.98)and persistent clean fuel users(adjusted RR 0.71,95%CI 0.57–0.89)had significantly lower risk of chronic respiratory diseases(p<0.001 for trend).Consistent associations were observed in the period of 2011–2015 and 2011–*** energy transition from solid to clean fuels could reduce the risk of chronic respiratory *** is a valuable lesson for policy-makers and the general public to accelerate energy switching to alleviate the burden of chronic respiratory diseases and achieve health benefits,particularly in low-and middle-income countries.
