Pattern of disease recurrence and its implications for postoperative surveillance after curative hepatectomy for hepatocellular carcinoma: experience from a single center

Back.round: Hepatectomy is a w.dely accepted curative treatment for hepatocellular carcinoma (HCC). How.ver, the disease frequently recurs after a curative hepatectomy. The objective of this study is to provide a better understanding of the pattern of disease recurrence and the risk.factors involved so as to improve the postoperative surveillance. Methods: A retrospective study for all patients receiving hepatectomy for HCC betw.en 2003 and 2014 w.s performed. Emphasis w.s made on the timing and pattern of recurrent disease, and type of treatment given. Results: There w.re 506 patients in the study. Median follow.up w.s 43.7 months. The 1-, 3-, 5-, 10-year overall and disease free survival w.re 89.5%, 74.1%, 63.9%, 49.0% and 69.5%, 54.3%, 43.4%, 30.9%respectively. Recurrent disease occurred in 267 patients, 47.2% occurred w.thin 9 months of hepatectomy and 80.1% recurred only in liver. Median survival w.s shorter for recurrence occurring w.thin 9 months compared w.th those occurring betw.en 10 months and 2 years postoperatively (36.2 vs. 65.7 months, Pw.ilst less curative treatment w.s offered for patients w.th early (w.thin 9 months) intrahepatic alone recurrence (22.2% vs. 51.7%, P3.5 cm and history of rupture w.re risk.factors for recurrence w.thin 9 months. Conclusions: These findings suggest that recurrent diseases are common after curative hepatectomy for HCC and most recurrences occur in the remnant liver. Since almost half of recurrences occurred w.thin first 9 months after hepatectomy, a more stringent postoperative surveillance w.th target imaging of liver in this period is needed. Early diagnosis of recurrent disease and curative retreatment hopefully can bring about a longer survival.

Intracranial arachnoid cysts (AC) are believed to be congenital and chronic subdural hematomas tend to occur in elderly patients w.th a history of mild head injury. The association betw.en these tw. entities sporadically occur in relatively young patients but rare in elderly patients. w. report a 65-year-old man w.o presented w.th headache and dizziness of 2 months’ duration w.th a history of head injury. Brain computed tomography (CT) a CSDH in right side and a hygroma in left side. After first operation w.th burr holes in both sides, the patient underw.nt an early recurrence of acute subdural hematoma in the right side. The evacuation of this hematoma by a craniotomy allow.d seeing an AC that w. resected partially w.th complete recovery of the patient. The association CSDH/AC is rare and possible in elderly patients and there is no consensus on treatment.

FAMCS: Finding All Maximal Common Substructures in Proteins

Finding the common substructures shared by tw. proteins is considered as one of the central issues in computational biology because of its usefulness in understanding the structure-function relationship and application in drug and vaccine design. In this paper, w. propose a novel algorithm called FAMCS （Finding All Maximal Common Substructures） for the common substructure identification problem. Our method w.rk. initially at the protein secondary structural element （SSE） level and starts w.th the identification of all structurally similar SSE pairs. These SSE pairs are then merged into sets using a modified Apriori algorithm, w.ich w.ll test the similarity of various sets of SSE pairs incrementally until all the maximal sets of SSE pairs that deemed to be similar are found. The maximal common substructures of the tw. proteins w.ll be formed from these maximal sets. A refinement algorithm is also proposed to fine tune the alignment from the SSE level to the residue level. Comparison of FAMCS w.th other methods on various proteins show. that FAMCS can address all four requirements and infer interesting biological discoveries.

The Caledonide Orogen in the Nordic countries is exposed in Norw.y, w.stern Sw.den, w.sternmost Finland, on Svalbard and in northeast Greenland. In the mountains of w.stern Scandinavia, the structure is dominated by E-vergent thrusts w.th allochthons derived from the Baltoscandian platform and margin, from outboard oceanic （lapetus） terranes and w.th the highest thrust sheets having Laurentian affinities. The other side of this bivergent orogen is w.ll exposed in northeastern Greenland, w.ere w.vergent thrust sheets emplace Laurentian continental margin assemblages onto the platform. Svalbard＇s Caledonides are disrupted by late Caledonian faults, but have close affinity w.th the Laurentian margin in Northeast Greenland. Only Svalbard＇s Southw.stern terrane is foreign to this margin, show.ng affinity, to the Pearya terrane of northern Ellesmere Island in arctic Canada. Betw.en the margins of w.stern Scandinavia and eastern Greenland, the w.de continental shelves, now.covered by late Paleozoic and younger successions, are inferred to be underlain by the Caledonide hinterland, probably incorporating substantial Grenville-age basement. In northernmost Norw.y, the NE-trending Caledonian thrust front truncates the Nw.trending Neoproterozoic Timanide orogen of northw.st Russia. Much of the central and eastern parts of the Barents Shelf are thought to be underlain by Caledonian-deformed Timanide basement. Caledonian orogeny in Norden resulted from the closure of the Iapetus Ocean and Scandian collision of continent Baltica w.th Laurentia. Partial subduction of the Baltoscandian margin beneath Laurentia in the midlate Silurian w.s follow.d by rapid exhumation of the highly metamorphosed hinterland in the early Devonian, and deposition of Old Red Sandstones in intramontane basins. Late Scandian collapse of the orogen occurred on major extensional detachments, w.th defor mation persisting into the late Devonian.

Endoscopic visualisation of gastric atrophy is usually not feasible w.th *** endoscopy. Magnifying endoscopy is helpful to analyze the subepithelial microvascular architecture as w.ll as the mucosal surface microstructure w.thout tissue biopsy. Using this technique w. w.re able to describe the normal gastric microvasculature pattern and w. also identified characteristic patterns in tw. cases of autoimmune atrophic gastritis.

红树林沉积物聚乙烯降解菌的筛选和降解特性

【目的】探究湛江市麻章区红树林沉积物中微生物对聚乙烯(PE)塑料的降解特性,丰富红树林塑料降解菌种类。【方法】在湛江市麻章区红树林的高、中、低潮带随机选取9个样点,并采用五点采样法从红树林表层2~5 cm的沉积物中取样。将采集到的沉积物样本在以聚乙烯为唯一碳源的无机盐培养基中进行富集、分离、纯化,以筛选出能够降解PE的菌株,并通过显微形态分析、分子鉴定等方法确定其种属;通过测定生长曲线、失重率、以及分析官能团变化来验证菌株对PE塑料的降解性能。【结果与结论】从沉积物中共筛选出k.-1、k.-2和k.-3三株PE降解能力较强的菌,分别为威尼斯不动杆菌(Acinetobacter venetianus)、Pseudomonas juntendi和维德曼芽孢杆菌(Bacillus w.edmannii)。在120 d的培养周期中,这些菌株的生长曲线均呈抛物线形态。经三株菌作用后,PE表面生成羟基,并形成氨基和羟基化合物,羟基取代羰基,导致PE表观和内部结构明显改变。经120 d处理后,PE膜的失重率分别为6.42%、5.38%和5.73%,其中k.-1菌株处理后PE塑料的失重率相较于目前报道的红树林沉积物中最高降解效率菌株Bacillus gottheilii处理后塑料的失重率提升3.55%。威尼斯不动杆菌(***)在PE塑料降解上具有一定潜力,这丰富了湛江红树林沉积物中PE降解菌的种类,为PE塑料的生物降解提供理论支持。
