婴幼儿颅缝早闭的额眶畸形矫正

目的　探讨手术治疗婴幼儿颅缝早闭引起的额眶骨畸形的手术方法和手术时机。方法　采用额骨上眶骨联合截骨塑形前移的手术方法。连续收治了 11名患儿 ,2名女性 ,9名男性 ,年龄 6～ 9个月。其中 ,额缝早闭 6例 ,非综合征性单侧冠状缝早闭 2例 ,多骨缝早闭 1例 ,Apert综合征和Saethre Chotzen综合征累及冠状缝等骨缝早闭各 1例。结果　随访 2～ 11个月 ,均取得满意的矫正效果。其中 1例术后发现颅骨顶部 (非术区 )局部隆起 ,经戴头盔 3个月得以控制。未发生明显并发症。结论　额骨上眶骨联合截骨塑形前移方法能够安全、有效地矫正颅缝早闭引起的额眶发育不良。

Variations in ice mass deform the Earth and modify its gravity field,a process known as Glacial Isostatic Adjustment(GIA).GIA in Antarctica remains poorly constrained due to the cumulative effect of past and present ice-mass changes,the unknown history of the past ice-mass change,and the uncertainties on the mechanical properties of the *** paper investigates the effect of using Andrade and Burgers viscoelastic rheologies,rather than the commonly used Maxwell rheology,to model GIA-induced deformation in *** Love number and Green's function formalism are used to compute the radial surface displacements and the gravity changes induced by the past and present ice-mass *** consider an Earth model whose elastic properties and radial structure are averaged from the Preliminary Reference Earth Model and two viscosity profiles to account for the recently published results on the present ice-mass *** the three rheological laws affects the temporal response of the Earth differently,leading to smaller discrepancies than those induced by the two viscosity *** differences are the largest between Maxwell and Burgers rheologies during the 100-1000 years following the beginning of the surface-mass *** show that using the Andrade and Burgers rheologies allows the Earth to respond on decennial to centennial time scales,up to 10 m more than *** only the recent ice-mass changes,the deformation rates derived from Burgers and Andrade rheologies are several times larger than those estimated by Maxwell rheology.

目的探讨和评价下颌骨牵引成骨技术在治疗儿童小下颌畸形伴阻塞性睡眠呼吸暂停综合征(obstructivesleepapneasyndrome,OSAS)中的应用价值。方法6例先天性小下颌发育不全畸形伴重度OSAS,年龄4个月至9岁。每例患者术前术后均行X射线头影测量及睡眠多导图仪检查。用牵引成骨技术牵引双侧下颌骨,带动舌和口底肌肉组织前移,使口咽腔通畅,达到治疗小下颌畸形患者OSAS的目的。结果6例12侧下颌骨牵引成骨,最大牵引延长距离为25mm,最小15mm,平均19.2mm。后气道间隙由平均4.5mm增至10.1mm。牵引时间最短10d,最长20d,平均14.5d。所有病例在截骨、牵引器固定过程中均未损伤下牙槽神经血管束。牵引过程顺利,牵引区成骨良好。随访2至11个月。1例需要进行继续治疗,5例均恢复正常呼吸,去除鼻咽导气管和气管切开导管。结论下颌骨牵引成骨术是治疗OSAS重要的有效治疗手段,能够有效地矫正口咽腔气道狭窄,改善呼吸,可在年幼儿童应用。随着应用例数的增多会更详尽阐明其所发挥的作用。

背景:严重小下颌畸形伴阻塞性睡眠呼吸暂停综合征(obstructivesleepapneasyndrome,OSAS)常常危及患者生命,并影响患者的呼吸功能。常规手术治疗风险较大,同时不适用于发育中的患者不能获得满意的效果。目的:探讨和评价下颌骨牵引成骨技术在治疗小下颌畸形伴OSAS中的应用价值及对呼吸功能的改善作用。设计:前后对照研究。单位:中国医学科学院中国协和医科大学整形外科医院,澳大利亚墨尔本皇家儿童医院颅面外科。对象:2001-10/2004-07在中国医学科学院中国协和医科大学整形外科医院和澳大利亚墨尔本皇家儿童医院颅面外科就治的小下颌畸形伴OSAS患者8例,男5例,女3例,年龄4个月～17岁。方法:8例患者均行牵引成骨技术牵引双侧下颌骨治疗,经口内或口外途径共安放16个牵引器,牵引速度1～1.5mm/d,固定4～12周,随访2～18个月。每例患者术前术后均行X射线头影测量或睡眠多导图仪检查。主要观察指标:8例患者术后的牵引延长距离、后气道间隙大小、呼吸功能改善情况。结果:8例16侧下颌骨牵引成骨,所有病例截骨、牵引过程顺利,牵引区成骨良好。最大牵引延长距离为25mm,最小15mm,平均19.12mm。后气道间隙由平均4.5mm增至11mm。1例需要进行继续治疗,7例均恢复正常呼吸,去除鼻咽导气管和气管切开导管。随访2～18个月。

Heterotopic ossification(HO)is a consequence of traumatic bone and tissue damage,which occurs in 65%of military casualties with blast-associated ***,the mechanisms behind blast-induced HO remain *** models are used to study blast-induced HO,but developing such models is challenging,particularly in how to use a pure blast wave(primary blast)to induce limb fracture that then requires an *** studies,including our recent study,have developed platforms to induce limb fractures in rats with blast loading or a mixture of blast and impact ***,these models are limited by the survivability of the animal and repeatability of the *** this study,we developed an improved platform,aiming to improve the animal's survivability and injury repeatability as well as focusing on primary blast *** platform exposed only one limb of the rat to a blast wave while providing proper protection to the rest of the rat's *** obtained very consistent fracture outcome in the tibia(location and pattern)in cadaveric rats with a large range of size and ***,the rats did not obviously move during the test,where movement is a potential cause of uncontrolled *** further conducted parametric studies by varying the features of the design of the *** factors,such as how the limb is fixed and how the cavity through which the limb is placed is sealed,significantly affect the resulting *** platform and test setups enable well-controlled limb fracture induced directly by pure blast wave,which is the fundamental step towards a complete in vivo animal model for blast-induced HO induced by primary blast alone,excluding secondary and tertiary blast *** addition,the platform design and the findings presented here,particularly regarding the proper protection of the animal,have implications for future studies investigating localized blast injuries,such as blast induced brain and lung injuries.

The spatiotemporal distributions of microbes in soil by different methods could affect the efficacy of the microbes to reduce the soil hydraulic *** this study,the specimens of bio-mediated sands were prepared using three different methods,***,mixing,and pouring a given microbial so-lution onto compacted sand *** hydraulic conductivity was measured by constant-head tests,while any soil microstructural changes due to addition of the microbes were observed by scan-ning electron microscope(SEM)and mercury intrusion porosimetry(MIP)*** amount of dextran concentration produced by microbes in each type of specimen was quantified by a *** show that dextran production increased exponentially after 5-7 d of microbial settling with the supply of culture *** injection and mixing methods resulted in a similar amount and uniform dis-tribution of dextran in the *** pouring method,however,produced a nonuniform distri-bution,with a higher concentration near the specimen *** the supply of culture medium discontinued,the dextran content near the surface produced by the pouring method decreased dramatically due to high competition for nutrients with foreign *** dextran concentration was negatively and correlated with hydraulic conductivity of bio-mediated soils exponentially,due to the clogging of large soil pores by *** hydraulic conductivity of the injection and mixing cases did not change significantly when the supply of culture medium was absent.
