Tuberculosis among Chronic Hemodialysis Patients: A Senegalese Single Center Experience

Summary: Tuberculosis is a common infectious disease in chronic hemodialysis due to alteration of the immune system associated with chronic kidney disease. The objectives of this study are to determine the prevalence of tuberculosis in chronic hemodialysis patients and to identify its diagnostic and therapeutic difficulties. Methods and patients: This was a descriptive retrospective study over a period of 20 years (1994-2014). It includes the records of periodic hemodialysis patients in the Nephrology Department of the Aristide Le Dantec University Teaching Hospital in Dakar which clinical symptoms and laboratory favor tuberculosis. Results: Of 258 chronic hemodialysis patients treated in Hospital Aristide Le Dantec hemodialysis center, 29 cases (11.4%) of tuberculosis disease are diagnosed. The mean age is 43.21 ± 12.48 years, and the sex-ratio is 0.8. The median time to onset of tuberculosis after initiation of hemodialysis is 22.86 ± 28.86 months. The diagnosis of tuberculosis is sure only in 17% of cases. Extra-pulmonary sites are found in 79% of cases. The average duration of treatment is 9.39 ± 1.64 months (6 - 13 months). Various treatment protocols are adopted. Mortality is 21%, 50% due to disseminated tuberculosis. Conclusion: The diagnosis of tuberculosis in the chronic hemodialysis patients is often difficult due to the atypical symptoms, the frequency of extra-pulmonary location and the lack of evidence of sure diagnosis.

Introduction: Therapeutic patient education is a continuous, integrated, patient-centered process. A study was conducted in the Asthma School in the pneumophtisiology department of Senegal. Overall goal was to determine the influence of therapeutic education on the quality of life of asthmatics. Method: Transverse study, descriptive and evaluative therapeutic education provided in the school of asthma on quality of life for asthma patients. Data were analyzed through IBM SPSS (Statistical Package for Social Science) version 20. Results: 100 asthmatic patients were included with a sex ratio of 0.37. The average age of the patients was 36.76 years old. The majority (81.3%) (n = 39) being new participants. The average scores for symptoms, exposure to stimuli, emotions and limiting activities stay higher for adherent patients as well as the score overall quality of life. Conclusion: The school of asthma, associated with the therapeutic management, can improve the control of the disease and the quality of life of the patient.

Numerical Simulation of the Thermal Behaviour of a Building with or without Typha Using the Commercial Software Visual TTH

The use of low thermal conductivity materials plays an important role in the construction of energy-efficient buildings. Indeed, the improvement of the thermal properties of building walls reduces energy consumption. This study aims to determine the thermal and energy balances of a building enveloped with Typha australis on the one hand, and a building without Typha on the other hand (a building constructed with conventional materials) using the commercial software Visual TTH. The results of the heat loss calculations show that the Typha building is 62% more efficient than the conventional building. Also, the studies on air-conditioning needs resulted in energy savings of 1577.136 kWh per year for the Typha building, i.e. 1219 kg per year of CO2 avoided in the air according to the SENELEC emission factor 0.773/MWh in Senegal.

Analysis and Prediction of the COVID-19 Pandemic in Senegal Using the SIR Model

In this study, the mathematical SIR model (Susceptible-Infected-Recovered (cured and deceased)) was applied to the case of Senegal during the first two waves of the COVID-19 pandemic. During this period, from March 1, 2020, to March 30, 2021, the transmission and recovery rates as well as the number of reproduction were calculated and analyzed for the impact of the decisions taken by the Senegalese government. In both waves, the variation of the basic reproduction number as a function of time, with values below one towards the end of each study period, confirms the success of the Senegalese government in controlling the epidemic. The results show that the solution of mandatory mask-wearing is the best decision to counter the spread of the disease. Indeed, the mean number of reproduction is 2.11 in the first wave, and the second wave has a lower mean value of 1.23, while the decisions are less restrictive during this latter wave. Also, a short-term prediction model (about 4 months) was validated on the second wave. The validation criteria of this model reveal a good match between the results of the simulated model and the COVID-19 data reported via the Ministry of Health, Solidarity, and Social Action of Senegal.

Spilanthes oleracea *** an herbaceous plant whose scientific literature attributes among others,anti-malarial and anti-bilharzia *** virtues justify the placing on the market of drugs based on the *** study consisted on the one hand to evaluate the biomass of the plant on a soil of dune amended and on soil of unamended dune and to test its vegetative multiplication by transplanting,cuttings and *** results show that the growth of the species is greater on dune soil amended with an average biomass of 106.06 g compared to 71.06 g for un-amended soil *** transplanting of the plants and the layering were techniques that made it possible to multiply the *** oleracea *** be produced using this agronomic data.

Sensitivity Analysis of the Penman-Monteith Reference Evapotranspiration to Climatic Variables: Case of Burkina Faso

Sensitivity analysis is important in understanding the effect of each climatic input variable on the variation of the evapotranspiration which is a significant element for hydrological modelling, irrigation programs, and water resources management. This study investigates the responses of evapotranspiration to the variation of maximum and minimum temperature, solar radiation, wind speed and maximum and minimum relative humidity. Daily data from 1998 to 2012 of eight stations are used. For each climatic variable, a variation of 5% up to ±25% was performed in order to evaluate the sensitivity of the evapotranspiration to the input variables. Results show that evapotranspiration is more sensitive to the variation of solar radiation, maximal temperature and wind speed, respectively.

Finite Element Modeling of a Solar Box Cooker: Temperature and Fluid Velocity Distributions

We studied the temperature distribution and fluid velocity in a box-type solar cooker by using the Finite Element Method (FEM) in Ziguinchor southern of Senegal. Indeed, this is one of the sunniest countries in the world: more than 3000 hours of sunshine per year with an average temperature of around 30˚C. This abundant and exploitable solar energy contributes to the development of more efficient, profitable and clean sources of energy. This will help to satisfy the increasing demand of energy. This numerical model was validated by comparing the numerical results with those of the experiment carried out on a single day. The relative error obtained is below 3%. The model results confirmed the performance of this cooker as its cooking temperature is available for more than seven hours. They have shown that the temperature and internal fluid velocity fields are not homogeneous. The results, although preliminary and encouraging, are a first step towards the complete simulation of a solar cooker integrated into a drying column.

Osseous Dysplasia Secondary Infection: A Case Report

Introduction: Osseous dysplasia is a benign tumor of the jaws predominantly occurring in melanoderm women. The pathogenesis remains unknown. Its often fortuitous discovery usually is in the fourth decade of life. Its treatment involves surgery. Our aim was to report a case of osseous dysplasia secondary infection in Senegal. Observation: The over sixty-year-old patient initially came for recurring old suppuration. A prior history of dental avulsions and self-medication was found out. The clinical features were predominant chronic osteitic manifestations. Its radiologic presentation suggested a florid form. The combination of spiramycin-metronidazole had to be given twice. The isolated sequestrum was enucleated and the anatomopathology confirmed the diagnostics. Discussion: The patient’s age conformed to late discovery due to silent progression and poor access to medical care. The prior iatrogenic avulsions can be explained by the absence of systematic radiological exploration and by the unawareness of the inconspicuous tumor. The clinical and radiological features at this stage of the osseous dysplasia secondary infection were typical. The treatment and the follow-up were dependent on the prevailing work conditions. Conclusion: The chronic osteitic manifestations are indicative of osseous dysplasia in elderly women in Senegal.

Impact of Non-Compliance of Exposure Classes in Reinforced Concrete Structures

From the National Agency for the Organization and Construction of Infrastructures (ANOCI) program to the Emerging Senegal Plan (PSE), construction in Senegal has improved considerably. However, deficiencies remain in the specification of materials used mainly in hydraulic concrete. They are generally related to the specification of aggregates for alkali reactivity and the choice of exposure classes specified in NF EN 206-1. The purpose of this article is to study the incidence of non-compliance with exposure classes in reinforced concrete structures. To carry out this study, surveys were carried out at several sites (districts) in Dakar (Cité Avion, Touba Ouakam, Cité Asecna, Cité Batrain, Cité Comico, Cité Assemblée and Terme Sud) in order to collect information on the formulation and implementation methods used. The comparison of the various readings carried out made it possible to deduce conclusions and to give recommendations when using standard NF EN 206-1.

Updating Methods for Real Time Flood Forecasting： A Comparison through Senegal River Basin Upstream Bakel

Heavy floods occur frequently in the Senegal River Basin, causing catastrophic flooding downstream the river rating station of Bakel. Anticipating the occurrence of such phenomena is the only way to reduce the resulting damages. Flood forecasting is a necessity. Flood forecasting plays also an important role in the implementation of flood management scenarios and in the protection of hydro electric structures. Many methods are applied. The most complete are based on the conservation laws of physics governing the free surface flow. These methods need a complete description of the geometry of the river and their implementation requires also huge investments. In practice the river basin can be considered as a system of inputs-outputs related by a transfer function. In this paper the authors first used a multiple linear regression model with constant parameters estimated by the ordinary least square method to simulate the propagation of the floods in the upstream part of the Senegal river basin. The authors then apply statistical and graphical criteria of goodness-of-fit to test the suitability of this model. Three procedures of parameters updating have then been added to this linear model： the Kalman filter method, the recursive least square method, and the stochastic gradient method The criteria of goodness-of-fit used above have shown that the stochastic gradient method, although more rudimentary, represents better the flood propagation in the head basin of the Senegal river upstream Bakel. This result is particularly interesting because data influenced by Manantali Dam are used.
