Giant cerebriform.nevus cell nevus of the scalp is an extrem.ly rare form.of congenital m.lanocytic nevus. Giant cerebriform.nevus of the scalp has a m.jor psychosocial im.act because of its unsightly appearance with fetid m.ceration. We report the case of a 35-year-old wom.n who had a painless, m.lodorous swelling of the cerebriform.scalp m.asuring 20 × 17 cm.in diam.ter with a wide base of insertion at the occipital level adhering to the deep planes. The excision associated with a skin plasty was carried out. Histology concluded that there was a giant cerebriform.naevo-cellular nevus of the scalp.

Introduction The distribution of congenital heart disease (CHD) in sub-Saharan Africa is highly im.recise and varies from.one region to another due to the inequality of diagnostic facilities. The aim.of this study was to determ.ne the in-hospital prevalence of congenital heart disease in children at the m.ther-Child hospital of Bingerville (Hm.) by specifying the diagnostic, therapeutic and evolutionary aspects. m.terials and m.thods We conducted a retrospective, descriptive, cross-sectional study at Hm. of Bingerville from.January 2021 to Decem.er 2022. All newborns with congenital heart disease confirm.d by echocardiography were included in the study. Results Of 656 adm.ssions to the neonatology departm.nt over the study period, congenital heart disease accounted for 14.9% (98/656) of cases. In our series, 76.7% were diagnosed before the 1st week of life, with a m.an chronological age of 5.18 days and extrem.s of 0 and 46 days. There were as m.ny m.le patients (50%) as fem.le (50%), i.e. a sex ratio of 1. These newborns were prem.ture in 60.2% of cases, with a m.an and m.dian gestational age of 34 weeks’ am.norrhea. m.st were left-right shunts (90.8%). Persistent ductus arteriosus (PDA) (48.9%) predom.nated, followed by atrial septal defect (38.7%), ventricular septal defect (13.3%), com.on trunk artery (CTA) (3.1%) and open septal pulm.nary atresia (OSPA) (1%) as the prim.ry cyanogenic heart disease. Pulm.nary arterial hypertension (PAH) (50%) was prim.ry in 38.8% and secondary (61.2%). The m.rtality rate was 30.6%, and all CTA patients died (100%), with a significant statistical relationship (p = 0.027). Progression under treatm.nt was m.rked by clinical stabilization (68/98) in 69.4% of cases. Conclusion: Congenital heart disease is relatively com.on at the Bingerville Hm.. Access to echocardiography should be facilitated in neonatology departm.nts for rapid diagnosis and optim.l m.nagem.nt of congenital heart disease in newborns.

The large m.gnitude of the dim.nsionless Rayleigh num.er （Ra-10^8） for Earth＇s -3 000 km.thick m.ntle is considered evidence of whole m.ntle convection. However, the current form.lation assum.s behavior characteristic of gases and liquids and also assum.s Cartesian geom.try. Issues arising from.neglecting physical properties unique to solids and ignoring the spherical shapes for planets include：（1） Planet radius m.st be incorporated into Ra, in addition to layer thickness, to conserve m.ss during radial displacem.nts. （2） The vastly different rates for heat and m.ss diffusion in solids, which result from.their decoupled transport m.chanism., prom.te stability. （3） Unlike liquids, sub- stantial stress is needed to deform.solids, which independently prom.tes stability. （4） High interior com.ression stabilizes the m.ntle in additional m.nor ways. Therefore, representing conditions for convection in solid, self-gravitating spheroids, requires m.difying form.lae developed for bottom. heated fluids near am.ient conditions under an invariant gravitational field. To derive stability criteria appropriate to solid spheres, we use dim.nsional analysis, and consider the effects of geom.try, force com.etition, and m.croscopic behavior. We show that internal heating has been im.roperly ac- counted for in the Ra. We conclude that the lower m.ntle is stable for two independent reasons： heat diffusion far outpaces m.ss diffusion （creep） and yield strength of solids at high pressure exceeds the effective deviatoric stress. We discuss the role of partial m.lt in lubricating plate m.tion, and explain why the Ra is not applicable to the m.lti-com.onent upper m.ntle. When conduction is insufficient to transport heat in the Earth, m.lt production and ascent are expected, not convection of solid rock.

To the Editor:Total pancreatectom. with islet cell autotransplantation(TPIAT)is a viable treatm.nt option upon failed endoscopic and m.dical therapy for patients with chronic pancreatitis.This procedure involves surgical rem.val of the entire pancreas,isolation of islet cells and reinfusion of these cells into the liver via portal vein[1,2].The risk of contam.nation to the final islet cell product can occur at several stages of the isolation procedure[3].In order to ensure the sterility of the islet cell product,m.ltiple sam.les from.the preservation and cannulation solution,and the final islet cell product are sent for bacterial cultures.Prior studies have found variable clinical consequences of these cultures on infectious com.plications or graft function[3-9].Herein we aim.d to determ.ne the incidence of infection in 60 days post-TPIAT and its association with the culture data.

Acute septic arthritis in children is an orthopaedic em.rgency.A delay in diagnosis and inappropriate treatm.nt can result in devastating dam.ge to the joint with lifelong disability as a consequence.The clinical presentation can be a diagnostic challenge,especially in young children.A recent system.tic review showed that joint tenderness and fever are im.ortant signals of septic arthritis.Ultrasound is helpful in detecting the presence of a joint effusion.Plain radiographs m.y show bone changes but m.gnetic resonance im.ging is the m.st reliable im.ging study for detecting concom.tant osteom.elitis.The diagnosis of acute septic arthritis is highly suggestive when pus is aspirated from.the joint,in case of a positive culture or a positive gram.stain of the joint fluid,or if there is a white blood-cell count in the joint fluid of m.re than 50000/m.3.Staphylococcus aureus is the m.st com.only cultured organism.Recent system.tic reviews have identified the m.st effective drainage techniques,including needle aspiration,arthroscopy and arthrotom.,depending on the affected joint.After the drainage procedure it is im.ortant to m.nitor the clinical and laboratory outcom.s.Additional drainage procedures m.y be necessary in select cases.
