Study Objectives: To determine the association of excessive daytime sleepiness（EDS） with the conversion of neurodegenerative diseases in patients with idiopathic REM sleep behavior disorder（iRBD）.Methods: A total of 179 patients with iRBD（79.1% males,mean age = 66.3 ± 9.8 years） were consecutively ***-five patients with Epworth Sleepiness Scale score ≥14 were defined as having ***,clinical,and polysomnographic data were compared between iRBD patients with and without *** risk of developing neurodegenerative diseases was examined using Cox proportional hazards ***: After a mean follow-up of 5.8 years（SD = 4.3 years）,50（27.9%） patients developed neurodegenerative *** was a significantly higher proportion of conversion in patients with EDS compared to those without EDS（42.2 % vs.23.1%,p =.01）.EDS significantly predicted an increased risk of developing neurodegenerative diseases（adjusted hazard ratios [HR] = 2.56,95% confidence interval [CI] 1.37 to 4.77） after adjusting for age,sex,body mass index,current depression,obstructive sleep apnea,and periodic limb movements during *** analyses demonstrated that EDS predicted the conversion of Parkinson’s disease（PD）（adjusted HR = 3.55,95% CI 1.59 to 7.89） but not dementia（adjusted HR = 1.48,95% CI 0.44 to 4.97）.Conclusions: EDS is associated with an increased risk of developing neurodegenerative diseases,especially PD,in patients with *** findings suggest that EDS is a potential clinical biomarker of α-synucleinopathies in iRBD.

Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely *** prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to *** Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke *** aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also ***,candidate genes were used for pathway and gene ontology enrichment *** Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of ***-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke *** of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke *** study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of ***,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic *** Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

Study Objectives: To determine the mortality and its risk factors in patients with rapid eye movement（REM） sleep behavior disorder（RBD）.Methods: A total of 205 consecutive patients with video-polysomnography confirmed RBD（mean age = 66.4 ± 10.0 y,78.5% males） were *** records and death status were systematically reviewed in the computerized records of the health care *** mortality ratio（SMR） was used to calculate the risk ratio of mortality in RBD with reference to the general ***: Forty-three patients（21.0%） died over a mean follow-up period of 7.1 ± 4.5 *** SMR was not increased in the overall sample（SMR（95% confidence interval[CI]） = 1.00（0.73-1.33））.However,SMR（95% CI） increased to 1.80（1.21-2.58） and1.75（1.11-2.63） for RBD patients in whom neurodegenerative diseases and dementia,respectively,eventually *** the Cox regression model,mortality risk was significantly associated with age（hazard ratio [HR] = 1.05;95% CI,1.01-1.10）,living alone（HR = 2.04;95% CI,1.39-2.99）,chronic obstructive pulmonary disease（HR =3.38;95% CI,1.21-9.46）,cancer（HR = 10.09;95% CI,2.65-38.42）,periodic limb movements during sleep（HR = 3.06;95% CI,1.50-6.24）,and development of neurodegenerative diseases（HR = 2.84;95% CI,1.47-5.45） and dementia（HR = 2.66;95% CI,1.39-5.08）.Conclusions: Patients with RBD have a higher mortality rate than the general population only if neurodegenerative diseases *** risk factors on clinical and sleep aspects are associated with mortality in RBD *** findings underscore the necessity of timely neuroprotective interventions in the early phase of RBD before the development of neurodegenerative diseases.

BACKGROUND： Gender differences have been reported in some common mental disorders. However, few studies have monitored gender differences in individuals with delirium. OBJECTIVE： To explore gender differences in challenging behaviors, management and outcomes in age-matched elderly patients with ***, TIME AND SETTING： A retrospective cohort study was performed in the Bankstown-Lidcombe Hospital, Sydney, Australia, from October 2008 to April 2009. METHODS： Patients, aged 65-90 years, diagnosed with delirium according to the International Classification of Diseases （ICD）-10 in the Psychogeriatric Unit of Bankstown Lidcombe Hospital from January 2002 to October 2008 were reviewed. All the patients were measured according to the Confusion Assessment Method upon admission. Those who developed delirium during hospitalization were *** OUTCOME MEASURES： Cause of delirium, wandering, aggression, duration of delirium, physical restraint, use of antipsychotic medicine, recovery from delirium, discharge back home, length of stay, one-to-one nursing care, falls and absconding ***： The 131 age-matched delirious patients comprised 54 males and 77 females. The behavioral disorders of wandering [odds ration （OR） = 2.612, 95% confidence interval （CI） = 1.26 -5.413, P = 0.009] and aggression （OR = 2.243, 95% CI = 1.028 - 4.891, P= 0.04） were more frequent in males than in females. More males received one-to-one nursing care （OR = 4.114, 95% CI = 1.355 - 12.491, P = 0.008）, were more likely to receive antipsychotic medications （OR = 2.24, 95% CI = 1.095-4.583, P = 0.021） and more likely to be physically restrained （OR = 2.07, 95% CI = 1.02-1.02, P = 0.043） compared with female patients. All absconding patients （3/131, 2.3%） were male. Combinatorial Complexity in a Transcriptionally Centered Signaling Hub in Arabidopsis

A subfamily of four Phytochrome （phy）-Interacting bHLH transcription Factors （PIFs） collectively promote skotomorphogenic development in dark-grown seedlings. This activity is reversed upon exposure to light, by photoacti- vated phy molecules that induce degradation of the PIFs, thereby triggering the transcriptional changes that drive a tran- sition to photomorphogenesis. The PIFs function both redundantly and partially differentially at the morphogenic level in this process, To identify the direct targets of PIF transcriptional regulation genome-wide, we analyzed the DNA-binding sites for all four PIFs by ChlP-seq analysis, and defined the genes transcriptionally regulated by each PIF, using RNA-seq analysis of pif mutants. Despite the absence of detectable differences in DNA-binding-motif recognition between the PIFs, the data show a spectrum of regulatory patterns, ranging from single PIF dominance to equal contributions by all four. Similarly, a broad array of promoter architectures was found, ranging from single PIF-binding sites, containing single sequence motifs, through multiple PIF-binding sites, each containing one or more motifs, with each site occupied prefer- entially by one to multiple PIFs. Quantitative analysis of the promoter occupancy and expression level induced by each PIF revealed an intriguing pattern. Although there is no robust correlation broadly across the target-gene population, examination of individual genes that are shared targets of multiple PIFs shows a gradation in correlation from strongly positive, through uncorrelated, to negative. This finding suggests a dual-layered mechanism of transcriptional regulation, comprising both a continuum of binding-site occupancy by each PIF and a superimposed layer of local regulation that acts differentially on each PIF, to modulate its intrinsic transcriptional activation capacity at each site, in a quantitative pattern that varies between the individual PIFs from gene to gene. ESTIMATION AND UNCERTAINTY QUANTIFICATION FOR PIECEWISE SMOOTH SIGNAL RECOVERY

This paper presents an application of the sparse Bayesian learning(SBL)algorithm to linear inverse problems with a high order total variation(HOTV)sparsity *** the problem of sparse signal recovery,SBL often produces more accurate estimates than maximum a posteriori estimates,including those that useℓ1 ***,rather than a single signal estimate,SBL yields a full posterior density estimate which can be used for uncertainty ***,SBL is only immediately applicable to problems having a direct sparsity prior,or to those that can be formed via *** paper demonstrates how a problem with an HOTV sparsity prior can be formulated via synthesis,and then utilizes *** expands the class of problems available to Bayesian learning to include,e.g.,inverse problems dealing with the recovery of piecewise smooth functions or signals from *** examples are provided to demonstrate how this new technique is effectively employed.

Maintenance therapy in advanced non-small cell lung cancer： a prime-time for change？

Lung cancer is the leading cancer in terms of incidence and mortality in China and its incidence in China is predicted to increase in the next 20 years.1 The majority of the lung cancer patients are diagnosed with advanced stage disease,for which chemotherapy or targeted therapies are the mainstay palliative *** the identification of single-driver mutations,like the epidermal growth factor receptor （EGFR） mutation,platinum-based doublet was the standard first line treatment for advanced stage nonsmall cell lung cancer （NSCLC） patients with a good performance status （PS）.However,patient prognosis remains poor and the modest treatment efficacy seems to have reached a plateau despite newer generation of platinum doublets.2 The recommendation for the duration of first line platinum doublet treatment is 4-6 cycles.3 The paradigm was based on the fact that a protracted course of chemotherapy did not prolong survival but introduced cumulative toxicities.4,5 The use of maintenance therapy （MT）,defined by Grossi et al6 as the prolongation of chemotherapy with the administration of additional drugs at the end of a defined number of initial chemotherapy cycles after achieving maximum tumor response,was not considered as a standard option in the past.
