Impact of gender and race on expectations and outcomes in total knee arthroplasty

BACKGROUND Total joint arthroplasty is one of the most common surgeries performed in the United States with total knee arthroplasty(TKA)being one of the most successful surgeries for restoring function and diminishing *** with the demonstrated success of TKA and a higher prevalence of arthritis and arthritis related disability among minorities,racial and gender disparity remains a constant issue in providing care for the adult reconstruction *** To assess the role of demographics and expectations on differences in perioperative patient reported outcomes(PRO)following *** One hundred and thirty-three patients scheduled for primary unilateral TKA secondary to moderate to severe osteoarthritis were enrolled in this twoinstitution prospective *** PRO questionnaires were collected at four time *** analysis was conducted to determine the impact of gender,ethnic background and expectation surveys responses to assess PRO at these time *** Females were associated with worse preoperative Knee Injury and Osteoarthritis Outcome Scores(KOOS)for symptoms,pain,and activities of daily *** Americans were associated with worse KOOS for pain,activities of daily living,and quality of *** worse preoperative scores,no difference was noted in these categories between the groups ***,all pre-operative psychometric scales were equivalent across groups except Geriatric Depression scale,which was significantly different between groups within the Race and Age Group(P<0.05).Conversely,Pain Catastrophizing Scale,was significantly different for all subscales and total score within Age Group(P<0.05),and the Magnification,Helplessness subscales as well as the Total score were significantly different between groups for Race and Relationship Status(P<0.01).CONCLUSION We conclude that female and African American patients have lower preoperative KOOS scores compared to white male *** postoper

This study aims to quantitatively explore the multifaceted determinants that influence earnings in the translation *** a dataset comprising 45000 translator profiles,the study focusses on delineating disparities correlated with demographic variables such as age,gender,home country wealth,and language *** study uses a random forest regression model to delineate the complex interaction between gender,the economic standing of a translator’s domicile country,age,and linguistic proficiency,as they relate to *** findings substantiate and,in many ways,extend existing qualitative and anecdotal evidence that has shaped the discourse in this *** rigorous empirical framework employed here can be replicated or adapted to study other sectors within the gig economy,thus contributing to a more comprehensive understanding of labour dynamics in the digital age.

Rethinking Global Health Governance in a Changing World Order for Achieving Sustainable Development:The Role and Potential of the'Rising Powers'

The idea of this paper is inspired by the dismal experience and lessons from the initially ineffective global(WHO-led)response to the 2014-2016 West African Ebola virus *** charts the evolution of global health policy and governance in the post-World War II international order to the current post-2015 UN Sustainable Development Goals *** order to respond adequately existing and emerging health and development challenges across developing regions,the paper argues that global health governance and related structures and institutions must adapt to changing socio-economic circumstances at all levels of *** the background of a changing world order characterised by the decline of US-led Western international liberalism and the rise of the emerging nations in the developing world,it identifies the 4 Rising Powers'(RPs)among the emerging economies and their soft power diplomacy and international development cooperation strategy as important tools for responding to post-2015 global health *** on analysis of illustrative examples from the'BRICS;a group of large emerging economies-Brazil,Russia,India,China and South Africa-the paper develops suggestions and recommendations for the RPs with respect to:(1)stimulating innovation in global health governance and(2)strengthening health systems and health security at country and regional *** that current deliberations on global health focus largely,but rather narrowly,on what resource inputs are needed to achieve the SDG health targets,this paper goes further and highlights the importance of the in terms of a leadership and driving role for the RPs:How can the RPs champion global governance reform and innovation aimed at producing strong,resilient and equitable global systems?How can the RPs use soft power diplomacy to enhance disease surveillance and detection capacities and to promote improved regional and international coordination in response to health threats?How can they

On-site solar PV generation and use: Self-consumption andself-sufficiency

As energy storage systems are typically not installed with residential solar photovoltaic (PV) systems,any “excess” solar energy exceeding the house load remains unharvested or is exported to the *** paper introduces an approach towards a system design for improved PV self-consumptionand self-sufficiency. As a result, a polyvalent heat pump, offering heating, cooling and domestic hotwater, is considered alongside water storage tanks and batteries. Our method of system analysisbegins with annual hourly thermal loads for heating and cooling a typical Australian house inGeelong, Victoria. These hourly heating and cooling loads are determined using Transient SystemSimulation (TRNSYS) software. The house’s annual hourly electricity consumption is analysed usingsmart meter data downloaded from the power supplier and PV generation data measured with aPV system controller. The results reveal that the proposed system could increase PV self-consumptionand self-sufficiency to 41.96% and 86.34%, respectively, resulting in the annual imported energybeing reduced by about 74%. The paper also provides sensitivity analyses for the hot and coldstorage tank sizes, the coefficient of performance of the heat pump, solar PV and battery *** establishing the limits of thermal storage size, a significant impact on self-efficiency can berealised through battery storage. This study demonstrates the feasibility of using a polyvalent heatpump together with water storage tanks and, ultimately, batteries to increase PV self-consumptionand self-sufficiency. Future work will concentrate on determining a best-fit approach to systemsizing embedded within the TRNSYS simulation tool.

Partial overlap of anti-mycobacterial,and anti-Saccharomyces cerevisiae mannan antibodies in Crohn's disease

AIM： To test whether humoral immune reaction against mycobacteria may play a role in anti- Saccharomyces cerevisiae antibodies （ASCA） generation in Crohn＇s disease （CD） and/or whether it correlates with clinical subtypes. METHODS： The dominant ASCA epitope was detected by Galanthus nivalis lectin （GNL）-binding assay. ASCA and IgG against mycobacterial lysates （M avium, M smegmatis, M chelonae, M bovis BCG M avium ssp. paratuberculosis （MAP）] or purified lipoarabinomannans （LAM） were detected by ELISA. ASCA and anti-mycobacterial antibodies were affinity purified to assess cross-reactivities. Anti-mycobacterial IgG were induced by BCG-infection of mice. RESULTS： GNL bound to different extents to mycobacterial lysates, abundantly to purified mannosecapped （Man） LAM from M tuberculosis, but not to uncapped LAM from M srnegrnatis. Fifteen to 45% of CD patients but only 0%-6% of controls were seropositive against different mycobacterial antigens. Anti-mycobacterial IgG correlated with ASCA （r = 0.37-0.64; P = 0.003-P 〈 0.001）. ASCA-positivity and deficiency for mannan-binding lectin synergistically associated with anti-mycobacterial IgG. In some patients, anti-mycobacterial antibodies represent crossreactive ASCA. Vice-versa, the predominant fraction of ASCA did not cross-react with mycobacteria. Finally, fistulizing disease associated with antibodies against M avium, M smegmatis and MAP （P = 0.024, 0.004 and 0.045, respectively）. CONCLUSION： Similar to ASCA, seroreactivity against mycobacteria may define CD patients with complicated disease and a predisposition for immune responses against ubiquitous antigens. While in some patients anti-mycobacterial antibodies strongly cross-react with yeast mannan; these cross-reactive antibodies only represent a minor fraction of total ASCA. Thus, mycobacterial infection unlikely plays a role in ASCA induction.

Effects of Placental Transfusion by Delayed Cord Clamping or Umbilical Cord Milking versus Immediate Cord Clamping on Neonatal Outcomes in Very Low Birth Weight Neonates

Background: Delayed cord clamping (DCC) and umbilical cord milking (UCM) are two recently advocated interventions to enhance placental transfusion at birth. DCC and MCM might increase blood volume and help neonates to transition from fetal to neonatal circulation. Objective: To evaluate the effects of delayed cord clamping (DCC), umbilical cord milking (UCM), and immediate cord clamping (ICC) on very low birth weight (VLBW) neonates. Methods: At our institution, ICC was routinely done until December, 2012 when it was replaced by DCC. UCM was implemented in March, 2015. Neonatal outcomes were compared among ICC, DCC, and UCM. Results: A total of 94 neonates met inclusion criteria. Comparing with ICC neonates, DCC and UCM neonates had higher hematocrits, fewer blood transfusions, and lower incidence of chronic lung disease at 36 completed weeks of gestation. All groups were similar in APGAR scores, vasopressor use, intraventricular hemorrhage (IVH) and necrotizing enterocolitis (NEC). Conclusion: Compared to ICC, DCC and UCM appear to be more beneficial to VLBW neonates without apparent adverse risks.

Bioadsorption of 4-Chlorophenol to the Activated Sludge

The adsorption behaviour of 4 chlorophenol from aqueous solution to activated sludge was quantitatively characterized in this paper. The effects of the initial pH values, initial chlorophenol concentration and adsorbent dosage on bioadsorption were investigated. The maximum adsorption capacity was found to be 110.5 mg/g at 100 mg/L initial concentration. The Freundlich and Langmuir adsorption isotherms were applied to describe the biosorption processes and the isotherm constants were evaluated. 

West Africa comprises 16 countries including *** Nigeria,there are two seasons;the dry season where midday temperature surpasses 38°C and rainy season with midday temperature as low as 12°*** losses are generated from every step of food and animal processing due to low technological *** West African countries have very high fraction of organic components in their waste stream(up to 56 percent) and are currently disposed of in dumpsites and in some cases uncontrolled *** average temperature,food and animal waste are major potentials for the anaerobic digester with energy *** economic appraisal of a dumpsite in Nigeria was carried out using the net present value(NPV) technique and adopting a shadow price of carbon of $10(1,557 naira).A negative NPV of $0.36million(56,107,144.33 naira) was *** a substantial portion of land can be reclaimed by adopting anaerobic digesters.
