BACKGROUND Ureteropelvic junction obstruction(UPJO)is a common congenital urinary tract disorder in *** can be diagnosed as early as in utero due to the presence of hydronephrosis or later in life due to symptomatic *** To evaluate the discrepancy between dynamic contrast-enhanced magnetic resonance urography(dMRU)and scintigraphy 99m-technetium mercaptoacetyltriglycine(MAG-3)for the functional evaluation of *** Between 2016 and 2020,126 patients with UPJO underwent surgery at Robert Debré*** these,83 received a prenatal diagnosis,and 43 were diagnosed during *** of the 126 patients underwent surgery based on the clinical situation and postnatal ultrasound findings without undergoing functional imaging *** renal function was evaluated preoperatively using scintigraphy MAG-3(n=28),dMRU(n=53),or both(n=40).In this study,we included patients who underwent surgery for UPJO and scintigraphy MAG-3+dMRU but excluded those who underwent only scintigraphy MAG-3 or *** patients were divided into groups A(10%discrepancy).We examined the discrepancy in split renal function between the two modalities and investigated the possible risk *** The split renal function between the two kidneys was compared in 40 patients(28 boys and 12 girls)using scintigraphy MAG-3 and *** renal function,as determined using both modalities,showed a difference of10%in 9 *** of the relative renal function using dMRU revealed an excellent correlation coefficient with renal scintigraphy MAG-3 for both *** Our findings demonstrated that dMRU is equivalent to scintigraphy MAG-3 for evaluating split renal function in patients with UPJO.

Tunneled tubularized bladder mucosal graft for long urethral reconstruction:is it a reliable option in selected complex cases in children and adolescents?

The reduction of nitrate to nitrite by the oral microbiota has been proposed to be important for oral health and results in nitric oxide formation that can improve cardiometabolic conditions. Studies of bacterial composition in subgingival plaque suggest that nitrate-reducing bacteria are associated with periodontal health, but the impact of periodontitis on nitrate-reducing capacity(NRC)and, therefore, nitric oxide availability has not been evaluated. The current study aimed to evaluate how periodontitis affects the NRC of the oral microbiota. First, 16S rRNA sequencing data from five different countries were analyzed, revealing that nitratereducing bacteria were significantly lower in subgingival plaque of periodontitis patients compared with healthy individuals(P < 0.05 in all five datasets with n = 20–82 samples per dataset). Secondly, subgingival plaque, saliva, and plasma samples were obtained from 42 periodontitis patients before and after periodontal treatment. The oral NRC was determined in vitro by incubating saliva with 8 mmol/L nitrate(a concentration found in saliva after nitrate-rich vegetable intake) and compared with the NRC of 15healthy individuals. Salivary NRC was found to be diminished in periodontal patients before treatment(P < 0.05) but recovered to healthy levels 90 days post-treatment. Additionally, the subgingival levels of nitrate-reducing bacteria increased after treatment and correlated negatively with periodontitis-associated bacteria(P < 0.01). No significant effect of periodontal treatment on the baseline saliva and plasma nitrate and nitrite levels was found, indicating that differences in the NRC may only be revealed after nitrate intake. Our results suggest that an impaired NRC in periodontitis could limit dietary nitrate-derived nitric oxide levels, and the effect on systemic health should be explored in future studies.

Transition to school for children with autism spectrum disorder:A systematic review

近年来,脑力负荷估计已经经历了广泛的研究,因为监测认知负荷的能力能够防止认知超负荷并且改善工作场所安全。脑电图(EEG)信号已经被发现是一种客观和非侵入性的脑力负荷的测量方式。然而,作为实时脑力负荷监测和脑机接口研究的重要一步,基于单试验EEG数据的认知负荷的评估一直是一个重大的挑战。最近,许多高级的特征提取方法和机器学习算法已经被采用于基于EEG的脑力负荷评估中。在本研究中,使用在具有2个难度水平的n-back任务的执行期间记录的EEG数据进行了单试验脑力负荷分类,测试了3种类型的特征提取的有效性(谱功率、离散小波变换和公共空间滤波),并评估了4种分类算法的性能(支持向量机、K-近邻、随机森林和梯度推进分类器)。研究结果表明,公共空间滤波是性能最好的基于单试验的脑力负荷分类的特征提取方法,而且最佳性能可以通过将来自谱功率或离散小波变换的特征与来自公共空间滤波的特征相结合,并采用随机森林分类器来实现。这项研究可能对基于单试验脑电图数据的脑力负荷评估中的特征提取方法以及机器学习算法的选择提供一些有用的指导。

Coagulopathy in liver disease:Lack of an assessment tool

There is a discrepancy between the information from clotting tests which have routinely been used in clinical practice and evidence regarding thrombotic and bleeding events in patients with liver disease. This discrepancy leads us to rely on other variables which have been shown to be involved in haemostasis in these patients and/or to extrapolate the behaviour of these patients to other settings in order to decide the best clinical approach. The aims of the present review are as follows:(1) to present the information provided by clotting tests in cirrhotic patients;(2) to present the factors that may influence clotting in these patients;(3) to review the clinical evidence; and(4) to put forward a clinical approach based on the first 3 points.
