Objective:Neonates with congenital heart disease(CHD)and perinatal stroke have high mortality and survivors are at risk for poor long-term neurodevelopmental *** aim of this study was to assess the risk factors and outcome of neonates with both CHD and MRI-confirmed perinatal stroke(Study Group)and compare those to the risk factors and outcome of infants matched for CHD without stroke(Control-1)and of infants matched for MRI-confirmed stroke without CHD(Control-2).Methods:We conducted a population-based case-control study enrolling 28 term neonates with CHD and MRI-confirmed acute perinatal stroke born between 2007–2017 in the Central-Hungarian *** of the control groups included 56 *** Bayley Scales of Infant Development-II,the Brunet-Lézine test and the Binet Intelligence scales-V were used for neurodevelopmental follow-up at a median age of 61 ***:Mortality was highest in the Study Group(25%compared to 5%and 2%,respectively,p=0.001).Adverse neurodevelopmental outcome was prevalent in the Study(53%)and Control-2 Groups(52%,p=0.03).Significantly different parameters among the three groups included Apgar scores,mode of delivery,gestational age at birth,cardiac interventions and twin *** a multivariable regression analysis adjusted for clinically relevant parameters,patients in the Study Group had significantly higher odds for mortality compared to patients in the Control-1 Group(OR:6.595%CI:1.1–39.4).Conclusions:Neonates with perinatal stroke and CHD are at a higher risk for dying compared to neonates with CHD without *** addition,the stroke-associated direct insult to the brain likely plays an important role in the development of neurodevelopmental morbidity in these patients.

China＇s gradual integration with the global market is responsible for exceptional rates of economic growth and for the substantial sociopolitical stability enjoyed by the country over the past decades. Nevertheless, the choice to increasingly adhere to the competitive dynamic of the market has thrown up a battery of problems for China＇s leadership, as increasing affluence has favored the pluralization and diversification of social interests. In a country where the existence of divergent and competing social claims has traditionally been regarded as a grave threat to stability, marketization, whereas on the one hand acting as a source of political legitimacy, has, on the other, paved the way for a potentially more strained relationship between relevant corporative interests and the central government. By studying China＇s recent development from the complementary perspectives of the ＂horizontal＂ and ＂vertical＂ forms of decentralization and devolution, this paper aims to highlight the tensions with which China has had to contend since its adoption of reform and opening-up policies in the late 1970s. It aims also to suggest how the Chinese leadership＇s instinct for continued social control has cut against market reform just enough to effect a process of controlled macroeconomic integration that has been more successful than wholesale resistance or immediate unquestioning acceptance would have been.

The study was aimed at investigating the phytochemicals,antioxidant and antibacterial activities of the ethanolic and methanolic extracts of Aloe elegans leaves *** phytochemical screening revealed the presence of phenols,flavonoids,anthraquinones,tannins,sapponins and terpenoids in both *** antioxidant activities of both extracts were assayed using ferric reducing antioxidant power(FRAP),conjugated diene(CD)and peroxide value(PV)methods using sunflower oil(SFO)as the oxidizable *** value for methanol and ethanol extracts of Aloe elegans leaves gel and the standard ascorbic acid in the ferric reducing power assay at 20μg/mL were found to be 65.32%,62.89%,59.4% *** peroxide value after 12 days at room temperature for the Sunflower oil(SFO)was found to be 45 mmol/L while the value for the substrate containing methanol,ethanol extract and vitamin E were found to be 36,38,43 mmol/L *** value of the absorption coefficients of conjugated dienes resulted from oxidations of the methanol and ethanol extracts and vitamin E were found to be 3.006,3.387,4.306 g^-1cm^-1mL *** all the antioxidant assays used,the methanolic extract was found to inhibit oxidation of sunflower oil to a better extent than its ethanolic *** to vitamin E,both extracts exhibited better antioxidant *** the antibacterial assay,both the extracts showed potent bactericidal effects against Escherichia coli and Staphyloccus aureus,though to a lesser extent than the standard antibiotic Gentamicin.

＜正＞Background: The role of surgical resection and thrombectomy for hepatocellular carcinoma （HCC） with portal vein tumor thrombus （PVTT） is controversial. This study aimed to evaluate the effects of the location and extent of PVTT on the long-term outcomes of surgical treatment for HCC. Methods: A total of 438 patients with HCC and PVTT underwent liver resection with or without thrombectomy. These 438 patients were divided into 2 groups: in group A, PVTT was located in the hepatic resection area or protruded into the first branch of the main portal vein beyond the resection line for ＜ 1 cm （286 patients）, and in group B, PVTT extended into the main portal vein （1S2 patients）. Concomitant thrombectomy was performed in 147 patients （51.4%） of group A and in all patients of group B. Results: PVTT recurrence within 6 months after surgery in group B was significantly higher than that in group A: 76.9% vs. 11.3%. Remnant liver recurrence within 1 year after surgery was 43.0% in group A and 78.8% in group B. The cumulative 1-, 2-. 3-, and 5-year overall survival rates were 58.7%, 39.9%, 22.7%, and 18.1% for group A and 39.5%. 20.4%, 5.7%, and 0% for group B, respectively. The overall survivals were significantly better in group A than group B （P ＜ .02）. Conclusions: Liver resection with thrombectomy yielded better outcomes in the HCC patients with PVTT confined to the first or second branch of the main portal vein compared with PVTT extending into the main portal vein.

A) What should be known about supplemental glutamine-(dipeptides) ?In numerous controled clinical trials it could be convincingly demonstrated that supp1emental glutamine- (di pep t ide ) nutrt ion was associated with improved nitrogen balance and mdri; increased protein synthesis and lyTnphocyte count (total and subpopulation); maintained intestinal function, intracellular muscel free glutamine Pool,gut permeability and function, reduced 3MeHis excretion, morbidity, length of hoSPital stay and sepsis frequency. A current European multi center study with ala-gln provides convincing evidence that the clinical use of glutamine dipeptide spares nitrogen and shortens length of hospital stay as compared with the controls. The infusion of the glutamine containing dipeptide soution was free of any side effects, and recovery was normal for each patient. Provision of glutamine- (dipepti de ) should be consi dered as a replacement of a de fic iency rat her t han a supplement. This implies that the beneficial effects observed are due to a correction of disadvantages preduced by an inadequacy of conventional nutrition.B)What is new in glutamine (dipeptide) research?1)It is pu^ed that tissue g1utathione synthesis is a crucial factor in causing reversal of the clinical,biochemical signs of critical illness, a condition associated with decreased glutathione status. lndeed, the decreased ratio GSH/Gase suggests oxidative stress in the tissues.
