Immune mechanisms and immuno -therapy in sepsis:where do we stand ?

Sepsis and septic shock have continued to produce significant morbidity and mortality, in recent times, in acutely injured patients as well as patients in the intensive care units despite advances in antibiotic therapy and in the cardi ovascular and pulmonary support for these patients. This emphasizes the need to gain a better understanding of the fundamental mechanisms of the pathogenesis of sepsis syndrome in the critically ill or injured patients. The present knowled ge of the mechanisms of septic pathogenesis clearly indicates involvement of mod ulations in the functions of the cells of body's immune defense system, namely, monocytes/macrophages, polymorphonuclear leukocytes, and lymphocytes. Most of su ch knowledge has been derived from laboratory experiments in animal models of se ptic injury, or from studies of immune-system cells, in vitro. Clinical stud ies have also supported the role of immune perturbations. Yet, to date, very few the rapeutic approaches are available to effectively counter the sepsis-related immu ne disturbances. Functional modulations in the immune system cells, in the injured/septic hos ts, can exert not only adaptive/beneficial effects but also profoundly adverse effects on the non-immune-system cells such as endothelial, epithelial, neurona l, endocrine, neuro-endocrine, smooth muscle, skeletal muscle, and cardiac muscl e cells. The primary functional modulation in the immune-system cells after inju ry or with critical illness is activation of such cells via molecules from pat hogens, for example, lipopolysaccharide from gram negative organisms, lipoteic hoic acid/peptidoglycan from gram positive organisms, or zymosan from fungi. Suc h pathogenic molecules activate monocytes and tissue macrophages to result in th e expression and release of cytokine mediators (TNFα, IL-1, IL-6, IL-8, and IL- 10), as well as certain lipid mediators (PGE2, LTB4, PAF). A new framework for evaluating along-wind responses of a transmission tower

In this paper, an analytical framework to evaluate the along-wind-induced dynamic responses of a transmission tower is presented. Two analytical models and a new method are developed：（1） a higher mode generalized force spectrum （GFS） model of the transmission tower is deduced; （2） an analytical model that includes the contributions of the higher modes is further derived as a rational algebraic formula to estimate the structural displacement response; and （3） a new approach, applying load with displacement （ALD） instead of force, to solve the internal force of transmission tower is given. Unlike conventional methods, the ALD method can avoid calculating equivalent static wind loads （ESWLs）. Finally, a transmission tower structure is used as a numerical example to verify the feasibility and accuracy of the ALD method.

This is a record of biostatistical data collected during the years 1981-84 at a clinic in the United States, from patients who came to seek acupuncture treatment. The records consist of patient referral, age, sex, main problem, and clinical diagnosis. The information obtained from such records may be of interest to other acupuncture practitioners. It is also hoped that this report will provide useful data in comparing the profiles of those patients in the East and West who seek acupuncture treatment.

A new look at the response surface method for reliability analysis using chaos theory

To overcome excessive computation errors and convergence failures encountered in an iterative calculation of the reliability index using the response surface method （RSM） for some nonlinear limit state functions, this study investigates an essential factor based on chaotic dynamics theory. The bifurcation diagrams of the reliability index are presented for some typical nonlinear limit state functions, and the computation results from the mapping functions due to the RSM iterations show the complicated dynamic phenomena such as the periodic oscillation, as well as bifurcation and chaos. From the numerical examples, it is concluded that the parameter of selection range fplays an important role in the convergence of the RSM iteration, and an improved RSM iterative algorithm is proposed with regard to the incorporation of the iterative sequential function of selection rangef The proposed method is shown to be efficient and to yield accurate results.

Halogen-specific total organic halogen analysis： Assessment by recovery of total bromine

Determination of halogen-specific total organic halogen（TOX） is vital for studies of disinfection of waters containing bromide, since total organic bromine（TOBr） is likely to be more problematic than total organic chlorine. Here, we present further halogen-specific TOX method optimisation and validation, focusing on measurement of TOBr. The optimised halogen-specific TOX method was validated based on the recovery of model compounds covering different classes of disinfection by-products（haloacetic acids, haloacetonitriles,halophenols and halogenated benzenes） and the recovery of total bromine（mass balance of TOBr and bromide concentrations） during disinfection of waters containing dissolved organic matter and bromide. The validation of a halogen-specific TOX method based on the mass balance of total bromine has not previously been reported. Very good recoveries of organic halogen from all model compounds were obtained, indicating high or complete conversion of all organic halogen in the model compound solution through to halide in the absorber solution for ion chromatography analysis. The method was also successfully applied to monitor conversion of bromide to TOBr in a groundwater treatment plant. An excellent recovery（101%）of total bromine was observed from the raw water to the post-chlorination stage. Excellent recoveries of total bromine（92%–95%） were also obtained from chlorination of a synthetic water containing dissolved organic matter and bromide, demonstrating the validity of the halogen-specific TOX method for TOBr measurement. The halogen-specific TOX method is an important tool to monitor and better understand the formation of halogenated organic compounds, in particular brominated organic compounds, in drinking water systems.

Physiologic magnetic fields on the order of magnitude 10-8gauss have been unified with their propitiators: quantum genetic particles, the gravitational potential of which is about a few ergs. As these fields are applied to the equations for solenoidal models, B=μNI/L, currents of about a microampere are derived; in perfect accord with recent clinical data indicating the therapeutic efficacy of weak currents in repair and growth of soft tissue, bone and nerve. The mechanism of reorientation of spin angular momentum of leptons and baryons influencing molecular magnetic domains to bring about "particle jumps" is presented to obtain a clinical picture. The clinical picture is that of an organism placed at right angles to flux lines in the midst of a solenoid immersed in water. The patient is then exposed to exogenously sourced resonant physiologic magnetic fields which convert malalligned atomic lattices of oncogenes and associated particles to homologous normal structures.

Statistical damage detection method for frame structures using a confidence interval

A novel damage detection method is applied to a 3-story frame structure, to obtain statistical quantification control criterion of the existence, location and identification of damage. The mean, standard deviation, and exponentially weighted moving average （EWMA） are applied to detect damage information according to statistical process control （SPC） theory. It is concluded that the detection is insignificant with the mean and EWMA because the structural response is not independent and is not a normal distribution. On the other hand, the damage information is detected well with the standard deviation because the influence of the data distribution is not pronounced with this parameter. A suitable moderate confidence level is explored for more significant damage location and quantification detection, and the impact of noise is investigated to illustrate the robustness of the method.

Neurosurgery is a highly specialized field:it often involves surgical manipulation of noble structures and cerebral retraction is frequently necessary to reach deep-seated brain *** are still no reliable methods preventing possible retraction *** objective of this study was to design work chambers well suited for transcranial endoscopic surgery while providing safe retraction of the surrounding brain *** chamber is designed to be inserted close to the intracranial point of interest;once it is best placed it can be *** should guarantee an appreciable workspace similar to that of current neurosurgical *** experimental aspect of this study involved the use of a force sensor to evaluate the pressures exerted on the brain tissue during the retraction *** pterional craniotomy,pressure measurements were made during retraction with the use of a conventional metal spatula with different *** that,although the force values necessary for retraction and exerted on the spatula by the neurosurgeon are the same,the local pressure exerted on the parenchyma at the edge of the spatula at different inclinations varied greatly.A new method of cerebral retraction using a chamber retractor(CR)has been designed to avoid any type of complication due to spatula edge overpressures and to maintain acceptable pressure values exerted on the parenchyma.
