BACKGROUND Several cutaneous manifestations such as urticarial rash,erythematous patches and chilblain-like lesions have been described in young adults with coronavirus disease 2019(COVID-19)and are present in up to 20%patients,but few reports exist describing histopathological and immunophenotypic characteristics of dermatological lesions in older *** aim was to characterize skin lesions in elderly patients during late stages of COVID-19 from clinical,histological and immunophenotypic *** SUMMARY Three patients,admitted for COVID-19,and who developed cutaneous manifestations underwent skin *** analysis for CD20,CD3,CD4 and CD8 was performed on skin biopsies to assess immune cell ***1a was used as a marker of Langerhans cells,and CD31 as a marker of endothelial *** the three study patients,cutaneous manifestations were evident in the late-stage of COVID-19(mean time from the first positive severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)swab to rash onset was 35 d).Skin biopsies showed a similar pattern of T lymphocyte infiltration in all ***,a chronic dermatitis with perivascular lymphocytic infiltrate was observed with predominance of CD3+T-cell(CD3+).CONCLUSION Our study confirms previous *** and immunophenotypic patterns in our patients confirm results described in the two previous reported *** pattern is similar to what is found in some lympho-proliferative ***,since these findings are non-specific,SARS-CoV-2 infection should be suspected.

崇明东滩旱作农田土壤磷素流失及其影响因素

对采用不同施肥和地表管理方式的梨园和菜地磷素的流失进行监测,研究了崇明东滩旱作农田土壤磷素的流失负荷、流失途径及其变化规律和影响因素,并分析了不同施肥和地表管理方式对农田土壤磷素流失的影响。结果表明,旱作农田土壤磷素径流流失、渗漏流失和泥沙流失浓度均超过水体富营养化总磷浓度临界值;土壤磷素流失以泥沙流失为主,泥沙总磷流失负荷占总磷流失总负荷的70.7%～82.2%;磷素流失主要发生在6、8和10月,这3个月总磷流失负荷占全年总磷流失总负荷的71.6%～73.0%。对磷素流失影响因素的相关分析表明,旱作农田土壤磷素流失与降雨量、降雨侵蚀力、径流量、径流水中总磷浓度等因素相关显著。与常规对照方式相比较,保持表土植被、覆盖地膜和精确滴灌,可减少旱作农田径流和泥沙产生量,降低总磷流失浓度,梨园和菜地总磷流失负荷消减率分别可达20.6%和12.6%。

Background: Acute hepatitis C virus infection leading to fulminant hepatitis is very rare whereas Hepatitis C virus (HCV) infection is one of the main causes of chronic liver disease worldwide. The repertoire of substances that accumulate in the blood in fulminant hepatic failure cause neurological abnormalities, aggravate injury to the liver and other organs, suppress the ability of residual hepatocytes to perform organ-specific functions (sick cell syndrome), and inhibit the hepatic regenerative response especially in fulminant hepatitis Virus C which has subacute clinical evolution and takes time to manifest. Liver support technology is evolving as different techniques become available that assist the remaining functional cell mass by providing specific liver functions. Case Presentation: We report a case of Fulminant C virus Hepatitis, successfully treated with albumin dialysis Molecular Adsorbent Recirculating System (MARS). At time of admittance the patient presented: Model End-stage Liver Disease (MELD)-36;Child Turcotte Pugh (CTP)-C(13);Sequential Organ Failure Assestment (SOFA)-12, Glasgow Coma Score (GCS)-11. The patient underwent six sessions of MARS in Intensive Care Unit (ICU) in association with standard medical therapy (SMT). The patient survived and was discharged from the hospital in good condition after 40 days without liver transplantation (LT).

AIM: To evaluate virological response to telaprevir or boceprevir in combination with pegylated interferon and ribavirin and resistance mutations to NS3/4A inhibitors in hepatitis C virus-human immunodeficiency virus(HCV-HIV) coinfected patients in a real life setting. METHODS: Patients with HCV genotype 1-HIV coinfection followed in Nice University Hospital internal medicine and infectious diseases departments who initiated treatment including pegylated interferon and ribavirin(Peg IFN/RBV) + telaprevir or boceprevir, according to standard treatment protocols, between August 2011 and October 2013 entered this observational study. Patient data were extracted from an electronic database(Nadis®). Liver fibrosis was measured by elastometry(Fibroscan®) with the following cut-off values: F0-F1: < 7.1 k Pa, F2: 7.1-9.5 k Pa, F3: 9.5-14.5 k Pa, F4: ≥ 14.5 k Pa. The proportion of patients with sustained virological response(SVR) twelve weeks after completing treatment, frequency and type of adverse events, and NS3/4A protease inhibitor mutations were described. RESULTS: Forty-one patients were included: 13(31.7%) patients were HCV-treatment na?ve, 22(53.7%) had advanced liver fibrosis or cirrhosis(Fibroscan stage F3 and F4); none had decompensated cirrhosis or hepatocellular carcinoma; all were receiving antiretroviral treatment, consisting for most them(83%) in either a nucleoside reverse-transcriptase inhibitor/protease inhibitor or/integrase inhibitor combination; all patients had undetectable HIV-RNA. One patient was lost to follow-up. SVR was achieved by 52.5% of patients. Five patients experienced virological failure during treatment and four relapsed. Seven discontinued treatment due to adverse events. Main adverse events included severe anemia(88%) and rash(25%). NS3/4A protease mutations were analyzed at baseline and at the time of virological failure in the 9 patients experiencing non-response, breakthrough or relapse. No baseline resistance mutation cou

The use of biological agents and immunomodulators for inflammatory bowel disease(IBD) is associated with an increased risk of opportunistic infections, in particular of viral or bacterial etiology. Despite the existence of international guidelines, many gastroenterologists have not adopted routine screening and vaccination in those patients with IBD, which are candidate for biologic therapy. Available strategies to screen, diagnose and prevent bacterial and viral infections in patients with IBD prior to start biological therapy are discussed in this review.
