In the last five years UK government policy has moved towards increasing the provision of urgent care in the *** meet these changes the emergency ambulance service is shifting from an organization designed to convey patients to hospital to a professional group capable of assessing urgency and delivering the appropriate treatment to the patient;’providing the right response,first time,in time’.This paper reports an ergonomic analysis of urgent care provision in acute and primary care ***-five patients presenting with six complaints(breathing difficulty,chest pain,head injury,laceration,neck pain and injury as a result of a fall) were observed at two emergency departments(acute) and one minor injuries unit(primary care).Data were collected and analysed using Link Analysis(LA) to explore layout and spatial issues affecting the treatment space and Hierarchical Task Analysis(HTA) to analyse procedural issues and equipment *** LA and HTA data were triangulated to identify requirements for the mobile treatment unit.A recommendation for a new mobile treatment unit(mobile pod) has been developed by comparing the clinical environments,tasks and technologies used to provide assessment,diagnosis and treatment for six urgent healthcare *** proposed mobile pod has been critically reviewed by a national group of ambulance managers with positive *** research will include a wider range of clinical complaints and the development of a full size mock-up to test and evaluate the efficiency and safety of the new clinical *** mobile pod development is part of the Smart Pods project which is also exploring the operational systems(macro) and portable pods(micro) to support the change in clinical activities.

Meta-analyses on prevalence of selected Parkinson’s nonmotor symptoms before and after diagnosis

Background:Nonmotor symptoms are common among patients with Parkinson’s disease(PD)and some may precede disease ***:We conducted a meta-analysis on the prevalence of selected nonmotor symptoms before and after PD diagnosis,using random-effect *** searched PubMed(1965 through October/November 2012)for the following symptoms:hyposmia,constipation,rapid eye movement sleep behavior disorder,excessive daytime sleepiness,depression,and *** studies were publications in English with original data on one or more of these ***:The search generated 2,373 non-duplicated publications and 332 met the inclusion criteria,mostly(n=320)on symptoms after PD *** all symptoms,the prevalence was substantially higher in PD cases than in controls,each affecting over a third of the *** was the most prevalent(75.5%in cases vs.19.1%in controls),followed by constipation(50%vs.17.7%),anxiety(39.9%vs.19.1%),rapid eye movement sleep behavior disorder(37.0%vs.7.0%),depression(36.6%vs.14.9%),and excessive daytime sleepiness(33.9%vs.10.5%).We observed substantial heterogeneities across studies and meta-regression analyses suggested that several factors might have contributed to ***,the prevalence estimates were fairly robust in several sensitivity *** 20 studies had data on any symptoms prior to PD diagnosis,but still the analyses revealed higher prevalence in future PD cases than in ***:These symptoms are common among PD patients both before and after *** studies are needed to understand the natural history of nonmotor symptoms in PD and their etiological and clinical implications.

Current practices and future prospects for the management of gallbladder polyps: A topical review

A gallbladder polyp is an elevation of the gallbladder mucosa that protrudes into the gallbladder lumen. Gallbladder polyps have an estimated prevalence in adults of between 0.3%-12.3%. However, only 5% of polyps are considered to be "true" gallbladder polyps, meaning that they are malignant or have malignant potential. The main radiological modality used for diagnosing and surveilling gallbladder polyps is transabdominal ultrasonography. However, evidence shows that other modalities such as endoscopic ultrasound may improve diagnostic accuracy. These are discussed in turn during the course of this review. Current guidelines recommend cholecystectomy for gallbladder polyps sized 10 mm and greater, although this threshold is lowered when other risk factors are identified. The evidence behind this practice is relatively low quality. This review identifies current gaps in the available evidence and highlights the necessity for further research to enable better decision making regarding which patients should undergo cholecystectomy, and/or radiological follow-up.

Continuous application of organic fertilizers can cause accumulation of organic phosphorus(P)in soil,especially in the lowmolecular-weight organic phosphorus(LMWOP)*** organic P pool represents a potentially important source of P for both plants and *** understand the effect of long-term fertilization(30 years)(P-rich soil)***(P-poor soil)on the bioavailability and fate of LMWOP in subtropical paddy soils,we determined the sorption and mineralization of 14 C-labeled adenosine,adenosine monophosphate(AMP),adenosine diphosphate(ADP),and adenosine triphosphate(ATP)in each *** contents of carbon,nitrogen,and P in the P-rich soil were more than two times greater than those in the P-poor *** mineralization rates of the LMWOP compounds were faster in the P-rich soil compared to the P-poor soil,and followed the order AMP>ADP>*** sterilized soil,all forms of adenosine-P were strongly sorbed to the solid phase and reached saturation in a short time,with the adsorbance increasing with the number of phosphate *** concluded that the mineralization of LMWOP compounds was repressed slightly by sorption to the solid phase,but only in the short ***,LMWOP compounds serve as readily available sources of C for microorganisms,making P available for themselves as well as for the ***,P accumulation and the progressive saturation of the P sorption sites in highly fertile soils may increase the potential risk of P runoff.

Introduction Clostridioides difficile infection(CDI)is a common gastrointestinal illness worldwide,with increased prevalence and worse outcomes in patients with inflammatory bowel disease(IBD)[1].Fecal microbiota transplant(FMT)is an effective treatment for recurrent CDIs(RCDIs)with minimal serious adverse events(SAEs)reported[2].While immunosuppressive medications increase susceptibility to infections,studies have shown that FMT does not increase SAE risk in immunocompromised patients(including IBD patients on immunosuppressive therapy)[3].

Adaptive Classification Methods for Predicting Transitions in the Nursing Workforce

Earlier analyses of transitions from licensed practical nurse (LPN) to registered nurse (RN) in the North Carolina (NC) nursing workforce in terms of 11 categorical predictors were limited by not considering parsimonious classifications based on these predictors and by substantial amounts of missing data. To address these issues, we formulated adaptive classification methods. Secondary analyses of data collected by the NC State Board of Nursing were also conducted to demonstrate adaptive classification methods by modeling the occurrence of LPN-to-RN transitions in the NC nursing workforce from 2001-2013. These methods combine levels (values) for one or more categorical predictors into parsimonious classifications. Missing values for a predictor are treated as one level for that predictor so that the complete data can be used in the analyses;the missing level is imputed by combining it with other levels of a predictor. An adaptive nested classification generated the best model for predicting an LPN-to-RN transition based on three predictors in order of importance: year of first LPN licensure, work setting at transition, and age at first LPN licensure. These results demonstrate that adaptive classification can identify effective and parsimonious classifications for predicting dichotomous outcomes such as the occurrence of an LPN-to-RN transition.

Impacts of Increasing Temperature on the Future Incidence of West Nile Neuroinvasive Disease in the United States

Multiple studies have identified links between climate and West Nile virus disease since the virus arrived in North America. Here we sought to extend these results by developing a Health Impact Function (HIF) to generate county-level estimates of the expected annual number of West Nile neuroinvasive disease (WNND) cases based on the county’s historical WNND incidence, annual average temperature, and population size. To better understand the potential impact of projected temperature change on WNND risk, we used the HIF to project the change in expected annual number of WNND cases attributable to changing temperatures by 2050 and by 2090 using data from five global climate models under two representative concentration pathways (RCP4.5 and RCP8.5). To estimate the costs of anticipated changes, as well as to enable comparisons with other public health impacts, projected WNND cases were allocated to nonfatal and fatal outcomes, then monetized using a cost-of-illness estimate and the U.S. Environmental Protection Agency’s value of a statistical life, respectively. We found that projected future temperature and population changes could increase the expected annual number of WNND cases to ≈2000 - 2200 cases by 2050 and to ≈2700 - 4300 cases by 2090, from a baseline of 970 cases. Holding population constant at future levels while varying temperature from a 1995 baseline, we estimated projected temperature change alone is responsible for ≈590 and ≈960 incremental WNND cases in 2050 and 2090 (respectively) under the RCP4.5 scenario, and ≈820 and ≈2500 cases in 2050 and 2090 (respectively) for the RCP8.5 scenario, with substantial regional variation. The monetized impact of these temperature-attributable incremental cases is estimated at $0.5 billion in 2050 and $1.0 billion in 2090 under the RCP4.5 scenario, and $0.7 billion in 2050 and $2.6 billion in 2090 under the RCP8.5 scenario (undiscounted 2015 U.S. dollars).
