Paclitaxel-eluting balloon dilation of biliary anastomotic stricture after liver transplantation

AIM:To investigate the safety and effectiveness of endoscopic therapy with a paclitaxel-eluting balloon(PEB) for biliary anastomotic stricture(AS) after liver transplantation(LT).METhODS:This prospective pilot study enrolled 13 consecutive eligible patients treated for symptomatic AS after LT at the University hospital of Münster between January 2011 and March *** patients were treated by endoscopic therapy with a PEB and followed up every 8 wk by endoscopic retrograde cholangiopancreatography(ERCP).In cases of re-stenosis,further balloon dilation with a PEB was ***-up was continued until 24 mo after the last ***:Initial technical feasibility,defined as successful balloon dilation with a PEB during the initial ERCP procedure,was achieved in 100% of ***-term clinical success(LTCS),defined as no need for further endoscopic intervention for at least 24 mo,was achieved in 12 of the 13 patients(92.3%).The mean number of endoscopic interventions required to achieve LTCS was only 1.7 ± *** failure,defined as the need for definitive alternative treatment,occurred in only one patient,who developed recurrent stenosis with increasing bile duct dilatation that required stent ***:Endoscopic therapy with a PEB is very effective for the treatment of AS after LT,and seems to significantly shorten the overall duration of endoscopic treatment by reducing the number of interventions needed to achieve LTCS.

To evaluate cholangioscopy in addition to endoscopic retrograde cholangiopancreatography (ERCP) for management of biliary complications after liver transplantation (LT).METhODSTwenty-six LT recipients with duct-to-duct biliary reconstruction who underwent ERCP for suspected biliary complications between April and December 2016 at the university hospital of Muenster were consecutively enrolled in this observational study. After evaluating bile ducts using fluoroscopy, cholangioscopy using a modern digital single-operator cholangioscopy system (SpyGlass DS™) was performed during the same procedure with patients under conscious sedation. All patients received peri-interventional antibiotic prophylaxis and bile was collected during the intervention for microbial analysis and for antibiotic susceptibility ***-three biliary complications were found in a total of 22 patients, whereas four patients showed normal bile ducts. Anastomotic strictures were evident in 14 (53.8%) patients, non-anastomotic strictures in seven (26.9%), biliary cast in three (11.5%), and stones in six (23.1%). A benefit of cholangioscopy was seen in 12 (46.2%) patients. In four of them, cholangioscopy was crucial for selective guidewire placement prior to planned intervention. In six patients, biliary cast and/or stones failed to be diagnosed by ERCP and were only detectable through cholangioscopy. In one case, a bile duct ulcer due to fungal infection was diagnosed by cholangioscopy. In another case, signs of bile duct inflammation caused by acute cholangitis were evident. One patient developed post-interventional cholangitis. No further procedure-related complications occurred. Thirty-seven isolates were found in bile. Sixteen of these were gram-positive (43.2%), 12 (32.4%) were gram-negative bacteria, and Candida species accounted for 24.3% of all isolated microorganisms. Existing conventional megajoule plasma focus machines with 2–3 MA are producing fusion neutron yields of several times 10^(11) in deuterium operation,the fusion yields predominantly being the beam-gas *** the current to 10 MA and using 50%–50%D-T mixture will scale the neutron yield towards 10^(16) D-T fusion *** this work,we derive the Lawson criterion for plasma focus devices with a beam-target fusion neutron mechanism,so that we may glimpse what future technological advancements are needed for a break-even Q=1 plasma *** perform numerical experiments with a present-day feasible 0.9 MV,8.1 MJ,11 MA machine operating in 100 Torr in 50%–50%D-T *** Lee Code simulation gives a detailed description of the plasma focus dynamics through each phase,and provides plasma and yield parameters which show that out of 1.1×10^(19) fast beam ions produced in the plasma focus pinch,only 1.24×10^(14) ions take part in beam-target fusion reactions within the pinch,producing the same number of D-T *** remnant beam ions,numbering at least 10^(19),exit the focus pinch at 1.9 MeV,which is far above the 115 keV ion energy necessary for an optimum beam-target *** propose to regain the lost fusion rates by using a high-pressure D-T-filled drift-tube to attenuate the energy of the remnant beam ions until they reach the energy for the optimum fusion *** a fusion enhancement tube would further harvest beam-target fusion reactions by increasing the interaction path length(1 m)at increased interaction density(6 atm).A gain factor of 300 is conservatively estimated,with a final yield of 3.7×10^(16) D-T neutrons carrying kinetic energy of 83.6 kJ,demonstrating Q=0.01.

The effects of a harmonically exciting monopole source on an infinitely long cylindrical cavity embedded entirely within a fluid-saturated poroelastic formation of infinite extent are examined theoretically. It is assumed that the source is located outside the cavity at a specified distance from the borehole axis. The magnitudes of the hoop and radial stresses beside the pore pressures exerted on the interface and inside the porous medium surrounding the borehole are calculated and discussed. Biot's poroelastic modeling along with three types of boundary conditions for the cylindrical interface including the ideal fluid, empty borehole, and rigid inclusion with a hard boundary is employed for the analysis. Utilizing a proper translational addition theorem for expressing the incident spherical wave in terms of cylindrical wave expansions, the proposed boundary conditions at the interface are satisfied. Stresses are formulated by means of wave potential functions in a three-dimensional(3D) manner. The effects of the frequency and the radial distance between the source and borehole on the induced stresses are examined for the first cylindrical modes over frequency spectra. Two permeability conditions for the interface and three types of soils for the porous formation are considered throughout the analysis. To give an overall outline of the study, a numerical example is presented. The results clearly indicate that the distance is a key parameter and has considerable effects on the induced stress values. In addition, the interface permeability condition and soil characteristics play an important role in determining the dynamic response of the borehole. Finally,the obtained results are compared with the relevant analyses existing in the literature for some limit cases, and good agreement is achieved.

This paper considers the problem of robust disturbance attenuation for a class of uncertain nonlinear networked control systems. Takagi-Sugeno fuzzy models are firstly employed to describe the nonlinear plant. Markov processes are used to model the random network-induced delays and data packet dropouts. The Lyapunov-Razumikhin method has been used to derive such a controller for this class of nonlinear systems such that it is stochastically stabilizable with a disturbance attenuation level. Sufficient conditions for the existence of such a controller are derived in terms of the solvability of bilinear matrix inequalities. An iterative algorithm is proposed to change this non-convex problem into quasi-convex optimization problems, which can be solved effectively by available mathematical tools. The effectiveness of the proposed design methodology is verified by a numerical example.
