Despite the bi-directional interaction between gut microbiota and the brain not being fully understood,there is increasing evidence arising from animal and human studies that show how this intricate relationship may facilitate inflammatory bowel disease(IBD)pathogenesis,with consequent important implications on the possibility to improve the clinical outcomes of the diseases themselves,by acting on the different components of this system,mainly by modifying the *** the emergence of precision medicine,strategies in which patients with IBD might be categorized other than for standard gut symptom complexes could offer the opportunity to tailor therapies to individual *** aim of this narrative review is to elaborate on the concept of the gutbrain-microbiota axis and its clinical significance regarding IBD on the basis of recent scientific literature,and finally to focus on pharmacological therapies that could allow us to favorably modify the function of this complex system.

The continued need to develop less invasive alternatives to surgical and radiologic interventions has driven the development of endoscopic ultrasound(EUS)-guided *** include EUS-guided drainage of pancreatic fluid collections,EUS-guided necrosectomy,EUS-guided cholangiography and biliary drainage,EUSguided pancreatography and pancreatic duct drainage,EUS-guided gallbladder drainage,EUS-guided drainage of abdominal and pelvic fluid collections,EUS-guided celiac plexus block and celiac plexus neurolysis,EUSguided pancreatic cyst ablation,EUS-guided vascular interventions,EUS-guided delivery of antitumoral agents and EUS-guided fiducial placement and *** these procedures are technically challenging and require expertise in both EUS and interventional endoscopy,such as endoscopic retrograde cholangiopancreatography and gastrointestinal *** undertook a systematic review to record the entire body of literature accumulated over the past 2decades on EUS-guided interventions with the objective of performing a critical appraisal of published articles,based on the classification of studies according to levels of evidence,in order to assess the scientific progress made in this field.

Although pharmaceuticals have beendetected in the environment only in the range from ng/L to μg/L, it has beendemonstrated that they can adversely affect the health status of aquatic organisms. Lysosomal membrane stability （LMS） has previously been applied as an indicator of cellularwell-being todetermine health status in bivalve mussels. The objective of this study is to evaluate LMS in Ruditapes philippinarum haemolymph using the neutral red retention assay （NRRA）. Clamswere exposed in laboratory conditions to caffeine （0.1, 5, 15, 50 μg/L）, ibuprofen （0.1, 5, 10, 50 μg/L）, carbamazepine and novobiocin （both at 0.1, 1, 10, 50 μg/L） for35days. Results show adose-dependent effect of the pharmaceuticals. The neutral red retention time measured at the end of the bioassaywas significantly reduced by 50% after exposure to environmental concentrations （p 〈 0.05）（caffeine = 15 μg/L; ibuprofen = 10 μg/L; carbamazepine = 1 μg/L and novobiocin = 1 μg/L）, compared to controls. Clams exposed to these pharmaceuticalswere considered to present adiminished health status （retention time 〈45 min）, significantlyworse than controls （96 min）（p 〈 0.05）. The predicted no environmental effect concentration （PNEC） results showed that these pharmaceuticals are very toxic at the environmental concentrations tested. Measurement of the alteration of LMS has been found to be a sensitive technique that enables evaluation of the health status of clams after exposure to pharmaceuticals under laboratory conditions, thus representing a robust Tier-1 screening biomarker.

为研究温度和湿度对生土力学特性的影响,自主研发了温、湿控制生土三轴试验装置。基于常规三轴压力室,借助电涡流非接触位移传感器搭建高精度变形测试系统,实现环境变化下生土应变的连续性捕捉。针对法国里昂地区生土材料,开展不同湿度、围压和温度作用下生土三轴循环加载-卸载试验,实测生土在不同工况下的三轴剪切破坏强度、应力-应变曲线及体积变化,通过分析滞回曲线,研究弹性模量和残余应变随应力水平的演化特征;同时进行了变湿度条件下生土气态水迁移效应试验。结果表明:该装置能够稳定控制生土环境变化条件,模拟再现生土单元体实际服役工况,精确测量试验过程中生土力学指标及湿度、温度与压力等环境指标,初步检验了装置性能的可靠性和准确性。相关成果可为荷载-湿-热耦合作用下非饱和生土变形机制的研究与本构模型的建立提供技术支撑。

Celiac disease(CD) is frequently associated with neurological disorders,but very few reports concern the association with ischemic stroke.A 26-year-old woman affected by CD with secondary amenorrhea,carrier of a homozygous 5,10-methylenetetrahydrofolate reductase mutation with hyperhomocysteinemia,was affected by two occipital ischemic strokes within a period of 5 *** the time of the second stroke,while she was being treated with folic acid,acetylsalicylic acid and a gluten-free diet,she had left hemianopsia,left hemiparesthesias,and gait *** magnetic resonance imaging showed a subacute right occipital ischemic lesion,which was extended to the dorsal region of the right thalamus and the ipsilateral thalamocapsular *** and deamidated gliadin peptide antibodies were no longer present,while antinuclear antibodies,antineuronal antibodies and immune circulating complexes were only slightly *** the patient was taking folic acid,her homocysteine levels were almost normal and apparently not sufficient alone to explain the clinical event.A conventional cerebral angiography showed no signs of ***,rare causes of occipital stroke in young patients,such as Fabry's disease and mitochondrial myopathy,encephalomyopathy,lactic acidosis and stroke-like symptoms,were also excluded by appropriate ***,the most probable cause for the recurrent strokes in this young woman remained CD,although the mechanisms involved are still *** two main hypotheses concern malabsorption(with consequent deficiency of vitamins known to exert neurotrophic and neuroprotective effects) and immunemediated *** should be kept in mind in the differential diagnosis of ischemic stroke in young patients.

Cystic dystrophy of the duodenal wall is a rare form of the disease which was described in 1970 by French authors who reported the presence of focal pancreatic disease localized in an area comprising the C-loop of the duodenum and the head of the ***-man authors have defined this area as a"groove".We report our recent experience on cystic dystrophy of the paraduodenal space and systematically review the data in the literature regarding the alterations of this space.A MEDLINE search of papers published between 1966 and 2010 was carried out and 59 paperswere considered for the present study;there were 19 cohort studies and 40 case *** majority of patients having groove pancreatitis were middle *** age was significantly higher in patients having groove *** diagnosis of cystic dystrophy of the duodenal wall can now be assessed by multi-detector computer tomography,magnetic resonance imaging and endoscopic *** latter two techniques may also add more information on the involvement of the remaining pancreatic gland not involved by the duodenal malformation and they may help in differentiating"groove pancreatitis"from "groove adenocarcinoma".In conclusion,chronic pan-creatitis involving the entire pancreatic gland was present in half of the patients with cystic dystrophy of the duodenal wall and,in the majority of them,the pan-creatitis had calcifications.

The fascinating properties of two dimensional (2D) crystals have gained increasing interest for many applications. The synthesis of a 2D silicon structure, namely silicene, is attracting great interest for possible development of next generation electronic devices. The main difficulty in working with silicene remains its strong tendency to oxidation when exposed to air as a consequence of its relatively highly buckled structure. In this work, we univocally identify the Raman mode of air-stable low-buckled silicene nanosheets synthesized on highly oriented pyrolytic graphite (HOPG) located at 542.5 cm−1. The main focus of this work is Raman spectroscopy and mapping analyses in combination with ab initio calculations. Scanning tunneling microscopy images reveal the presence of a patchwork of Si three-dimensional (3D) clusters and contiguous Si areas presenting a honeycomb atomic arrangement, rotated by 30° with respect to the HOPG substrate underneath, with a lattice parameter of 0.41 ± 0.02 nm and a buckling of the Si atoms of 0.05 nm. Raman analysis supports the co-existence of 3D silicon clusters and 2D silicene. The Raman shift of low-buckled silicene on an inert substrate has not been reported so far and it is completely different from the one calculated for free-standing silicene and the ones measured for silicene grown on Ag(111) surfaces. Our experimental results are perfectly reproduced by our ab initio calculations of deposited silicene nanosheets. This leads us to conclude that the precise value of the observed Raman shift crucially depends on the strain between the silicene and the HOPG substrate.

PV （photovoltaic） solar panels generally produce electricity in the 6% to 12% efficiency range, the rest is being dissipated in thermal losses. To recover this amount, hybrid photovoltaic thermal systems （PV/T） have been devised. These are devices that simultaneously convert solar energy into electricity and heat. It is thus interesting to study the PV/T system as part of a closed loop single phase water CDU （coolant distribution unit） in laminar forced convection. In particular, the analysis was conducted on the optimal cooling performance of the thermal part, testing polynomial channel profiles of varying order （from zero to fourth） for channels of a real industrial module heat sink, under the following conditions： ideal flux of 1,000 W/m2 on one side, insulation on the opposite side, periodic conditions on the remaining sides, fully developed thermal and velocity profile in laminar flow of water. Through the use of a genetic algorithm, we have optimized the shape of the channel＇s sidewalls in terms of heat transfer maximization. In terms of Nusselt number, results show that fourth order profiles are the most efficient. When limits to allowable pressure loss and module weight are introduced, these bring generally to a lower efficiency of the system than the unconstrained case.

BACKGROUND:Hyperamylasemia can be observed anecdotally during the course of severe sepsis or septic *** study aimed to investigate the possibility of pancreatic involvement in patients with septic shock using serum pancreatic enzyme determinations and imaging techniques in 21 consecutive patients with septic shock and 21 healthy subjects as controls. METHODS:The serum activity of pancreatic amylase and lipase was assayed initially in all subjects and 24 and 48 hours after the initial observation in the 21 patients with septic shock All patients also underwent radiological examination to detect pancreatic abnormalities. RESULTS:The serum activity of pancreatic amylase was significantly higher in the 21 patients with septic shock than in the 21 control subjects during the study period while the serum activity of lipase was similar to that of the control *** and lipase serum activity did not significantly changed throughout the study period in the 21 patients with septic *** of the patients with pancreatic hyperenzymemia had clinical signs or morphological alterations compatible with acute pancreatitis CONCLUSION:The presence of pancreatic hyperenzymemia in septic shock patients is not a biochemical manifestation of acute pancreatic damage,and the management of these patients should be dependent on the clinical situation and not merely the biochemical results.
