Comprehensive lifestyle intervention vs soy protein-based meal regimen in non-alcoholic steatohepatitis

BACKGROUND Non-alcoholic steatohepatitis(NASH) has become one of the leading causes of liver disease in the western world. In obese patients weight reduction is recommended. Up to now there are no specific guidelines for weight loss in order to reduce hepatic fat *** To investigate the effects of a 24-wk guided lifestyle intervention program compared to a meal replacement regimen based on soy *** Twenty-six subjects with NASH participated in a randomized single-center study. They were randomly assigned to either meal replacement group(MR-G)with soy-yogurt-honey preparation or to guided lifestyle change group(LC-G)with endurance activity and nutrition counselling. Serum alanine transaminase(ALT), aspartate transaminase(AST), lipid parameters, and adipokines were measured. Liver fat content and lipid composition were determined by magnetic resonance imaging and magnetic resonance spectroscopy. Body fat mass and lean body mass were assessed using Bod Pod? device. Pre-and post-intervention monitoring of parameters was performed. Statistical analyses were conducted with SPSS software, results were expressed as median(interquartile range).RESULTS Twenty-two subjects(MR-G, n = 11 and LC-G, n = 11) completed the study(9 women, 13 men; age 52.1(15.0) years, body mass index(BMI) 32.3(3.3) kg/m^2).In both groups a significant weight loss was achieved(MR-G:-6.4(3.6) kg, P <0.01; LC-G:-9.1(10.4) kg, P < 0.01). BMI dropped in both groups(MR-G:-2.3(1.5)kg/m^2, P = 0.003; LC-G:-3.0(3.4) kg/m^2, P = 0.006). Internal fat and hepatic lipid content were markedly reduced in both groups in comparable amount. There was a strong correlation between reduction in liver fat and decrease in ***, both groups showed an improvement in glycemic control and lipid profile. Modification of the full-length cDNA clone of Newcastle disease virus isolated from an outbreak in the goose

Dementia is a serious complication for many patients with advanced Parkinson’s disease(PD)and its incidence is increased in PD patients harbouring a mutation in GBA1[1].However,cognitive impairment is not limited to late disease stages and can also manifest in patients with early untreated disease,leading to diminished social function and increased disability and caregiver *** have previously shown that reduced activity of the GBA1 protein product,the lysosomal enzyme glucocer-ebrosidase(GCase),is linked with long-term progres-sion to PD dementia[2].In this study,we extend this by determining the association of cerebrospinal fluid(CSF)GCase activity with the development of cognitive impair-ment through the analysis of neuropsychological test data collected over 10 years of prospective follow-up of patients with incident PD.

Determination of the Occurrence and Elimination of Endocrine Disrupting Compounds (EDCs) in Municipal Wastewater Treatment Plants (WWTP)

In recent years, many research and development activities have focussed on endocrine disrupting compounds (EDCs) in rivers, lakes and surface waters as the potential cause of reproductive disturbances in different aquatic organisms e.g. fish and mollusk. The effluent of wastewater treatment plants was identified as main source for EDCs entering the aquatic environment. The purpose of the present study was to determine the estrogenic activity of wastewater and the elimination efficiency of various WWTPs in the different purification steps using the E-screen assay, an in-vitro test system based on the increasing proliferation of human breast cancer cells (MCF-7) in response to the presence of hormonal active substances. In contrast to expensive and time-intensive targeted instrumental single substance analysis an effect-related biological testing provides a sum parameter for the entirety of compounds contributing to the total estrogenic activity (agonists and antagonists, anti-estrogenic and also toxic compounds) in concentration units of the reference substance 17b-estradiol. The current standard purification methods of biological wastewater treatment in particular the activated sludge process significantly reduce estrogenicity (average 95%). Sorption on activated carbon and subsequent precipitation leads to a further reduction of the overall estrogenic activity up to 99%.

AIM To establish patient-individual tumor models of rectal cancer for analyses of novel biomarkers, individual response prediction and individual therapy *** Establishment of cell lines was conducted by direct in vitro culturing and in vivo xenografting with subsequent in vitro culturing. Cell lines were in-depth characterized concerning morphological features, invasive and migratory behavior, phenotype, molecular profile including mutational analysis, protein expression, and confirmation of origin by DNA fingerprint. Assessment of chemosensitivity towards an extensive range of current chemotherapeutic drugs and of radiosensitivity was performed including analysis of a combined radioand chemotherapeutic treatment. In addition, glucose metabolism was assessed with 18 F-fluorodeoxyglucose(FDG) and proliferation with 18 *** We describe the establishment of ultra-low passage rectal cancer cell lines of three patients suffering from rectal cancer. Two cell lines(HROC126, HROC284 Met) were established directly from tumor specimens while HROC239 T0 M1 was established subsequent to xenografting of the tumor. Molecular analysis classified all three cell lines as CIMP-0/non-MSI-H(sporadic standard) type. Mutational analysis revealed following mutational profiles: HROC126: APC^(wt), TP53^(wt), KRAS^(wt), BRAF^(wt), PTEN^(wt); HROC239 T0 M1: APC^(mut), P53^(wt), KRAS^(mut), BRAF^(wt), PTEN^(mut) and HROC284 Met: APC^(wt), P53^(mut), KRAS^(mut), BRAF^(wt), PTEN^(mut). All cell lines could be characterized as epithelial(EpCAM+) tumor cells with equivalent morphologic features and comparable growth kinetics. The cell lines displayed a heterogeneous response toward chemotherapy, radiotherapy and their combined application. HROC126 showed a highly radio-resistant phenotype and HROC284 Met was more susceptible to a combined radiochemotherapy than HROC126 and HROC239 T0 M1. Colorectal anastomotic leakage: Aspects of prevention, detection and treatment

All colorectal surgeons are faced from time to time with anastomotic leakage after colorectal surgery. This complication has been studied extensively without a significant reduction of incidence over the last 30 years. New techniques of prevention, by innovative anastomotic techniques should improve results in the future, but standardization and "teachability" should be guaranteed. Risk scoring enables intra-operative decision-making whether to restore continuity or deviate. Early detection can lead to reduction in delay of diagnosis as long as a standard system is used. For treatment options, no firm evidence is available, but future studies could focus on repair and saving of the anastomosis on the one hand or anastomotical breakdown and definitive colostomy on the other hand.
