Chronic pain,one of the most ancient and costly health problems,is poorly controlled and its underlying mechanisms are not well *** now,increasing evidence indicates that astrocytes may play an important role in the development and maintenance of chronic pain,but the cellular mechanisms involved remains *** neurons,the key pain modulator prodynorphin and dynorphin are known to be involved in chronic pain and are regulated by the Downstream Regulatory Element Antagonist Modulator(DREAM),which is a multifunctional Cabinding *** were generally believed to have no prodynorphin ***,we identified a novel signal pathway of pain cascades in astrocytes:DREAM-prodynorphin-dynorphin,which are three key factors involved in pain ***,as a transcriptional repressor,exhibited nuclear and cytoplasmic distribution and two different polymeric forms in *** with K and exposure to glutamate resulted in down-regulation of DREAM in astrocyte via N-methyl-D-aspartate(NMDA) receptor and PI-3 kinase,followed by the up-regulation of prodynorphin mRNA and dynorphin protein ***,DREAM in astrocytes exhibited different structural properties and cellular locahzation compared to that in glutamatergic and GABAergic *** differential characteristics implicated different roles of DREAM in regulating prodynorphin expression in neurons and *** data suggests that astrocytes may present prodynorphin-dynorphin-related functions and may have the potential in regulating chronic pain through different mechanisms than neurons via the DREAM-prodynorphin-dynorphin pathway.

Water movement plays vital roles in both physiological and pathological conditions in the *** are responsible for regulating this water movement,and are the major contributors to brain edema in pathological ***（AQPs） in astrocytes play critical roles in the regulation of water movement in the ***1,3,4,5,8 and 9 have been reported in the *** with AQP1,4 and 9,AQP3,5 and 8 are less *** the lesser known AQPs,AQP5,which has multiple functions identified outside the central nervous system,is also indicated to be involved in hypoxia injury in *** our study,AQP5 expression could be detected both in primary cultures of astrocytes and ***5 was localized on the cytoplasmic membrane and in the cytoplasm of ***5 expression was down-regulated during ischemia treatment and up-regulated after scratch-wound injury,which was also confirmed in a MCAO model and a stab-wound injury model in *** AQP5 increased after scratch injury was polarized to the migrating processes and cytoplasmic membrane of astrocytes in the leading edge of the scratch-wound,and AQP5 over-expression facilitated astrocyte process elongation after scratch *** together,mese results indicate that AQP5 might be an important water channel in astrocytes that is differentially expressed during various brain injuries.

14-3-3γ is a multifunctional acidic protein that is induced by ischemia in neural cells,including astrocytes,and has a neuroprotective *** ischemia,a lack of oxygen（hypoxia） and glucose,and an accumulation of toxins,*** order to elucidate which ischemia components caused the damage to astrocytes and the induction of the protective protein 14-3-3γ,we examined the effects of hypoxia,glucose deprivation,and the accumulation of toxins,separately,and compared the degree of damage to ischemia *** caused a rapid upregulation of 14-3-3γthat occurred within 1 to 4 *** alone elevated 14-3-3γ expression,while glucose deprivation did not cause a detectable change in the 14-3-3γ mRNA and protein *** changes of toxic metabolites barely affected14-3-3γ gene and protein expression in the time course *** addition,comparing the activation of the ERK1/2,Akt,P38 and JNK signaling pathways under ischemia and hypoxia in astrocytes revealed mat ischemia activated all four pathways,while hypoxia had little effect on any of *** also observed how glutamate（Glu） and arachidonic acid（AA）,the main toxic substances released by neural cells during ischemia injury,affected the expression of the14-3-3 protein *** all members of the 14-3-3 family,β and γ were up-regulated rapidly after 3 hr of treatment with G1u or AA,while η and ε increased until 6 hr and ζ increased after *** results demonstrated that the three components of ischemia played diverse roles in the regulation of 14-3-3Y) that different isoforms of the 14-3-3family showed disparate responses to Glu and AA treatments,and that the JNK/c-Jun/AP-1 signaling pathway was involved in 14-3-3γ up-regulation.

Objective:To correlate the utility of the Fundamentals of Laparoscopic Surgery(FLS)manual skills program with the Objective Structured Assessment of Technical Skills(OSATS)global rating scale in evaluating operative ***:The Asian Urological Surgery Training and Educational Group(AUSTEG)Laparoscopic Upper Tract Surgery Course implemented and validated the FLS program for its usage in laparoscopic surgical ***’basic laparoscopic skills were assessed using three different training models(peg transfer,precision cutting,and intra-corporeal suturing).They also performed live porcine laparoscopic surgery at the same *** surgery skills were assessed by blinded faculty using the OSATS rating ***:From March 2016 to March 2019,a total of 81 certified urologists participated in the course,with a median of 5 years of post-residency *** differences in task time did not reach statistical significance,those with more surgical experience were visibly faster at completing the peg transfer and intra-corporeal suturing FLS ***,they took longer to complete the precision cutting task than participants with less *** OSATS scores correlated weakly with all three FLS tasks(peg transfer time:r=0.331,r^(2)=0.110;precision cutting time:r=0.240,r^(2)=0.058;suturing with intracorporeal knot time:r=0.451,r^(2)=0.203).Conclusion:FLS task parameters did not correlate strongly with OSATS globing rating scale *** FLS task models demonstrated strong validity,it is important to assimilate the inconsistencies when benchmarking technical proficiency against real-life operative competence,as evaluated by FLS and OSATS,respectively.

Prostate cancer detection upon transrectal ultrasound-guided biopsy in relation to digital rectal examination and prostate-specific antigen level： what to expect in the Chinese population？

We investigated the prostate cancer detection rates upon transrectal ultrasound （TRUS）-guided biopsy in relation to digital rectal examination （DRE） and prostate-specific antigen （PSA）, and risk factors of prostate cancer detection in the Chinese population. Data from all consecutive Chinese men who underwent first TRUS-guided prostate biopsy from year 2000 to 2013 was retrieved from our database. The prostate cancer detection rates with reference to DRE finding and PSA level of 〈 4, 4-10, 10.1-20, 20.1-50 and 〉 50 ng ml^-1 were investigated. Multivariate logistic regression analyses were performed to investigate for potential risk factors of prostate cancer detection. A total of 2606 Chinese men were included. In patients with normal DRE, the cancer detection rates were 8.6%, 13.4%, 21.8%, 41.7% and 85.2% in patients with PSA 〈 4, 4-10, 10.1-20, 20.1-50 and 〉 50 ng ml^-1 respectively. In patients with abnormal DRE, the cancer detection rates were 12.4%, 30.2%, 52.7%, 80.6% and 96.4% in patients with PSA 〈 4, 4-10, 10.1-20, 20.1-50 and 〉 50 ng m1-1 respectively. Older age, smaller prostate volume, larger number of biopsy cores, presence of abnormal DRE finding and higher PSA level were associated with increased risk of prostate cancer detection upon multivariate logistic regression analyses （P 〈 0.001）. Chinese men appeared to have lower prostate cancer detection rates when compared to the Western population. Taking the different risk factors into account, an individualized approach to the decision of TRUS-guided biopsy can be adopted.

The performance characteristics of prostate-specific antigen and prostate-specific antigen density in Chinese men

We investigated the performance characteristics of prostate-specific antigen （PSA） and PSA density （PSAD） in Chinese men. All Chinese men who underwent transrectal ultrasound-guided prostate biopsy （TRUS-PB） from year 2000 to 2013 were included. The receiver operating characteristic （ROC） curves for both PSA and PSAD were analyzed. The sensitivity, specificity, positive predictive value （PPV） and negative predictive value （NPV） at different cut-off levels were calculated. A total of 2606 Chinese men were included. For the ROC, the area under curve was 0.770 for PSA （P〈 0.001） and 0.823 for PSAD （P〈 0.001）. PSA of 4.5 ng ml^-1 had sensitivity of 94.4%, specificity of 14.1%, PPV of 29.5%, and NPV of 86.9%; PSAD of 0.12 ng ml^-1cc^-1 had sensitivity of 94.5%, specificity of 26.6%, PPV of 32.8%, and NPV of 92.7%. On multivariate logistic regression analyses, PSA cut-off at 4.5 ng ml^-1 （OR 1.61, 95% CI 1.05-2.45, P = 0.029） and PSAD cut-off at 0.12 ng ml^-1 cc^-1 （OR 6.22, 95% CI 4.20-9.22, P 〈 0.001） were significant predictors for prostate cancer detection on TRUS-PB. In conclusion, the performances of PSA and PSAD at different cut-off levels in Chinese men were very different from those in Caucasians. PSA of 4.5 ng ml^-1 and PSAD of O. 12 ng ml^-1 cc^-1 had near 95% sensitivity and were significant predictors of prostate cancer detection in Chinese men.

Rheumatoid arthritis(RA)is a chronic inflammatory disease characterized by cartilage and bone damage with the presence of pannus formation which causes uncontrollable proliferation and invasion of fibroblast-like synoviocytes(FLS).Since rheumatoid arthritis is a chronic disorder,the patients normally need to undergo prolonged antirheumatic treatment with the use of disease-modifying antirheumatic drugs(DMARDs),steroids,and nonsteroidal anti-inflammatory drugs(NSAIDs).The efficacy of such long-term pharmaceutical intervention can be significantly affected by the development of drug *** pathological relationship between rheumatoid arthritis and cancer hinted that some chemotherapeutic drugs,such as methotrexate(MTX),could be used for RA *** idea was reinforced by the analysis of mutations in p53 tumor suppressor *** 50%of p53 somatic mutations observed from various cancers are also identified in the synovium of RA patients1(Fig.1A).Of note,the hyperplastic synovium in RA with overexpressed p53 mutants contributed to the destruction of joints in patients with erosive RA.

Association of time to prostate-specific antigen nadir and logarithm of prostate-specific antigen velocity after progression in metastatic prostate cancer with prior primary androgen deprivation therapy

We investigated the association of time to prostate-specific antigen nadir （TTPN） and logarithm of prostate-specific antigen velocity after progression Log（PSAVAP） in metastatic prostate cancer with prior primary androgen deprivation therapy （ADT）. All metastatic prostate cancer patients treated with primary ADT from 2000 to 2009 were reviewed. Patients who developed disease progression were included in the subsequent analyses. Patients were categorized into three groups according to their TTPN： TTPN of 〈3 months, 3-17 months, and 〉17 months. We compared the Log（PSAVAP） between the different TTPN groups using Mann-Whitney U-test and Kruskal-Wallis test. Further multiple linear regression analyses on Log（PSAVAP） were performed to adjust for other potential confounding factors. Among 419 patients who were treated with primary ADT, 306 patients developed disease progression with a median follow-up of 28 months, Longer TTPN was associated with lower Log（PSAVAP）（P = 0.008） within all subgroup analyses （TTPN of 〈3 vs 3-17 months, P = 0.020; TTPN of 3-17 vs 〉17 months, P = 0.009; and TTPN of 〈3 vs 〉17 months, P = 0.001）. Upon multiple linear regression analyses, baseline PSA （regression coefficient 0.001, P = 0.045）, PSA nadir （regression coefficient 0.002, P = 0.040）, and TTPN （regression coefficient -0.030, P = 0.001） were the three factors that were significantly associated with Log（PSAVAP）. In conclusion, a longer TTPN was associated with lower Log（PSAVAP） in metastatic prostate cancer patients following primary ADT. TTPN cut-offs at 3 months and 17 months appeared to have prognostic significance in predicting Log（PSAVAP）. TTPN may serve as a good prognostic indicator in deciding the treatment strategy in patients with disease progression.

Background:We evaluated factors associated with eczema severity in ***:Nottingham Eczema Severity Score(NESS),family and personal history of atopy,skin prick test for common food and aeroallergens,highest serum IgE level and eosinophil count were *** with paired NESSs(childhood-NESS is NESS performed at10 years)were further ***:Adolescence-NESS(n=383 patients)was associated with eczema onset in infancy,dust mite and food allergen sensitization,dietary avoidance,use of wet wrap,traditional Chinese medicine,immunomodulant(azathioprine or cyclosporine),high IgE level,eosinophil count,but not with family/personal history of ***-two patients had both childhood-NESS and adolescence-NESS(mean follow-up of 6.8 years)showing that adolescence-NESS was associated with childhood-NESS severity grades(P=0.034).Of these patients,48%remained in the same severity grades,whereas 39%improved,and 13%deteriorated from childhood to ***:It is not possible to assure parents that their child can outgrow *** eczema prognosis research,long-term follow-up is warranted.
