Colonoscopy surveillance for high risk polyps does not always prevent colorectal cancer

AIM To determine the frequency and risk factors for colorectal cancer(CRC) development among individuals with resected advanced adenoma(AA)/traditional serrated adenoma(TSA)/advanced sessile serrated adenoma(ASSA). METHODS Data was collected from medical records of 14663 subjects found to have AA, TSA, or ASSA at screening or surveillance colonoscopy. Patients with inflammatory bowel disease or known genetic predisposition for CRC were excluded from the study. Factors associated with CRC developing after endoscopic management of high risk polyps were calculated in 4610 such patients who had at least one surveillance colonoscopy within 10 years following the original polypectomy of the incident advanced polyp. RESUlTS84/4610(1.8%) patients developed CRC at the polypectomy site within a median of 4.2 years(mean 4.89 years), and 1.2%(54/4610) developed CRC in a region distinct from the AA/TSA/ASSA resection site within a median of 5.1 years(mean 6.67 years). Approximately, 30%(25/84) of patients who developed CRC at the AA/TSA/ASSA site and 27.8%(15/54) of patients who developed CRC at another site had colonoscopy at recommended surveillance intervals. Increasing age; polyp size; male sex; right-sided location; high degree of dysplasia; higher number of polyps resected; and piecemeal removal were associated with an increased risk for CRC developmentat the same site as the index polyp. Increasing age; right-sided location; higher number of polyps resected and sessile endoscopic appearance of the index AA/TSA/ASSA were significantly associated with an increased risk for CRC development at a different site. CONClUSION Recognition that CRC may develop following AA/TSA/ASSA removal is one step toward improving our practice efficiency and preventing a portion of CRC related morbidity and mortality.

We present radial velocity（RV） curve templates of RR lyrae first-overtone（RRc） stars constructed with the Mg I b triplet and Hα lines using time-domain Medium-Resolution Survey spectra of seven RRc stars from large Sky Area Multi-Object Fiber Spectroscopic Telescope（lAMOST） Data Release 9. Additionally, we derive the relation between the stellar RV curve amplitudes and g-band light curve amplitudes from Zwicky Transient Facility（ZTF）public survey. For those RRc stars without ZTF g-band light curves, we provide the conversions from the light curve amplitudes in ZTF r-and i-bands, Gaia G-band, and V-band from the All-Sky Automated Survey for Supernovae to those in ZTF g-band. We validate our RV curve templates using the RRc star SV Scl and find the uncertainties of systemic RV are less than 2.11 km s-1and 6.08 km s-1based on the Mg I b triplet and Hα lines,respectively. We calculate the systemic RVs of 30 RRc stars using the RV curve templates constructed with the Mg I b triplet and Hα lines and find the systemic RVs are comparable with each other. This RV curve template will be particularly useful for obtaining the systemic RV of RRc using the lAMOST spectroscopy.

Bitter gourd is a tropical wine grown mainly in India, China and South East Asia. The plant is cultivated mainly for its fruit part which is edible. Bitter gourd is unaccepted widely due to its bitter taste. Nevertheless, the fruit is a source of several key nutrients. The plant, as a whole contains, more than 60 phyto-medicines that are active against more than 30 diseases, including cancer and diabetes. Currently, the incorporation of the bioactive compounds isolated from bitter gourd into functional foods and beverages finds a new horizon. Nanoencapsulation and novel green extraction methods can be employed to improve the yield and quality of extracted compounds and their stability while incorporation into food products. The present review is an attempt to throw light to nutritional aspects, various bioactive compounds present and important nutraceutical properties of the bitter gourd plant in detail.

BACKGROUND The long-term impact of vitamin D deficiency and metabolic syndrome(MetS)on cardiovascular disease(CVD)and all-cause mortality are still a matter of *** To test the hypotheses that lower serum 25 hydroxyvitamin D[25(OH)D]concentrations(a marker of vitamin D level)and MetS have a long-term impact on the risk of CVD and all-cause mortality,and individuals with vitamin D deficiency can be identified by multiple *** A sample of 9094 adults,20 to 90 years of age,who participated in the Third National Health and Nutrition Examination Survey(NHANES III,1988 to 1994)were followed through December 2015 was *** associations of serum 25(OH)D concentrations and MetS with CVD and all-cause mortality were analyzed longitudinally using Cox regression *** and regression tree(CART)for machine learning was applied to classify individuals with vitamin D *** Of 9094 participants,30%had serum 25(OH)D concentrationsl(defined as vitamin D deficiency),39%had serum 25(OH)D concentrations between 20 to 29 ng/ml(insufficiency),and 31%had serum 25(OH)D concentrations≥30 ng/ml(sufficiency).Prevalence of MetS was 28.4%.During a mean of 18 years follow-up,vitamin D deficiency and MetS were significantly associated with increased risk of CVD and all-cause *** with both vitamin D deficiency and MetS had the highest risk of CVD mortality(HR=1.77,95%CI:1.22-2.58)and all-cause mortality(HR=1.62,95%CI:1.26-2.09),followed by those with both vitamin D insufficiency and MetS for CVD mortality(HR=1.59,95%CI:1.12-2.24),and all-cause mortality(HR=1.41,95%CI:1.08-1.85).Meanwhile,vitamin D sufficiency significantly decreased the risk of CVD and all-cause mortality for those who even had *** the total study sample,CART analysis suggests that being non-Hispanic Black,having lower serum folate level,and being female were the first three predictors for those with serum 25(OH)D *** Vitamin D defic
