Outcomes of kidney transplantation in patients with hepatitis B virus infection:A systematic review and meta-analysis

AIM To assess outcomes of kidney transplantation including patient and allograft outcomes in recipients with hepatitis B virus(HBV) infection, and the trends of patient's outcomes *** A literature search was conducted using MEDLINE, EMBASE and Cochrane Database from inception through October 2017. Studies that reported odds ratios(OR) of mortality or renal allograft failure after kidney transplantation in patients with HBV [defined as hepatitis B surface antigen(HBs Ag) positive] were included. The comparison group consisted of HBs Agnegative kidney transplant recipients. Effect estimates from the individual study were extracted and combined using random-effect, generic inverse variance method of Der Simonian and Laird. The protocol for this metaanalysis is registered with PROSPERO(International Prospective Register of Systematic Reviews; no. CRD42017080657).RESULTS Ten observational studies with a total of 87623 kidney transplant patients were enrolled. Compared to HBs Ag-negative recipients, HBs Ag-positive status was significantly associated with increased risk of mortality after kidney transplantation(pooled OR = 2.48; 95%CI: 1.61-3.83). Meta-regression showed significant negative correlations between mortality risk after kidney transplantation in HBs Ag-positive recipients and year of study(slopes =-0.062, P = 0.001). HBs Agpositive status was also associated with increased risk of renal allograft failure with pooled OR of 1.46(95%CI: 1.08-1.96). There was also a significant negative correlation between year of study and risk of allograft failure(slopes =-0.018, P = 0.002). These associations existed in overall analysis as well as in limited cohort of hepatitis C virus-negative patients. Primary left pulmonary vein stenosis is a very rare condition. It is associated with variable form of Pulmonary artery hypertension. Catheter based intervention is used to treat the disease like balloon dilation and stenting but this has high recurrence rate. Here is a case in which we did a sutureless repair of left pulmonary vein thus decreasing the possibility of recurrence. Sutureless surgical management is technically superior and less challenging than direct left atrial wall anastomosis and it provides better results than catheter based intervention in terms of recurrence of stenosis.

The present study was undertaken to evaluate in-vitro antioxidant and anthelmintic activity of ethanolic and aqueous extract from whole plant Sida cordifolia Linn (Malvaceae). The antioxidant activities are evaluated by various antioxidant assays like α, α-Diphenyl-β-picrylhydrazyl (DPPH) free radical scavenging, total reducing power, nitric oxide scavenging and hydrogen peroxide scavenging. The various antioxidant activities were compared to standard antioxidants such as ascorbic acid. The antioxidant activity of ethanolic extract is almost quantitatively equivalent to that of the standards used, ascorbic acid. The further anthelmintic activity of whole plant is investigated through using Indian earthworm (Pheretima posthuma) showed that it is one of the most important local medicinal plants both for ritual and ethnomedical practices. Various concentrations of ethanol and aqueous extract (10, 20, 30, 40 mg/ml) of whole plant of Sida cordifolia Linn were tested in the bioassay, which involve determination of time of paralysis of the worms. Albendazole was included as reference standard. The most activity was observed with aqueous extract as compared to standard drug. The results from the above studies indicate that plant Sida cordifolia Linn. possesses potent antioxidant and anthelmintic activity.

Coal is a dirty fuel contaminated with F- and other elements. Several million tons of coal are burnt in Korba basin, central India to generate electricity with pouring fluorine and other elements into the environment. The water is contaminated with F- and other chemicals beyond the permissible limits. The contaminated water is consumed by the human and animals by excreting the balanced toxicants through the stool and urine. Several folds higher concentration of F- in urine (44 mg/L) and stool (266 mg/kg) samples of the cattle are observed. The prevalence of fluorosis diseases in cattle of the Korba basin is reported.

The study of heavy metal (HMs) contamination of environment is of great interest due to their serious health hazard. In this work, the contamination of tree leaves with the HMs in the most polluted industrial city, Korba, India is described. The leaves of common trees i.e. Azadirachta indica, Butea monosperma, Eucalyptus, Ficus religiosa, Mangifera indica and Tectona grandis were selected for assessment of the HMs contamination as bioindicator. The elevated concentration of HMs (i.e. As, Fe, Cr, Mn, Cu, Zn, Cd, Pb and Hg) in the tree leaves was observed, ranging from 2.8 - 43, 728 - 5182, 8.6 - 49, 48 - 1196, 43 - 406, 79 - 360, 1.12 - 1.65, 1.6 - 16.4 and 0.13 - 0.76 mg/kg, respectively. The concentration, enrichment and sources of the HMs in the leaves are described. Azadirachta indica leaves, accumulating higher concentration of the HMs, showed a higher efficiency as bioindicator for the urban pollution.

From the evolution of the mankind, Turmeric has been used in conventional medication. India is in lead for producing, marketing and exporting the Turmeric and its value added products. Curcuma longa (Turmeric) is an Indian rhizomatous medicinal herb from the Zingiberaceae family that is common and widely available across the globe. The components of Turmeric are curcumin, demethoxycurcumin and bisdemethoxycurcumin and these are collectively known as curcuminoids. Curcumin, the active ingredient of Turmeric is generally investigated by the scientific community for its wide range of antioxidant activity, anti-Inflammatory properties and anti-cancer activity, anti-metabolic syndrome activities, neuroprotective activity, antimicrobial effects, anti-arthritis effects, anti-viral effects, anti-asthma and anti-diabetic effects, anti-obesity, cardio and liver toxicity protection activity, anti-depression and anxiety activities. Turmeric has been widely used as a typical household treatment for cough, sore throat, respiratory ailments and could be an effective immunity booster against SARS-CoV-2 therapy during the ongoing pandemic situation. Safety evaluation studies indicate that both turmeric and curcumin are well tolerated at a very high dose without any toxic effects. Thus, turmeric and its constituents have the potential for the development of modern medicine for the treatment of various diseases. So in this review, we describe the various metabolic roles of curcumin and activities for the benefit of human health.
