In this paper, we focus on Elliptic Curve Cryptography based approach for Secure Multiparty Computation (SMC) problem. Widespread proliferation of data and the growth of communication technologies have enabled collaborative computations among parties in distributed scenario. Preserving privacy of data owned by parties is crucial in such scenarios. Classical approach to SMC is to perform computation using Trusted Third Party (TTP). However, in practical scenario, TTPs are hard to achieve and it is imperative to eliminate TTP in SMC. In addition, existing solutions proposed for SMC use classical homomorphic encryption schemes such as RSA and Paillier. Due to the higher cost incurred by such cryptosystems, the resultant SMC protocols are not scalable. We propose Elliptic Curve Cryptography (ECC) based approach for SMC that is scalable in terms of computational and communication cost and avoids TTP. In literature, there do exist various ECC based homomorphic schemes and it is imperative to investigate and analyze these schemes in order to select the suitable for a given application. In this paper, we empirically analyze various ECC based homomorphic encryption schemes based on performance metrics such as computational cost and communication cost. We recommend an efficient algorithm amongst several selected ones, that offers security with lesser overheads and can be applied in any application demanding privacy.

A Distributed Weighted Cluster Based Routing Protocol for MANETs

Mobile ad-hoc networks (MANETs) are a form of wireless networks which do not require a base station for providing network connectivity. Many MANETs’ characteristics that distinguish MANETs from other wireless networks also make routing a challenging task. Cluster based routing is a MANET routing schemes in which various clusters of mobile nodes are formed with each cluster having its own clusterhead which is responsible for routing among clusters. In this paper we propose and implement a distributed weighted clustering algorithm for MANETs. This approach is based on combined weight metric that takes into account several system parameters like the node degree, transmission range, energy and mobility of the nodes. We have evaluated the performance of the proposed scheme through simulation in various network situations. Simulation results show that improved distributed weighted clustering algorithm (DWCAIMP) outperforms the original distributed weighted clustering algorithm (DWCA).

Epidemiology of Maxillofacial Trauma in Pediatric and Adolescent Population: An Institutional Experience of 6 Years

Introduction: Facial fractures are uncommon injuries in children. Pediatric maxillofacial fractures constitute 1% to 15% of all facial fractures, but show different clinical features when compared with adult patients. Worldwide, the major causes of fractures in children are accidents, falls, violence, and sports- related accidents. Facial trauma in children may result in injury to the facial growth centers, leading to subsequent developmental abnormalities in the injured area. Patients & Methods: Data of pediatric and adolescent patients (0 - 17 years), with a history of trauma, has been compiled over a period of 6 years. History, clinical and radiographic data records were analyzed. Compilation was done in the following categories;age and sex distribution, etiology, site of trauma and associated soft tissue injuries were recorded. The treatment rendered has also been mentioned. Results: A total of 340 patient records were assessed. Demographic data, etiology, type of fracture, associated injury and treatment rendered were recorded. It was found that males (54.7%) were more frequently affected than females (45.3%). Incidence of trauma was more in children belonging to the pre-school age group (38.5%) and the most common etiology was sports-related injuries (43.8%). Dentoalveolar fractures were the most common type (58.5%) of fractures seen in the patients. Most of the patients were managed by conservative treatment (35%) or closed reduction (48%), while a few required open reduction (7.9%). Conclusion: It is felt that this data will be useful in determining the pattern and etiology of maxillofacial trauma in pediatric patients in an Indian setting. Additionally, an outline of the treatment plan in different types of fractures is also summarized.

Colorectal Inflammatory Pseudopolyps: A Retrospective Analysis of 70 Patients

Introduction: Inflammatory pseudopolyps (IPs) are a well-recognized entity in patients with inflammatory bowel disease (IBD), most likely developing from long-standing chronic inflammation. Similarly, IPs have been associated with ischemic and infectious colitis, intestinal ulcers, and mucosal anastomoses. This study aimed to analyze inflammatory pseudopolyps without a history of these known associated pathologies. Materials and Methods: A database search was conducted for patients who underwent biopsies at Thomas Jefferson University Hospital from 2003-2013 for the presence of colorectal IPs. Exclusion criteria consisted of patients with a history of IBD, mucosal anastomoses, ischemic and infectious colitis. Spatial and temporal associations between colonic pathologies and IPs were assessed via Fisher’s exact and chi-square test, respectively. Results: Seventy-five polyps from 70 patients fulfilled the database search criteria. Forty-one pseudopolyps (55%) arose from the rectosigmoid region. Twenty-two patients had no associated colon pathology (31%);35 patients had epithelial polyps (50%), such as tubular adenomas, serrated adenomas, and hyperplastic polyps;10 patients had colonic adenocarcinoma (16%), and 18 patients had diverticulosis (26%). Epithelial polyps were significantly associated with IPs in the same region. However, diverticulosis was independent of IPs in regard to space and time. Conclusion: Colorectal inflammatory pseudopolyps may develop sporadically in up to one third of the cases while others frequently arise in the background of non-IBD colonic pathology. The increased presence of these polyps in the left colon raises the possibility that a subset of them may arise in predisposed mucosa. These polyps need to be differentiated from other morphologically similar colonic polyps.

Anomalous origin of coronary arteries may be encountered coincidentally in the presence of unrelated pathology or when these are affected directly. This may directly be responsible for affecting the procedure or outcome. Various types of anomalies of origin,?as well as course of coronary arteries,?have been classified in the past. Here we report 3 cases of anomalous origin of coronary arteries in different scenarios. First case had anomalous coronary with bicuspid aortic valve with dilated ascending aorta for which Bentall’s procedure was done, while the second and third cases were anomalous coronaries with coronary artery disease for which coronary artery bypass grafting was done.

Sutureless Repair of Left Primary Pulmonary Vein Stenosis—A Case Report

Primary left pulmonary vein stenosis is a very rare condition. It is associated with variable form of Pulmonary artery hypertension. Catheter based intervention is used to treat the disease like balloon dilation and stenting but this has high recurrence rate. Here is a case in which we did a sutureless repair of left pulmonary vein thus decreasing the possibility of recurrence. Sutureless surgical management is technically superior and less challenging than direct left atrial wall anastomosis and it provides better results than catheter based intervention in terms of recurrence of stenosis.

Unruptured Right Sinus of Valsalva Aneurysm Dissecting into Interventricular Septum Causing Complete Heart Block: Can Early Surgical Correction Revert Rhythm Disturbances?

A sinus of Valsalva aneurysm (SOVA) is abnormal dilatation of the either aortic sinuses, area of the aortic root between the aortic valve annulus and the sinotubular junction. Their clinical presentation may range from being asymptomatic as an incidental finding on cardiac imaging to symptomatic presentations related to the compression of adjoining structures or intracardiac shunting caused by rupture of the SOVA mostly into the right side of the heart. The compression leads to findings of tricuspid valve regurgitation, right ventricular outflow tract (RVOT) obstruction and rarely complete heart block (CHB). Dissection or erosion into interventricular septum is one of the rarest complications of SOVA. The symptomatic presentation is almost always a surgical emergency. Here we present a case report of a patient with unruptured sinus of valsalva originating from right sinus dissecting into interventricular septum causing complete heart bock. In this case after surgical correction the complete heart block reverted to sinus rhythm.

The Current Treatment and Prevention of Post Herpetic Neuralgia

Post herpetic neuralgia (PHN) is a chronic neuropathic pain syndrome in the area of the herpes zoster (HZ) rash, persisting after the cutaneous lesions have subsided. Despite numerous treatment advances, many patients remain refractory to the current therapies and continue to have pain, physical, economical and psychological distress. In this review, we will focus on the current treatment and prevention of PHN.
