Konno Procedure for Managing Small Aortic Root during Aortic Valve Replacement Surgery: An Experience of 12 Cases

Background: Small aortic annulus during aortic valve replacement can lead to implanting a smaller sized valve compared to the body surface area thereby causing patient prosthesis mismatch. Various aortic root enlargement techniques have been described depending on anterior or posterior approach. Konno procedure uses anterior approach for aortic root enlargement. In this study, we reviewed results of Konno procedure done from 2011 to 2019 by a single surgeon. Methods: 12 adult patients who underwent aortic valve replacement along with Konno procedure for small aortic root by a single surgeon at a single center between 2011 and 2019 were reviewed. Echocardiographic and demographic data and post-operative data were obtained from medical records. Symptomatic profile was assessed as per New York Heart Association Classification. Intraoperative findings and post-operative period findings were noted. Follow up symptom profile was assessed for these patients. Results: 12 patients underwent Konno procedure between 2011 and 2019 for small aortic root along with valve replacement. The main indication for surgery was aortic stenosis with small aortic annulus, with or without involvement of the mitral valve. Preoperatively, 3 patients had NYHA class II and 9 patients had NYHA class III symptoms. Mean age at operation was 26.42 years, minimum age 10 years, and maximum age 39 years. 3 were females and 9 were males. Mean bypass time was 106.4 minutes and aortic cross clamp time was 80.67 minutes. Mechanical aortic valves were implanted in all patients. Mean post-operative blood loss was 134.2 ml and duration of ventilation before extubation was 14.5 hours. Mean duration of intensive care unit (ICU) stay was 2.83 days and hospital stay was 9.1 days. Mean gradient in the post-operative period was 10.75 mm Hg. There was no mortality in these 12 patients and no reoperation was needed in the follow up period. Follow up in the outpatient department suggested all patients had NYHA class I

R-spondins are secretory proteins localized in the endoplasmic reticulum and Golgi bodies and are processed through the secretory *** the R-spondin family,RSPO2 has emanated as a novel regulator of Wnt signaling,which has now been acknowledged in numerous in vitro and in vivo *** is an abnormal growth of cells that proliferates and spreads uncontrollably due to the accumulation of genetic and epigenetic factors that constitutively activate Wnt signaling in various types of *** cancer (CRC) begins when cells in the colon and rectum follow an indefinite pattern of division due to aberrant Wnt activation as one of the key ***-long progress in research on R-spondins has demonstrated their oncogenic function in distinct cancer types,particularly *** a critical regulator of the Wnt pathway,it modulates several phenotypes of cells,such as cell proliferation,invasion,migration,and cancer stem cell ***,RSPO mutations,gene rearrangements,fusions,copy number alterations,and altered gene expression have also been identified in a variety of cancers,including *** this review,we addressed the recent updates regarding the recurrently altered R-spondins with special emphasis on the RSPO2 gene and its involvement in potentiating Wnt signaling in *** addition to the compelling physiological and biological roles in cellular fate and regulation,we propose that RSPO2 would be valuable as a potential biomarker for prognostic,diagnostic,and therapeutic use in CRC.

This work investigates a prey-predator model featuring a Holling-type II functional response,in which the fear effect of predation on the prey species,as well as prey refuge,are ***,the model assumes that the growth rate of the prey population decreases as a result of the fear of ***,the detection of the predator by the prey species is subject to a delay known as the fear response delay,which is incorporated into the *** paper establishes the preliminary conditions for the solution of the delayed model,including positivity,boundedness and *** paper discusses the existence and stability of equilibrium points in the *** particular,the paper considers the discrete delay as a bifurcation parameter,demonstrating that the system undergoes Hopf bifurcation at a critical value of the delay *** direction and stability of periodic solutions are determined using central manifold and normal form ***,the global stability of the model is established at axial and positive equilibrium *** extensive numerical simulation is presented to validate the analytical findings,including the continuation of the equilibrium branch for positive equilibrium points.

Genus Prunus comprising around 430 species is a vast important genus of family Rosaceae, subfamily amygdalaoidae. Among all 430 species, around 19 important species are commonly found in Indian subcontinent due to their broad nutritional and economic importance. Some most common species of genus Prunus are Prunus amygdalus, Prunus persica, Prunus armeniaca, Prunus avium, Prunus cerasus, Prunus cerasoides, Prunus domestica, Prunus mahaleb, etc. A newly introduced species of Prunus i.e Prunus sunhangii is recently discovered which is morphologically very similar to Prunus cerasoides. Plants of Prunus species are short to medium-sized deciduous trees mainly found in the northern hemisphere. In India and its subcontinent,it extends from the Himalayas to Sikkim, Meghalaya,Bhutan, Myanmar etc. Different Prunus species have been extensively studied for their morphological,microscopic, pharmacological and phytoconstituents characteristics. Total phenolic content of Prunus species explains the presence of phenols in high quantity and pharmacological activity due to phenols. Phytochemical screening of species of genus Prunus shows the presence of wide phytoconstituents which contributes in their pharmacological significance and reveals the therapeutic potential and traditional medicinal significance of this genus. Genus Prunus showed a potent antioxidant activity analyzed by 1,1-diphenyl-2-picryl-hydrazyl radical assay. Plant species belonging to the genus Prunus is widely used traditionally for the treatment of various disorders. Some specific Prunus species possess potent anticancer, anti-inflammatory,hypoglycemic etc. activity which makes the genus more interesting for further research and findings. This review is an attempt to summarize the comprehensive study of Prunus species from its distribution, morphological characters to phytoconstituents, and pharmacological activity.

Background The impact of changing land-use patterns and associated anthropogenic threats on scale-dependent habitat use of semi-aquatic mustelids in scent-marking and social behaviour can provide important insights into the habitat ecology of smooth-coated otters(Lutrogale perspicillata).Methods We sampled 180 stream segments(100–400 m) as spatial replicates of 60 1-km2 sites to record indirect evidence(i.e. spraints and mass latrine sites) and group sizes of smooth-coated otters along the Tungabhadra Otter Conservation Reserve(TOCR) during the dry season. To quantify habitat, we recorded stream characteristics, riparian vegetation, and anthropogenic disturbances at the local scale, and hydro-environmental characteristics and land uses at the landscape scale. Using Markovian-chain detection and occupancy models, we assessed the multi-scale habitat use of otters in their selection of suitable areas for scent-marking based on repeated presence-absence data on spraint/latrine locations along the TOCR. We further used linear regression techniques to explore relationships between the number of individuals in smooth-coated otter group and hydro-environmental characteristics, spraint/latrine encounter rate, anthropogenic pressure, land cover, topography, and *** At the local scale, the probability of spraint deposition and group size decreased with anthropogenic disturbance while the probability of spraint detections decreased with grass cover. At the landscape scale, the probability of otter site use for spraint deposition and group sizes increased in southeast flowing streams. Biochar is a carbon sink material with the potential to improve water retention in various ***,for the long‐term maintenance of green infrastructure,there is an additional need to regulate the water contents in the covers to maintain vegetation growth in semiarid *** this study,biochar‐amended soil was combined with subsurface drip irrigation,and the water preservation characteristics of this treatment were investigated through a series of one‐dimensional soil column *** ascertain the best treatment method specific to semiarid climatic conditions,the test soil was amended with 0%,1%,3%,and 5%*** irrigation devices equipped with soil moisture sensors were used to control the subsurface water content with the aim of enhancing vegetation *** soil column test lasted 150 h,during which the volumetric water contents and soil suction data were *** experimental results reveal that the soil specimen amended with 3%biochar is the most water‐saving regardless of the time *** with a higher biochar content(e.g.,5%)consumes a more significant amount of water due to the enhancement of the water‐holding *** on the experimental results,it can be concluded that the appropriate ratio can be determined within 1%–3%,which can reduce not only the amount of irrigated/used water but also the time *** technology can be explored for water content regulation in green infrastructure and the development of barriers for protecting the environment around deep underground waste containment.

The Kathmandu Valley has seen substantial urbanization over the past decades while being the nation’s economic centre. Built-up areas have expanded quickly along with the population, having a significantly negative influence on the environment. Recently, Kathmandu was named as the most polluted city in Asia. Urban sprawl has had a negative influence on Kathmandu’s residents in several ways. The state of urban sprawl and the effects it has had on the Kathmandu Valley have been examined using land sat imagery. In this study, IDW was used in GIS to analyze the pollution status using data of PM 2.5 and PM 10 obtained from various monitoring sites. A supervised classification was used to create a LULC map of Kathmandu for the years 2015, 2018, and 2020. To assess the state of the vegetation and determine whether the Kathmandu Valley is being affected by urban heat, NDVI and Land sat temperature calculations were also made. The study’s results were obtained using remote sensing and GIS technology. The built-up area in Kathmandu Valley has grown by 20% over the past five years, impacting land use patterns and deteriorating vegetation cover. Due to the rise of built-up area, which is a good heat absorber, the temperature in the Kathmandu Valley is rising along with the degradation of the vegetation cover. The pollution in the Kathmandu Valley is at its worst, and residents are compelled to breathe air that is significantly more polluted than the prescribed limit.

Comparative study of thulium fiber laser versus holmium:yttrium-aluminum-garnet laser for ureteric stone management with semi-rigid ureteroscopy: A prospective, single-center study

Objective:To compare the efficacy and safety of thulium fiber laser(TFL)and holmium:yttrium-aluminum-garnet(Ho:YAG)laser for ureteric stone management with semi-rigid ***:In a prospective study from January 2020 to December 2021,we compared 40 patients in each group who underwent semi-rigid ureteroscopic lithotripsy with TFL and that with Ho:YAG *** volume,stone density,stone fragmentation rates,total lasing time,total operative time,endoscopic vision,retropulsion and stone free rates were analyzed in both groups and ***:Mean stone volume was comparable in the TFL group and the Ho:YAG laser group(282.45[standard deviation,SD 139.79]mm3 vs.279.49[SD 312.52]mm3;p=0.964).Mean stone density was also comparable in the TFL group and the Ho:YAG laser group(1135.30[SD 317.04]Hounsfield unit vs.1131.75[SD 283.03]Hounsfield unit;p=0.959).The mean stone fragmentation rates calculated as stone volume divided by lasing time were 25.85(SD 10.61)mm3/min and 21.37(SD 14.13)mm3/min in the TFL group and the Ho:YAG laser group,respectively(p=0.113).The mean total lasing time(10.15[SD]4.69 min vs.11.43[SD 4.56]min;p=0.222),mean operative time(25.13[SD 9.51]min vs.25.54[SD 10.32]min;p=0.866),and mean total hospital stay(2.62[SD 0.77]days vs.2.61[SD 0.84]days;p=0.893)were comparable in the TFL group and in the Ho:YAG *** vision was better and retropulsion was less in the TFL *** stone-free rate at 1 month postoperatively was slightly better in the TFL group(100%vs.90%;p=0.095).
