Reconstruction of post-traumatic upper extremity soft tissue defects with pedicled flaps: An algorithmic approach to clinical decision making

Purpose:Pedicled flaps are still the workhorse flaps for reconstruction of upper limb soft tissue defects in many centers across the world. They are lifeboat options for coverage in vessel deplete wounds. In spite of their popularity existing algorithms are limited to a particular region of upper limb;a general algorithm involving entire upper limb which helps in clinical decision making is lacking. We attempt to propose one for the day to day clinical practice.Methods:A retrospective analysis of patients who underwent pedicled flaps for coverage of post-traumatic upper extremity (arm, elbow, forearm, wrist & hand) soft tissue defects within the period of January 2016 to October 2017 was performed. Patients were divided into groups according to the anatomical location of the defects. The flaps performed for different anatomical regions were enlisted. Demographic data and complications were recorded. An algorithm was proposed based on our experience, with a particular emphasis made to approach to clinical decision making.Results:Two hundred and twelve patients were included in the study. Mean age was 27.3 years (range: 1-80 years), 180 were male, and 32 were female. Overall flap success rate was 98%, the following complications were noted marginal flap necrosis requiring no additional procedure other than local wound care in 32 patients (15%), partial flap necrosis requiring flap advancement or extra flap in 15 patients (7%), surgical site infection in 11 patients (5%), flap dehiscence requiring re-suturing in 5 patients (2.4%), total flap necrosis 4 patients (2%).Conclusion:The proposed algorithm allows a reliable and consistent method for addressing diverse soft tissue defects in the upper limb with high success rate.

Changes in Soil Physicochemical Properties under Selected Climate-Smart Agricultural Practices in Central African Republic

Soil nutrient depletion as a result of continuous cultivation with insufficient external nutrient replenishment is a major challenge in sub-Saharan Africa (SSA). A trial using the organic and mineral fertilizer was established in the cassava cropping system of the Pissa and Damara areas to address the declining soil fertility. The study aimed to evaluate the effect of cow manure and mineral fertilizer in combination or sole application on soil physico-chemical properties and macronutrients. The experimental trial adopted a randomized complete block design (RCBD) replicated four times. The treatments comprised of;T1 (Control), T2 (Peasant practice), T3 (sole NPK), and T4 (Cow manure + NPK). The data on soil physicochemical analysis was subjected to a one-way analysis of variance in SAS 9.4 and the mean was separated by Tukey’s HSD test at p < 0.005. The soil pH values ranged from 4.20 to 4.91;and 4.53 to 5.28 in Pissa and Damara respectively. According to the treatments, a low pH value is observed in T3 (4.13) in the Pissa. Combined use of cow manure and the mineral fertilizer resulted in higher Mg K and N in the Pissa region and higher soil pH in Damara. The use of sole NPK (T3) gave a higher soil carbon and CN ratio. In the Pissa region the CEC, Cu, Fe, and Zn were higher in the treatments with mineral fertilizer compared to the control. Conclusively, the use of mineral fertilizer and cow manure can be used with optimum rates to improve soil physico-chemical properties on a sustainable basis.

IQR-based approach for DDoS detection and mitigation in SDN

Software-defined networking(SDN) is a trending networking paradigm that focuses on decoupling of the control logic from the data plane. This decoupling brings programmability and flexibility for the network management by introducing centralized infrastructure. The complete control logic resides in the controller, and thus it becomes the intellectual and most important entity of the SDN infrastructure. With these advantages, SDN faces several security issues in various SDN layers that may prevent the growth and global adoption of this groundbreaking technology. Control plane exhaustion and switch buffer overflow are examples of such security issues. Distributed denial-of-service(DDoS) attacks are one of the most severe attacks that aim to exhaust the controller’s CPU to discontinue the whole functioning of the SDN network. Hence, it is necessary to design a quick as well as accurate detection scheme to detect the attack traffic at an early stage. In this paper, we present a defense solution to detect and mitigate spoofed flooding DDoS attacks. The proposed defense solution is implemented in the SDN controller. The detection method is based on the idea of an statistical measure — Interquartile Range(IQR). For the mitigation purpose, the existing SDN-in-built capabilities are utilized. In this work, the experiments are performed considering the spoofed SYN flooding attack. The proposed solution is evaluated using different performance parameters, i.e., detection time, detection accuracy, packet_in messages, and CPU utilization. The experimental results reveal that the proposed defense solution detects and mitigates the attack effectively in different attack scenarios.

1.Introduction There is a growing body of research supporting the idea that a plethora of health benefits can result from resistance training using a low volume,high-effort approach(e.g.,increased metabolic rate and bone mineral density,a reduction in blood pressure,and improved muscle quality and insulin sensitivity,among others.Further,muscular strength and muscle massprimary goals and benefits of participation in resistance trainingare independently strong predictors of longevity and quality of life. Thus,many have sought to identify how the manipulation of resistance training variables might lead to“optimal”adaptations in such outcomes,as evidenced by repeated attempts over recent decades to review the literature and provide consensus statements on this topic.5-10 One variable that is often hotly debated within resistance training is training to failure.

Evaluation of the cracking and aging susceptibility of asphalt mixtures using viscoelastic properties and master curve parameters

Aging can significantly affect the performance of asphalt mixtures, causing increase in stiffness, reduction in relaxation capability and increase in cracking susceptibility. It is also well known that fundamental viscoelastic properties are used for design and modelling of asphalt mixtures and pavement structures to addressing rutting, fatigue and thermal cracking concerns. The objective of this paper is to study how the viscoelastic properties of asphalt mixture change over time, and evaluate and identify the cracking and aging susceptibility of asphalt mixtures with different mix variables during material selection and mixture design. Ten mixtures are evaluated using different laboratory conditioning protocols to simulate a range of aging levels in the field. The complex modulus test is then conducted on the lab aged mixtures to measure the viscoelastic properties in order to construct the dynamic modulus and phase angle master curves. The mixture Glover-Rowe(G-Rm) parameter and the shape parameters of the dynamic modulus and phase angle master curves, including inflection point frequency(-β/γ), difference between the glassy modulus and the inflection point modulus(γ), peak value of phase angle(a) and the horizontal position(frequency) of the peak phase angle value(c), are determined and evaluated for the mixtures with different aging conditions and mix variables. The study indicates the ability of the G-Rmparameter and all the master curve shape parameters to capture the effect of different aging conditions on linear viscoelastic mixture properties, as well as the cracking and aging susceptibility of asphalt mixtures.

The integration of renewable distributed generation(RDG)into distribution networks is promising and increasing nowadays.However,high penetration levels of distributed generation(DG)are often limited as they may have an adverse effect on the operation of distribution networks.One of the operation challenges is the interaction between DG and voltage-control equipment,e.g.,an under-load tap changer(ULTC),which is basically designed to compensate for voltage changes caused by slow load variations.The integration of variable DGs leads to rapid voltage fluctuations,which can negatively affect the tap operation of ULTC.This paper investigates the impact of high penetration levels of RDG on the tap operation of ULTC in distribution networks through simulations.Various mitigation techniques that can alleviate this impact are also examined.Among these techniques,constant power-factor mode is regarded as the best trade-off between the simplicity and effectiveness of minimizing the number of tap operations.Simulations are performed on a Canadian benchmark rural distribution feeder using OpenDSS software.

Any alternatives to rice? Ethnobotanical insights into the dietary use of edible plants by the Higaonon tribe in Bukidnon Province, the Philippines

Though considered an agricultural country,the Philippines is the world’s largest importer of rice.The persistent problem of insufficient rice supply,however,has been exacerbated by economic crises and natural calamities.Yet,for the Higaonon tribe in Bukidnon Province,the Philippines,the rich agrobiodiversity and wild edible plants are vital for food security and resilience since the mountainous terrain in this province presents a challenge for rice cultivation.To gain insight from the indigenous edible plant knowledge of the Higaonon tribe,we conducted an ethnobotanical research to document the diversity,utilization,and biocultural refugia of both cultivated and wild edible plants.A total of 76 edible plant species belonging to 62 genera and 36 botanical families were documented.The most represented botanical families included the Fabaceae,Solanaceae,and Zingiberaceae.In terms of dietary usage,3 species were categorized as cereals;8 species were white roots,tubers,and plantains;3 species were vitamin A-rich vegetables and tubers;16 species were green leafy vegetables;12 species were categorized as other vegetables;2 species were vitamin A-rich fruits;27 species were classified as other fruits;7 species were legumes,nuts,and seeds;and 8 species were used as spices,condiments,and beverages.Using the statistical software R with ethnobotanyR package,we further calculated the ethnobotanical indices(use-report(UR),use-value(UV),number of use(NU),and fidelity level(FL))from 1254 URs in all 9 food use-categories.The species with the highest UV and UR were from a variety of nutrient-rich edible plants such as Ipomoea batatas(L.)Lam.,Musa species,Colocasia esculenta(L.)Schott,Zea mays L.,and Manihot esculenta Crantz.The extensive utilization of root and tuber crops along with corn and plantain that contain a higher amount of energy and protein,carbohydrates,minerals,and vitamins were shown to be an important nutrient-rich alternatives to rice.Whilst males appeared to be more knowledgeable of edible plant species collected

土体冲蚀流失是塌岸发展和库岸再造的重要诱发因素。为准确预测水流作用下岸坡土体流失进程,自主研制了能够实时精确量测土样质量变化的新型冲蚀试验装置,开展三峡库岸粉质黏土冲刷起动和冲蚀进程试验,分析有限厚度土体冲蚀演化规律和影响机制,并建立预测模型。结果表明:有限厚度土体的总冲蚀量随时间增加但增速逐渐趋缓,冲蚀速率逐渐衰减,且冲蚀速率及其衰减程度均与孔隙比或流速成正比;将反映密实度的孔隙比和反映剪切力的水流速度嵌入预测模型能较好地拟合粉质黏土冲蚀演化进程曲线。建立的有限厚度土体冲蚀进程预测模型,为水流冲刷作用下岸坡形态变化趋势预测研究提供参考。
