Primary liver transplantation vs transplant after Kasai portoenterostomy in children with biliary atresia: A retrospective Brazilian single-center cohort

BACKGROUND Biliary atresia(BA)is the most common indication for pediatric liver transplantation,although portoenterostomy is usually performed first.However,due to the high failure rate of portoenterostomy,liver transplantation has been advocated as the primary procedure for patients with BA.It is still unclear if a previous portoenterostomy has a negative impact on liver transplantation outcomes.AIM To investigate the effect of prior portoenterostomy in infants un-dergoing liver transplantation for BA.METHODS This was a retrospective cohort study of 42 pediatric patients with BA who underwent primary liver transplantation from 2013 to 2023 at a single tertiary center in Brazil.Patients with BA were divided into two groups:Those undergoing primary liver transplantation without portoenterostomy and those undergoing liver transplantation with prior portoenterostomy.Continuous variables were compared using the Student’s t-test or the Kruskal-Wallis test,and categorical variables were compared using theχ2 or Fisher’s exact test,as appropriate.Multivariable Cox regression analysis was performed to determine risk factors for portal vein thrombosis.Patient and graft survival analyses were conducted with the Kaplan–Meier product-limit estimator,and patient subgroups were compared using the two-sided log-rank test.RESULTS Forty-two patients were included in the study(25[60%]girls),23 undergoing liver transplantation without prior portoenterostomy,and 19 undergoing liver transplantation with prior portoenterostomy.Patients with prior portoenterostomy were older(12 vs 8 months;P=0.02)at the time of liver transplantation and had lower Pediatric End-Stage Liver Disease scores(13.2 vs 21.4;P=0.01).The majority of the patients(35/42,83%)underwent livingdonor liver transplantation.The group of patients without prior portoenterostomy appeared to have a higher incidence of portal vein thrombosis(39 vs 11%),but this result did not reach statistical significance.Prior portoenterostomy was not a protective factor against portal vein

Liver volumetric and anatomic assessment in living donor liver transplantation: The role of modern imaging and artificial intelligence

The shortage of deceased donor organs has prompted the development of alternative liver grafts for transplantation.Living-donor liver transplantation(LDLT)has emerged as a viable option,expanding the donor pool and enabling timely transplantation with favorable graft function and improved long-term outcomes.An accurate evaluation of the donor liver’s volumetry(LV)and anatomical study is crucial to ensure adequate future liver remnant,graft volume and precise liver resection.Thus,ensuring donor safety and an appropriate graftto-recipient weight ratio.Manual LV(MLV)using computed tomography has traditionally been considered the gold standard for assessing liver volume.However,the method has been limited by cost,subjectivity,and variability.Automated LV techniques employing advanced segmentation algorithms offer improved reproducibility,reduced variability,and enhanced efficiency compared to manual measurements.However,the accuracy of automated LV requires further investigation.The study provides a comprehensive review of traditional and emerging LV methods,including semi-automated image processing,automated LV techniques,and machine learning-based approaches.Additionally,the study discusses the respective strengths and weaknesses of each of the aforementioned techniques.The use of artificial intelligence(AI)technologies,including machine learning and deep learning,is expected to become a routine part of surgical planning in the near future.The implementation of AI is expected to enable faster and more accurate image study interpretations,improve workflow efficiency,and enhance the safety,speed,and cost-effectiveness of the procedures.Accurate preoperative assessment of the liver plays a crucial role in ensuring safe donor selection and improved outcomes in LDLT.MLV has inherent limitations that have led to the adoption of semi-automated and automated software solutions.Moreover,AI has tremendous potential for LV and segmentation;however,its widespread use is hindered by cost and availability.Therefore,the integrati

This study investigates the influence of electropolymerization conditions on the deposition of polypyrrole(PPy)onto cotton-derived carbon fiber(CF)modified with reduced graphene oxide(rGO)for supercapacitors applications using an experimental/theorical approach.The surface modification of CF by rGO and/or by PPy electrodeposited at 10,25 and 50 mV s^(-1) was thoroughly examined physicochemical and electrochemically.Composite electrodes comprising CF-rGo-PPy,synthesized via electropolymerization at 25 mV s^(-1),demonstrated a remarkable increase in capacitance,showcasing~742 F g^(-1) compared to 153 F g^(-1) for CF.SEM,N_(2)-surface area,XPS,and TD-DFT approach revealed that the higher capacitance observed in CF-rGo-PPy electrodes underscores the influence of morphology and charged nitrogen species on the electrochemical performance of these modified electrodes.Notably,this electrode material achieves a specific capacitance retention of~96%of their initial capacitance after 10000 cycles at 0.5 A g^(-1) measured in a two-electrodes cell configuration.This work also discusses the influence of the scan rate used for pyrrole electropolymerization on the pseudocapacitance contribution of PPy and its possible effect on the porosity of the material.These results highlight the importance of appropriate electropolymerization conditions that allow obtaining the synergistic effect between CF,rGO and PPy.

Water stress resilient cereal crops:Lessons from wild relatives

Cereal crops are significant contributors to global diets.As climate change disrupts weather patterns and wreaks havoc on crops,the need for generating stress-resilient,high-yielding varieties is more urgent than ever.One extremely promising avenue in this regard is to exploit the tremendous genetic diversity expressed by the wild ancestors of current day crop species.These crop wild relatives thrive in a range of environments and accordingly often harbor an array of traits that allow them to do so.The identification and introgression of these traits into our staple cereal crops can lessen yield losses in stressful environments.In the last decades,a surge in extreme drought and flooding events have severely impacted cereal crop production.Climate models predict a persistence of this trend,thus reinforcing the need for research on water stress resilience.Here we review:(i)how water stress(drought and flooding)impacts crop performance;and(ii)how identification of tolerance traits and mechanisms from wild relatives of the main cereal crops,that is,rice,maize,wheat,and barley,can lead to improved survival and sustained yields in these crops under water stress conditions.

According to World Health Organization(WHO)estimates,in 2020,there were 1.8 million human immunodeficiency virus(HIV)-infected children below 15 years old living with HIV and 150,000 children who acquired the infection[1].Every year,110,000 children die as a consequence of acquired immune deficiency syndrome-related causes[2].As in adults,optimal management of infected children depends on early diagnosis and treatment with antiretroviral drugs based on their viral genotype.

Redox, immune and genetic biomarker system for personalized treatments in colorectal cancer

BACKGROUND Identifying biomarkers for the risk of developing degenerative processes linked to aging and colorectal cancer(CRC) onset that could improve clinical strategies.AIM To determine valid targets and a predictive biomarker's system of chronicization of inflammation for cancer treatment.METHODS A group of 147 CRC patients was studied. Clinical diagnosis was confirmed histopathologically, and patients were sub-typed using the pathological tumornode-metastasis classification. Thirteen colon adenoma patients and 219 healthy subjects were also studied. A system biology study on Thioredoxin1/CD30 redox-immune systems(Trx1/CD30), T helper cytokines and polymorphisms of killer immunoglobulin-like receptors, FcγRIIa-131 H/R and FcγRIIIa-158 V/F was carried out. Enzyme-linked immunosorbent assay was performed to analyze sera.Genetic study was executed by polymerase chain reaction sequence-specific primers and sequence-based typing method. Statistical analysis was performed by using the "Statgraphics software systems".RESULTS We found a positive increase between Trx1/RTrx1 levels and sCD30 level and increased age. With respect to the gender relationships, there were distinct differences. Females showed a primary relationship between transforming growth factor beta(TGFβ) with Trx1, whereas males had one with TGFβ and RTrx1. Trx1/CD30 controls the redox immune homeostasis, and an imbalance in the relationship between the Trx1/RTrx1 and sCD30 levels is linked to the onset and progression of tumor. This event happens through different gender-specific cytokine pathways. Our study demonstrated that the serum levels ofTrx1/RTrx1, TGFβ/interleukin(IL)6 and TGFβ/IL4 combinations and the sCD30,IFNγ and IL2 combination constitute a predictive gender specific biomarker system. This is relevant for clinical screening to detect the risk of the potential development or progression of a tumor.CONCLUSION Oxidative stress on Trx1/CD30 is a trigger of cancer disease, and the selected oxidation and immune products are a biomarker system

Notch signaling in female cancers: a multifaceted node to overcome drug resistance

Drug resistance is one of the main challenges in cancer therapy,including in the treatment of female-specific malignancies,which account for more than 60%of cancer cases among women.Therefore,elucidating the underlying molecular mechanisms is an urgent need in gynecological cancers to foster novel therapeutic approaches.Notably,Notch signaling,including either receptors or ligands,has emerged as a promising candidate given its multifaceted role in almost all of the hallmarks of cancer.Concerning the connection between Notch pathway and drug resistance in the afore-mentioned tumor contexts,several studies focused on the Notch-dependent regulation of the cancer stem cell(CSC)subpopulation or the induction of the epithelial-to-mesenchymal transition(EMT),both features implicated in either intrinsic or acquired resistance.Indeed,the present review provides an up-to-date overview of the published results on Notch signaling and EMT-or CSC-driven drug resistance.Moreover,other drug resistance-related mechanisms are examined such as the involvement of the Notch pathway in drug efflux and tumor microenvironment.Collectively,there is a long way to go before every facet will be fully understood;nevertheless,some small pieces are falling neatly into place.Overall,the main aim of this review is to provide strong evidence in support of Notch signaling inhibition as an effective strategy to evade or reverse resistance in female-specific cancers.

Features of the motion of gel particles in a three-phase bubble column with non-foaming and foaming gas–liquid systems,determined by using experiments of radioactive particle tracking(RPT),have been compared.The tracer used is a gel particle which resembles typical immobilized biocatalyst.The tracer trajectory is analyzed to extract relevant information for design purposes.The solid velocity field,turbulence parameters,dispersion coefficients,mixing times and flow transitions are determined and compared.The presence of foam significantly affects many quantified parameters,especially within the heterogeneous flow regime.The hydrodynamic stresses are reduced in the presence of foam,especially close to the disengagement.The dispersion coefficients also decrease,and the solid mixing time is only slightly affected by the presence of foam.Gas holdup,inferred both from RPT experiments and from gamma ray scanning,is higher for foaming systems and leads to a shift in the transition gas velocity towards higher values.

Optic nerve compression:the role of the lamina cribrosa and translaminar pressure

AIM： To describe the morphological changes of the lamina cribrosa （LC） in patients with optic nerve compression. METHODS： Cross-sectional study. Twenty eyes with optic nerve compression, affected by Graves＇ ophthalmopathy （GO） were compared with 18 refractive error-matched healthy eyes. The following examinations were performed： best-corrected visual acuity （BCVA）, intraocular pressure, optic nerve echography, visual field, SD-OCT including the retinal nerve fiber layer （RNFL）, ganglion cell complex （GCC）, and LC thickness and extent. RESULTS： A-scan revealed significant differences in the subarachnoid space （SAS） between the affected and control groups. LC thickness and LC area were 233 pm （SD 23） and 0.41 mm2 （SD 0.19）, respectively. Average GCC thickness （P=-0.0005）, LC thickness （P=-0.001）, MD （P=-0.001） and PSD （P=-0.001） differed significantly between the two groups; whereas LC area （P=-0.2） and average RFNL （P=-0.1） did not. CONCLUSION： Optic nerve compression reduces the SAS thereby altering the morphology of LC thickness and causing GCC damage.

Elderly Feeding and Nutrition during Hospitalization: An Integrative Review

Objective: to search in the current literature the elements that exert an influence on feeding and nutrition in hospitalized aged people. Method: the following strategies were used for the integrative review stages: research question (Population or Patients;Exposure;Outcomes);analysis flowchart (Preferred Reporting Items for Systematic Reviews and Meta-analyses);and levels of evidence corresponding to the studies (Oxford Centre for Evidence-Based Medicine). The searches were conducted in the MEDLINE/PubMed, Biblioteca Virtual em Saúde, Embase, CINAHL and Scopus databases. Results: the initial search yielded 1808 studies, of which 34 comprised the analysis corpus after applying the inclusion/exclusion criteria. The assessments corresponding to nutritional status and to the risk factors for hospitalized aged people are fundamental in defining the diet. An association was found between inadequate energy intake, lack of appetite, infections, malignity, delirium and need for assistance in feeding. Dysphagia affected nutritional status and was associated with multimorbidities, cognitive impairment, malnutrition, higher dependence for the activities of daily living, and greater care need. Nutritional support improved the perioperative nutritional status, in addition to reducing the hospitalization times and the number of infectious complications. Conclusion: the main disorder that hinders meeting the nutritional needs is dysphagia, and is associated with aged people’s multimorbidity and dependence. The results provide geriatric and gerontological knowledge about elderly nutrition, in addition to targeting preventive and intervention treatments and care during hospitalization. .
