Non-initiation of hepatitis C virus antiviral therapy in patients with human immunodeficiency virus/hepatitis C virus co-infection

AIM: To assess whether reasons for hepatitis C virus(HCV) therapy non-initiation differentially affect racial and ethnic minorities with human immunodeficiency virus(HIV)/HCV ***: Analysis included co-infected HCV treatment-na?ve patients in the University of North Carolina CFAR HIV Clinical Cohort(January 1, 2004 and December31, 2011). Medical records were abstracted to document non-modifiable medical(e.g., hepatic decompensation, advanced immunosuppression), potentially modifiable medical(e.g., substance abuse, severe depression, psychiatric illness), and non-medical(e.g., personal,social, and economic factors) reasons for non-initiation. Statistical differences in the prevalence of reasons for non-treatment between racial/ethnic groups were assessed using the two-tailed Fisher's exact test. Three separate regression models were fit for each reason category. Odds ratios and their 95%CIs(Wald's) were ***: One hundred and seventy-one patients with HIV/HCV co-infection within the cohort met study inclusion. The study sample was racially and ethnically diverse; most patients were African-American(74%), followed by Caucasian(19%), and Hispanic/other(7%). The median age was 46 years(interquartile range = 39-50) and most patients were male(74%). Among the 171 patients, reasons for non-treatment were common among all patients, regardless of race/ethnicity(50% with ≥ 1 non-modifiable medical reason, 66% with ≥1 potentially modifiable medical reason, and 66% with ≥ 1 non-medical reason). There were no significant differences by race/ethnicity. Compared to Caucasians, African-Americans did not have increased odds of nonmodifiable [adjusted odds ratio(a OR) = 1.47, 95%CI: 0.57-3.80], potentially modifiable(a OR = 0.72, 95%CI: 0.25-2.09) or non-medical(a OR = 0.90, 95%CI: 0.32-2.52) reasons for ***: Race/ethnicity alone is not predictive of reasons for HCV therapy non-initiation. G protein-coupled receptors （GPCRs） are involved in all human physiological systems where they are responsible for transducing extracellular signals into cells. GPCRs signal in response to a diverse array of stimuli including light, hormones, and lipids, where these signals affect downstream cascades to impact both health and disease states. Yet, despite their importance as therapeutic tar- gets, detailed molecular structures of only 30 GPCRs have been determined to date. A key challenge to their structure determination is adequate protein expression. Here we report the quantification of protein expression in an insect cell expression system for all 826 human GPCRs using two different fusion constructs. Expression char- acteristics are analyzed in aggregate and among each of the five distinct subfamilies. These data can be used to identify trends related to GPCR expression between dif- ferent fusion constructs and between different GPCR families, and to prioritize lead candidates for future structure determination feasibility.

AIM： To evaluate the possible differences in morphology and immunohistochemical expression of CD3, transforming growth factor 131（TGF-131）, Smad7, α-smooth muscle actin （α-Sma）, and collagen types Ⅰ-Ⅶ of small and large intestine in Smad3 null and wild-type mice. METHODS： Ten null and ten wild-type adult mice were sacrificed at 4 mo of age and the organs （esophagus, small and large bowel, ureters） were collected for histology（hematoxylin and eosin, Masson thrichrome, silver staining）, morphometry and immunohistochemistry analysis. TGF-β1 levels of intestinal tissue homogenates were assessed by ElISA. RESUlTS： No macroscopic intestinal lesions were detected both in null and wild-type mice. Histological and morphometric evaluation revealed a significant reduction in muscle layer thickness of small and large intestine in null mice as compared to wild-type mice. Immunohistochemistry evaluation showed a significant increase of CD3＋T cell, TGF-β1 and Smad7 staining in the small and large intestine mucosa of Smad3 null mice as compared to wild-type mice. α-Sma and collagen Ⅰ-Ⅶ staining of small and large intestine did not differ between the two groups of mice. TGF-β1 levels of colonic tissue homogenates were significantly higher in null mice than in wildtype mice. In preliminary experiments a significant reduction of TNBS-induced intestinal fibrosis was observed in null mice as compared to wild-type mice.

Background: The present work aimed at evaluating the effect of the dietary replacement of soybean oil(S) by two types of insect fats extracted from black soldier fly larvae(H, Hermetia il ucens l.) and yel ow mealworm larvae(T, Tenebrio molitor l.) on growth performance, nutrient digestibility, blood parameters, intestinal morphology and health of growing ***: At weaning, 200 crossbred rabbits(36 days old) were allotted to five dietary treatments(40 rabbits/group): a control diet(C) containing 1.5% of soybean oil and four experimental diets where soybean oil was partially(50%) or totally(100%) substituted by H(H50 and H100) or T(T50 and T100) fats. Total tract digestibility was evaluated on 12 rabbits per treatment. The growth trial lasted 41 d and, at slaughtering(78 days old), blood samples were collected from 15 rabbits per treatment, morphometric analyses were performed on duodenum, jejunum and ileum mucosa,and samples of liver, spleen and kidney were submitted to histological ***: No difference was observed between the control and the experimental groups fed insect fats in terms of performance, morbidity, mortality and blood variables. The addition of H and T fats did not influence apparent digestibility coefficients of dry matter, protein, ether extract, fibre fractions and gross energy. Gut morphometric indices and organ histopathology were not affected by dietary inclusion of H and T ***: H and T fats are suitable sources of lipid in rabbit diets to replace soybean oil without any detrimental effect on growth performance, apparent digestibility, gut mucosa traits and health.

Influence of CYP2B6 516G > T and long Term HAART on Population Pharmacokinetics of Efavirenz in Rwandan Adults on HIV and Tuberculosis Cotreatment

Aim: To describe the pharmacokinetic parameters of efavirenz and estimate its clearance (Cl/F) accounting simultaneously for drug-drug interactions and CYP2B6 genetic polymorphism. Methods: Genotyping of 516G > T single nucleotide polymorphism of CYP2B6 was performed using a PCR-based technology and plasma efavirenz concentrations were measured by high performance liquid chromatography on blood samples from 76 HIV adults co-infected with tuberculosis who had received an efavirenz-based regimen. Data were analyzed using population modeling with NONMEM. Results: The absorption rate constant and the apparent volume of distribution in the final model were 1.9 h-1 and 580 l/70kg, respectively. The Cl/F at baseline was 11.8 l/h/70kg, 8.8 l/h/70kg and 3.9 l/h/70kg for patients carrying the G/G, G/T and T/T genotypes of CYP2B6 516G > T, respectively, in patients who were administered tuberculosis (TB) treatment prior to HIV treatment (Group A);and 16.7 l/h/70kg, 10.6 l/h/70kg and 1.8 l/h/70kg for G/G, G/T and T/T genotype patients respectively, in patients with previous exposure to HIV treatment (Group B). The Cl/F at baseline and steady state was always higher in Group B compared to Group A patients. Expectedly, carriers of CYP2B6 516G/G and T/T genotypes exhibited higher and lower Cl/F, respectively. Conclusion: Our results indicated that the Cl/F of efavirenz in the population studied was predictably different due to whether the patients were mono-treated for TB with HAART deferred or for HIV before initiation of TB therapy, and to CYP2B6 516G > T variant, implying that both CYP2B6 genetic polymorphisms and previous efavirenz-based HAART should be taken into account when adjusting efavirenz dose.

镇江市位于长江和京杭大运河"十字黄金水道"的交汇处,是我国历史上最为重要的水上交通枢纽之一,区域内水量充沛、河网密布.近年来,该区域水环境存在不同程度的退化,故调查镇江市主要河湖水体及沉积物污染现状,将为当地水环境质量的改善提供重要的基础数据.基于此,本研究选择镇江市典型河湖水体共21个点位(金山湖10个点位,古运河及其支流共11个点位),在冬夏两季采集上覆水及沉积物样品测定其理化指标和氮素含量,并通过有机指数及有机氮对沉积物污染程度进行评价.结果表明:(1)上覆水总氮(TN)浓度在冬夏季节分别介于1.95~15.71 mg·l^(-1)和0.64~12.09 mg·l^(-1)之间,平均值为4.01 mg·l^(-1)和4.07 mg·l^(-1),平均值浓度高于地表水Ⅴ类标准.上覆水氨氮(NH^(+)_(4)-N)浓度在季节上均表现为冬季湖泊;(2)沉积物TN含量在冬夏两季分别介于394.61~3288.09 mg·kg^(-1)和869.21~3598.04mg·kg^(-1)之间,平均值为1928.58 mg·kg^(-1)和2068.40 mg·kg^(-1).湖泊沉积物TN、NH^(+)_(4)-N、硝态氮(NO^(-)_(3)-N)和有机氮(Org-N)含量均表现为冬季>夏季,而河流沉积物TN平均含量为冬季夏季,表明河流沉积物在季节上的分布规律更为复杂;空间上,冬夏两季沉积物NO^(-)_(3)-N表现为湖泊>河流,NH^(+)_(4)-N含量则表现为湖泊<河流.C/N结果表明研究区域沉积物有机质主要来源于内源污染;(3)湖泊和河流沉积物超过60%点位属于有机氮污染Ⅳ水平,即存在有机氮污染,而超过70%和54.55%的湖泊和河流沉积物处于有机指数Ⅱ类水平,即较清洁水平,表明镇江市河湖沉积物主要为氮素污染.以上研究结果将为镇江市河流水体及沉积物氮素控制提供重要的基础数据.

Severe acute respiratory syndrome coronavirus 2 has thoroughly reshaped the medical landscape. Much has been written and discussed of the adaptations required by this pandemic, particularly in the realm of elective medical care. While some areas of the country have, in recent weeks, seen a plateau or even a decrease in coronavirus 2019 case-burden, others remain face-to-face with significant ongoing morbidity and mortality. Furthermore, given eased restrictions across numerous states and municipalities, a widespread resurgence of the disease is not precluded. To that end, we have attempted to summarize experiences and best practices in the handling of breast imaging against the backdrop of the novel coronavirus, and we consider future directions.

重金属作为主要的持久性环境污染物之一,由于其具有强毒性以及可以在生物体内积累、在食物网中富集等特点,对水环境质量的影响巨大,尤其是毒害性较大的铅（Pb）、铬（Cr）、镉（Cd）、锌（Zn）、铜（Cu）等元素,成为影响淡水生态系统的最严重的污染物之一,已经成为全球性的棘手问题而备受关注。近几十年,伴随着城市化、工业化以及农业的迅速发展,以及人口数量增加和人类活动的增强,淡水生态系统中重金属含量急剧增加,毒性效应逐渐加剧,对环境产生极大的风险。因此,研究重金属在自然水体中的分布特征评估其生态风险,将对水环境质量的改善和重金属污染物的控制起到至关重要的作用。综合现有研究成果后发现,由于河湖自身形态、水文、污染来源等差异,导致河流水体重金属污染水平以及生态风险差异较大。针对特定水域开展重金属污染分布特征的调查与评价研究,将为所选区域重金属的综合治理提供重要的基础数据。基于此,本研究以河湖密布、水量充沛的江苏省镇江市为研究区域,在该区域代表性湖泊和河流中共选择21个采样点,分别在冬、夏两季采集上覆水体及表面沉积物样品,测定重金属Cd、Cr、Cu、Pb以及Zn含量;采用单因素污染指数和内梅罗污染指数对上覆水体的污染指数进行评价;利用污染因子、地累积指数和潜在生态风险等对沉积物中重金属的污染水平及风险进行评估;通过相关性、主成分等多元分析方法,对上覆水样和表层沉积物中重金属的可能来源进行了辨析。研究结果将为镇江市河湖水体及沉积物重金属污染防治提供基础数据。研究结果表明:（1）上覆水体中重金属分布特征及生态风险评价结果表明:冬季采集的上覆水体中Cd、Cr、Cu、Pb、Zn平均含量分别为0.50、2.25、3.33、1.30和21.92μg/l,夏季上覆水样品中Cd、Cr、Cu、Pb、Zn平均含量则为0.40、1.91、2.72、1.23和30.76μg/l,该浓度与我国国其它河湖水体重金属含量相当;方差分析结果表明,除了Cd元素,河湖水体重金属含量呈现显著空间差异（P＜0.05）,而季节上无显著差异（P＞0.05）;单因子及内梅罗污染指数结果表明,所有样品的单因子及内梅罗污染指数值均低于0.7,表明所采样品均没有受到Cd、Cr、Cu、Pb和Zn元素的影响;相关性分析结果表明,元素Cd、Cr、Cu和Pb之间存在显著的正相关（P＜0.01）,但Zn与其它元素之间均没有显著的正或负相关关系（P＞0.05）,表明Cd、Cr、Cu和Pb元素的来源较为一致,而Zn元素来源则较为复杂,可能主要受人为活动的频繁影响。（2）沉积物重金属分布特征及生态风险评价结果表明:在冬夏两季,沉积物Cd浓度介于0.84-3.10和0.43-3.31 mg/kg中间,Cu浓度介于24.85-117.3 mg/kg和28.64-128.07 mg/kg之间,Cr浓度介于60.86-128.85 mg/kg和34.38-95.29 mg/kg之间,Pb浓度介于11.77-54.72 mg/kg和12.28-52.13 mg/kg之间,Zn浓度介于29.64-218.30 mg/kg和61.62-1310.78 mg/kg之间,重金属浓度的平均值递减规律为:Cr?Zn?Cu?Pb?Cd（冬季）,Zn?Cu?Cr?Pb?Cd（夏季）。此外,富集因子及地累积指数的计算结果表明,相比于其他四种元素,Cd在表层沉积物中的富集及累积最大。单项生态风险评价结果表明,Cd元素在不同的采样点分别表现为适中、相当高及非常高的生态风险,而其他元素（Cu、Cr、Pb、Zn）对各采样点的生态风险程度较低。此外,各元素对潜在生态风险的贡献率表现为Cd?Pb?Cu???Cr?Zn（冬季）和Cd?Cu?Pb?Cr?Zn（夏季）,表明Cd元素是导致研究区域生态风险值高的主要元素,其次为Pb和Cu元素。多元统计分析结果表明,Cu、Pb和Zn这三种元素的地球化学特征相似,其来源可能较为一致,而Cd和Cr来源较为复杂,可能受自然来源或人类活动的综合影响。

Emerging contaminants have commonly been observed in environmental waters but have not been included in water quality assessments at many locations around the *** evaluate the availability of reclaimed water in Kinmen,Taiwan,this study provides the first survey of the distribution of thirty-three pharmaceuticals and five perfluorinated chemicals in lake waters and water from local wastewater treatment plants(WWTPs).The results showed that the target emerging contaminants in Kinmen lakes were at trace ng/l *** addition,most of the target compounds were present in the Jincheng and Taihu WWTP influents at ng/l concentrations levels,of which 5 compounds(erythromycin-H2O(1340 ng/l),ibuprofen(1763 ng/l),atenolol(1634 ng/l),acetaminophen(2143 ng/l),and caffeine(3113 ng/l))reached mg/l *** overall treatment efficiencies of the Jincheng and Taihu WWTPs with respect to these pharmaceuticals and perfluorinated chemicals were poor;half of the compounds were less than 50%*** compounds(sulfamethoxazole,erythromycin-H2O,clarithromycin,ciprofloxacin and ofloxacin)with risk quotient(RQ)values>1 in the effluent should be further investigated to understand their effects on the aquatic *** and advanced treatment units are found necessary to provide high-quality recycled water and sustainable water resources.
