Recent Epidemic-Like Anthrax Outbreaks in Italy: What Are the Probable Causes?

In Italy, in an area of about 50 km2 between basilicata and Campania Southern regions, 28 outbreaks of anthrax occurred from August 28th to September 27th 2011. Different species were affected: laboratory tests confirmed anthrax in cattle, horses and sheep. The genetic analysis of strains isolated from infected animals indicated that outbreaks occurring in the two regions were not correlated. Intriguingly, the incidence was highly significant in horses compared to that of cattle and sheep or goats, which were prevalent species in the animal population. In addition, allinfected horses and many cattle developed a fatal, sub-acute form of anthrax, characterized by the presence of massive edema, usually absent in hyperacute forms. The characteristics of these outbreaks suggested a possible role of tabanids as vectors in the transmission of b. anthracis like-epidemic infection.

Predictability and Management of OARs Toxicity in Patients with Prostate Cancer Treated with High-Dose Radiotherapy

Aim: To evaluate the predictability of toxicity analyzing the dose-volume histograms (DVHs) and to verify the effectiveness of preventive measures limiting side toxicity considering the evolution of the radiation techniques for prostate cancer treatment. Materials and Methods: 208 patients with localized prostate cancer were treated with exclusive radiotherapy until 73.8 Gy (group A) or 79.2 Gy (group b) with the dose escalation technique. Preventive measures to minimize the side effects were recommended in group b. Results: The assessment of genitourinary toxicity was similar while gastrointestinal toxicity was better in group b. Valuating the treatment plans, we found that most of the patients developing toxicity had “borderline” DVHs. Conclusion: Our analysis led to the establishment of a protocol for the management of patients with “border-line” DVH.

Impact of tertiary Gleason pattern 5 on prostate cancer aggressiveness:Lessons from a contemporary single institution radical prostatectomy series

Objective:To better evaluate tertiary Gleason pattern reporting and to evaluate the impact of tertiary Gleason pattern 5(TP5)on prostate cancer pathological features and biochemical recurrence at our large single ***:We retrospectively reviewed 1962 patients who underwent radical prostatectomy(RP)for prostate cancer;TP5 was reported in 159 cases(8.1%).Men with Gleason score(GS)7 and GS 8 disease were divided into subgroups with and without TP5,and histopathological features were *** analyses were conducted to assess the impact on TP5 on biochemical-free survival(bFS).Results:Tumors possessing GS 3+4 with TP5 were more likely to exhibit extraprostatic extension(EPE)and had a larger tumor diameter(TD)than GS 3+4 *** 3+4 with TP5 was also associated with positive surgical margins(SM),seminal vesicle involvement(SVI),and higher pre-operative prostate-specific antigen(PSA)values,but without statistical *** 4+3 with TP5 more commonly presented with EPE,positive SM,SVI,and greater TD and pre-operative PSA level than GS 4+3 *** multivariate analysis,Gleason score,EPE,and TP5 were overall independent risk factors for PSA recurrence in this ***,GS 4+3 with TP5 was associated with shorter time to recurrence versus GS 4+3 ***:Our results emphasize the importance of TP5 and suggest that criteria for tertiary pattern reporting in prostate cancer should be *** studies are needed to evaluate the role of tertiary patterns in prognostic models.

Long Lasting Effects of breastfeeding on Metabolism in Women with Prior Gestational Diabetes

background & Aims: breastfeeding improves glucose tolerance in the early postpartum period of women with prior gestational diabetes GDM, but it is unclear whether future risk of metabolic alterations, like type 2 diabetes, is reduced. The aim of this study was to investigate the effect of lactation, three years after pregnancy, on glucose and lipid metabolism in women with prior gestational diabetes. Materials & Methods: A population of women with prior gestational diabetes (Carpenter and Coustan Criteria) was evaluated with comparison of results for “lactating” [bF] versus “nonlactating women” [non bF]. breast feeding was defined [bF] if lasting? 4 weeks. In each woman a 75-g oral glucose tolerance test (OGTT) was performed to analyze the glucose tolerance, insulin sensitivity/resistance and b-cell function. Fasting serum was used to study their lipid profile (total cholesterol, high-density lipoprotein [HDL] cholesterol, low-density lipoprotein [LDL] cholesterol, and triglycerides), apolipoprotein b, apolipoprotein A1, homocysteine, fibrinogen, hsCRP, uric acid, microalbuminuria. Statistics: Paired and Un-paired t-test, Mann-Whitney and?χ2?tests were used, as appropriate. Results: A total of 81 women were evaluated (62 [bF] and 19 [non bF]). Maternal age (37.1?±?4.6 vs 37.4?±?4.9 years), body mass index (26.3?±?5.6 vs 26.4?±?5.3 kg/m2), parity (1.9?±?0.8 vs 1.7?±?0.8) and length of follow-up (32.2?±?20.2 vs 32.1±?20,0) were not different between the two groups. No effect was visible on glucose tolerance, HOMA-IR and other b-cell function indexes as well as hs-CRP (not significantly lower in non bF), uric acid, total cholesterol, HDL and LDL cholesterol. Levels of significance were only reached for “HOMA-IS” [bF] 1.0?±?0.7 vs [non bF] 0.6?±?0.4, p = 0.04) and triglycerides [bF] 83.8?±46.7 vs [non bF] 123.2?±?94.0 mg/dl, p = 0.02). Aspergillus parasiticus, a common fungal contaminant in food, produces aflatoxin b1, which is classified as human carcinogen. Kefir is an ancient fermented beverage obtained by the fermentation of different substrates with kefir grains. A very important waste produced by the dairy cheese industry is the whey permeate, which nowadays is a strong ambient contaminant. The aim of this work was to assess the effect of whey permeates fermented with kefir grains against A. parasiticus growth, aflatoxin b1 biosynthesis, and the kefir microorganisms protection against the cell damage produced by aflatoxin b1. It was observed that kefir-cell-free-supernatants (CFS) produced fungal inhibition. A fungicidal effect was observed with 65% v/v of CFS in the culture medium (final pH 4.55 and total undissociated lactic and acetic acid concentration 34.08 mM). Under these conditions, aflatoxin production was not detected. Finally, it was found that non-viable kefir microorganisms protected HepG2 cells from the damage produced by aflatoxin b1.

Isolation of <i>bacillus cereus</i>in a Facility Preparing School Meals

Food safety is a fundamental requirement in mass catering, as large numbers of meals are served each day to potentially vulnerable consumers, such as children. Food business Operators implement plans for the microbiological monitoring of the meals prepared and served in the catering sector, and for the swab-sampling of surfaces. From January 2018 to June 2019, our laboratory analyzed both food and swab samples from four catering facilities. Considering the EFSA 2018 data, we specifically focused on samples analyzed for bacillus cereus. Our data substantially showed episodic contamination due to a piece of equipment that is not usually subjected to microbiological control, thus suggesting that every aspect should be scrutinized in order to identify critical points. While bacillus cereus is widespread in nature and common in soil, it is adapted for growth in the intestinal tracts of insects and mammals. It is often present in a variety of foods, and may cause an emetic or a diarrheal type of food-associated illness. b. cereus produces several toxins. Multiplex PCR enables seven toxin genes to be detected (hblC, hblD, hblA, nheA, nheb, nheC and cytK).

High circulating D-dimers are associated with ascites and hepatocellular carcinoma in liver cirrhosis

AIM: To measure plasma D-dimer levels in cirrhotic patients with and without ascites, assessing the effect of ascites resolution in D-dimer concentration. METHODS: Seventy consecutive cirrhotic patients (M = 44, F = 26, mean age 65 years, SD ± 13), observed from October 2005 to March 2006 were enrolled. Circulating D-dimer levels were measured using a latex-enhanced, immunoturbidimetric test. In patients with ascites (n = 42) the test was repeated after ascites resolution. RESULTS: Ascites was present in 42 patients (group A) and absent in 28 (group b). Group A patients had more advanced liver disease. Hepatocellular carcinoma (HCC) was diagnosed in 14 patients and was more frequent in group b. Above normal range D-dimers were found in 45/70 patients. High D-dimers were more frequent in group A than in group b (P = 0.001). High D-dimers were associated with presence of HCC (P = 0.048) only in group b. After ascites resolution, obtained in all patients, mean D-dimer values decreased in those 34 patients with high basal levels (P = 0.007), returning to normal in 17. CONCLUSION: In patients with liver cirrhosis, ascites and HCC are the main factors associated with increased fibrinolytic activity.
