Proton-pump inhibitor use does not affect semen luality in subfertile men

Proton-pump inhibitors （PPIs） are among the most widely used drugs worldwide. PPI use has recently been linked to adverse changes in semen quality in healthy men; however, the effects of PPI use on semen parameters remain largely unknown specifically in cases with male factor infertility. We examined whether PPI use was associated with detrimental effects on semen parameters in a large population of subfertile men. We retrospectively reviewed data from 12 257 subfertile men who had visited our fertility clinic from 2003 to 2013. Patients who reported using any PPIs for 〉3 months before semen sample collection were included; 7698 subfertile men taking no medication served as controls. Data were gathered on patient age, medication use, and conventional semen parameters; patients taking any known spermatotoxic medication were excluded. Linear mixed-effect regression models were used to test the effect of PPI use on semen parameters adjusting for age. A total of 248 patients （258 samples） used PPIs for at least 3 months before semen collection. In regression models, PPI use （either as the only medication or when used in combination with other nonspermatotoxic medications） was not associated with statistically significant changes in semen parameters. To our knowledge, this is the largest study to compare PPI use with semen parameters in subfertile men. Using PPIs was not associated with detrimental effects on semen quality in this retrospective study.

Thermal nanoimprinting is a fast and versatile method for transferring the anti-reflective properties of subwavelength nanostructures onto the surface of highly reflective substrates, such as chalcogenide glass optical fiber end faces. In this paper, the technique is explored experimentally on a range of different types of commercial and custom-drawn optical fibers to evaluate the influence of geometric design, core/cladding material, and thermo-mechanical properties. Up to32.4% increased transmission and 88.3% total transmission are demonstrated in the 2–4.3 μm band using a mid-infrared(IR) supercontinuum laser.

Purpose:This study was aimed to analyze the mediation role of cardiorespiratory fitness(CRF)on the association between fatness and cardiometabolic risk scores(CMRs)in European ***:A cross-sectional study was conducted in adolescents(n=525;46%boys;14.1±1.1 years old,mean ±SD)from 10 European cities involved in the Healthy Lifestyle in Europe by Nutrition in Adolescence *** was measured by means of the shuttle run test,while fatness measures included body mass index(bMI),waist to height ratio,and fat mass index estimated from skinfold thicknesses.A clustered CMRs was computed by summing the standardized values of homeostasis model assessment,systolic blood pressure,triglycerides,total cholesterol/highdensity lipoprotein cholesterol ratio,and ***:Linear regression models indicated that CRF acted as an important and partial mediator in the association between fatness and CMRs in 1217-year-old adolescents(for bMI:coefficients of the indirect role b=0.058(95%confidence interval(95%CI):0.023-0.101),Sobel test z=3.11(10.0%mediation);for waist to height ratio:b=4.279(95%CI:2.242-7.059),z=3.86(11.5%mediation);and for fat mass index:b=0.060(95%CI:0.020-0.106),z=2.85(9.4%mediation);all pbetween fatness and CMRs could be partially decreased with improvements to fitness levels;therefore,CRF contribution both in the clinical field and public health could be important to consider and promote in adolescents independently of their fatness levels.

Redefining the properties of an osmotic agent in an intestinal-specific preservation solution

AIM: To investigate the effects of dextrans of various molecular weights (Mw) during a 12 h cold storage time-course on energetics, histology and mucosal infiltration of fluorescein isothiocyanate (FITC)-dextran. METHODS: Rodent intestines were isolated and received a standard University of Wisconsin vascular flush followed by intraluminal administration of a nutrientrich preservation solution containing dextrans of varying Mw: Group D1, 73 kdal; Group D2, 276 kdal; Group D3, 534 kdal; Group D4, 1185 kdal; Group D5, 2400 kdal. RESULTS: Using FITC-labeled dextrans, fluorescent micrographs demonstrated varying degrees of mucosal infiltration; lower Mw (groups D1-D3: 73-534 kdal) dextrans penetrated the mucosa as early as 2 h, whereas the largest dextran (D5: 2400 kdal) remained captive within the lumen and exhibited no permeability even after 12 h. After 12 h, median injury grades ranged from 6.5 to 7.5 in groups D1-D4 (73-1185 kdal) representing injury of the regenerative cryptal regions and submucosa; this was in contrast to group D5 (2400 kdal) which exhibited villus denudation (with intact crypts) corresponding to a median injury grade of 4 (P between Mw and adenosine triphosphate (r 2 = 0.809), total adenylates (r 2 = 0.865) and energy charge (r 2 = 0.667).CONCLUSION: Our data indicate that dextrans of Mw > 2400 kdal act as true impermeant agents during 12 h ischemic storage when incorporated into an intraluminal preservation solution.

Current trends in management of hepatitis b virus reactivation in the biologic therapy era

Hepatitis b virus (HbV) reactivation represents an emerging cause of liver disease in patients undergoing treatment with biologic agents. In particular, the risk ofHbV reactivation is heightened by the use monoclonalantibodies, such as rituximab (anti-CD20) and alemtuzumab (anti-CD52) that cause profound and longlasting immunosuppression. Emerging data indicatethat HbV reactivation could also develop following theuse of other biologic agents, such as tumor necrosis factor (TNF)-α inhibitors. When HbV reactivation is di-agnosed, it is mandatory to suspend biologic treatmentand start antiviral agents immediately. However, preemptive antiviral therapy prior to monoclonal antibodyadministration is crucial in preventing HbV reactivationand its clinical consequences. Several lines of evidencehave shown that risk of HbV reactivation is greatlyreduced by the identifi cation of high-risk patients andthe use of prophylactic antiviral therapy. In this article, we discuss current trends in the management of HbV reactivation in immunosuppressed patients receiving biologic therapy, such as rituximab, alemtuzumab and TNF-α antagonists.

Effect of the Number of Donor Atoms in Calix[4]arene Derivatives for Selective Complexation and Extraction of Mercury(II) Cations, Thermodynamics and Extraction Studies

Water pollution due to gold mining activity is increasingly worrying. One way to selectively mitigate it is through the use of complexing agents such as calixarenes, which selectively functionalized with soft donor atoms such as sulphur, can selectively remove this pollutant, for the selective complexation of mercury from contaminated water, which was compared by conductimetry and liquid-liquid extraction studies with results that show that the derivative with the highest sulphur atoms is its structure which presents greater selectivity compared to the derivative with the lowest number of sulphur atoms, in both cases, they present very stable complexes.

Like mother, like daughter： heritability of female Richardson＇s ground squirrel Urocitellus richardsonii cortisol stress responses

Activation of the hypothalamic-pituitary-adrenal （HPA） axis liberates glucocorticoids, which provides an acute indication of an individual＇s response to stressors. The heritability of the stress response in wild mammals, however, remains poorly documented. We quantified the cortisol stress response of female Richardson＇s ground squirrels （RGSs） to handling and physical restraint, testing for：（1） the effects of individual age, time of day, and sample latency; （2） repeatability within individuals; （3） narrow-sense heritability; and （4） differences among individuals owing to potential genetic and/or environmental effects. We detected a positive linear relationship between baseline plasma cortisol （bL-cortisol） concentration and stress-induced plasma cortisol （Sl-cortisol） concentration that defined each individual＇s cortisol stress response. bL-cortisol, SI-cortisol, and stress response did not differ according to the time the sample was taken, or by subject age. Cortisol stress response was highly repeatable within individuals, had a mother-offspring heritability of h2 =0.40 ± 0.24 （mean ±SE）, full-sibling heritability of h2s = 0.37 ± 0.71, and half-sibling heritabil- ity of hHS^2 = 0.75 ± 1.41. Stress responses of sibling groups, immediate-family groups, and squir- rels within a given area did not differ, whereas those of individuals from more distantly related matrilines did. Our results highlight the natural variability in HPA axis reactivity among individuals by quantifying both bL- and SI-cortisol levels, demonstrate partial heritability of the stress response that is not attributable to environmental variation, and suggest that at least part of an indi- vidual＇s stress response can be accounted for by differences in matrilineal history.

Hepatitis b vaccine by intradermal route in non responder patients:An update

Vaccination is the main prophylactic measure to reduce the mortality caused by hepatitis b virus (HbV) infection in healthy subjects since the immune response to hepatitis b recombinant vaccination occurs in over 90% of general population. Individuals who develop an anti-Hbs titer less than 10 mIU/mL after primary vaccination cycle are defined “no responders”. Many factors could cause a non response to the HbV vaccination, such as administration of the vaccine in buttocks, impaired vaccine storage conditions, drug abuse, smoking, infections and obesity. Moreover there are some diseases, like chronic kidney disease, human immunodeficiency virus infection, chronic liver disease, celiac disease, thalassaemia, type I diabetes mellitus, down’s syndrome and other forms of mental retardation that are characterized by a poorer response to HbV vaccination than healthy subjects. To date it is still unclear how to treat this group of patients at high risk of hepatitis b infection. Recent studies seem to indicate that the administration of HbV recombinant vaccine by the intradermal route is very effective and could represent a more useful strategy than intramuscular route. This review focuses on the use of anti hepatitis b vaccine by intradermal route as alternative to conventional intramuscular vaccine in all non responder patients. A comprehensive review of the literature using PubMed database, with appropriate terms, was undertaken for articles in English published since 1983. The literature search was undertaken in September 2013.
