Modulation of immunity and inflammatory gene expression in the gut, in inflammatory diseases of the gut and in the liver by probiotics

The potential for the positive manipulation of the gut microbiome through the introduction of beneficial microbes, as also known as probiotics, is currently an active area of investigation. The FAO/WHO define probiotics as live microorganisms that confer a health benefit to the host when administered in adequate amounts. However, dead bacteria and bacterial molecular components may also exhibit probiotic properties. The results of clinical studies have demonstrated the clinical potential of probiotics in many pathologies, such as allergic diseases, diarrhea, inflammatory bowel disease and viral infection. Several mechanisms have been proposed to explain the beneficial effects of probiotics, most of which involve gene expression regulation in specific tissues, particularly the intestine and liver. Therefore, the modulation of gene expression mediated by probiotics is an important issue that warrants further investigation. In the present paper, we performed a systematic review of the probiotic-mediated modulation of gene expression that is associated with the immune system and inflammation. Between January 1990 to February 2014, PubMed was searched for articles that were published in English using the MeSH terms “probiotics' and 'gene expression' combined with “intestines', 'liver', 'enterocytes', 'antigen-presenting cells', 'dendritic cells', 'immune system', and 'inflammation'. Two hundred and five original articles matching these criteria were initially selected, although only those articles that included specific gene expression results (77) were later considered for this review and separated into three major topics: the regulation of immunity and inflammatory gene expression in the gut, in inflammatory diseases of the gut and in the liver. Particular strains of Bifidobacteria, Lactobacilli, Escherichia coli, Propionibacterium, Bacillus and Saccharomyces influence the gene expression of mucins, Toll-like receptors, caspases, nuclear factor-κ

Poly-contextural Logic： New Resource for Transdisciplinary Research in Sociology

A couple of decades ago natural phenomena began to be approached from a comprehensive and transdisciplinary point of view, as it was understood that living beings and their environments are not linear but complex. There is no doubt that this perspective of visualizing complexity and working inter- and trans-disciplinarily has to be applied. The reflection on the theoretical observation （i.e., meta-observation） involved in the concept of poly-contexturality is the framework in which a theory of complex systems is possible, which in turn enables an observation that oscillates （a concept of chaos theory） between models structured in a hierarchical order （normally linked to a logical-deductive formalization） and models structured in hetero-hierarchy. And this would allow this reflection to be done in a formalized language that does not follow either the principles of the Aristotelian logic or the postulates of the Kantian transcendental reflection. It is precisely this liberation from the dictates of mono-contextural logic what paves the way to an observation of complexity, in which one or the other language is used to model the states of things, such as the epistemological problems of molecular biology or the social systems. And-what is gaining relevance-it also paves the way to a true transdisciplinary meta-observation, since each discipline chooses its own contexture and only the use of poly-contexturality makes it possible to formulate transdisciplinary relationships within the framework of such meta-logic.

Prevalence of meeting 24-Hour Movement Guidelines from pre-school to adolescence: A systematic review and meta-analysis including 387,437 participants and 23 countries

Malignant colorectal polyps

Nowadays, the number of cases in which malignant colorectal polyps are removed is increasing due to colorectal cancer screening programmes. Cancerous polyps are classified into non-invasive high grade neoplasia (NHGN), when the cancer has not reached the muscularis mucosa, and malignant polyps, classed as T1, when they have invaded the submucosa. NHGN is considered cured with polypectomy, while the prognosis for malignant polyps depends on various morphological and histological factors. The prognostic factors include, sessile or pedunculated morphology of the polyp, whether partial or en bloc resection is carried out, the degree of differentiation of the carcinoma, vascular or lymphatic involvement, and whether the polypectomy resection margin is tumor free. A malignant polyp at T1 is considered cured with polypectomy ifit is a pedunculated polyp (Ip of the Paris classification), it has been completely resected, it is not poorly differentiated, the resection edge is not affected by the tumor and there is no vascular or lymphatic involvement. The sessile malignant polyp (Is of the Paris classification) at T1 is considered not cured with polypectomy. Only in some cases (e.g. older people with high surgical risk) local excision(polypectomy or endoscopic submucosal dissection or conventional endoscopic mucosal resection) is considered the definitive treatment.

背景社区获得性肺炎(CAP)风险在初级保健中的信息是有限的。我们评估了不同生活方式和共病条件作为初级保健成人CAP的危险因素。方法一项回顾性观察对照研究。使用国家初级保健数据监测系统检索了2009至2013年在西班牙初级保健部门诊断的成人CAP病例。匹配年龄和匹配性别的对照组采用发病率密度抽样(比例2∶1)。以百分比和比值比来表示关联分析。为了避免偏倚效应,建立二项回归模型。结果51 139例患者和102 372例对照组做比较。平均年龄为61.4(19.9)岁。危险因素与CAP的显著相关性如下:艾滋病[比值比(95%可信区间)5.21(4.35~6.27)]、慢性阻塞性肺病(COPD)[2.97(2.84~3.12)]、哮喘[2.16(2.07~2.26)]、吸烟[1.96(1.91~2.02)]和口腔卫生问题[1.45(1.41~1.49)]。患者组中有危险因素存在的平均患病率为82.2%,对照组为69.2%。CAP患病率的增加与危险因素和年龄的累积有关:1个危险因素时,在18~60岁老人中的风险相关性是1.42,在>60岁人中是1.57;2个危险因素时,是1.88和2.35;≥3个危险因素时,是3.11和4.34。结论成人CAP初级保健中的危险因素普遍较高。主要与危险因素相关的是艾滋病、COPD、哮喘、吸烟、口腔卫生问题。我们的风险叠加结果可以帮助临床医生确定肺炎风险较高的患者。

Giant Spinal Schwannomas. Presentation of Two Cases with Interdisciplinary Surgical Resection

Purpose: To report two quite rare cases of giant schwannomas of the cervical and thoracic spine. Surgical resection is usually the treatment of choice for these neoplasms. Methods: The first patient is a 48-year-old male that came to our department, complaining about weakness and paresthesia of both lower extremities and upper left extremity, and a considerably big dorsal palpable lump. MRI and CT scan were used to diagnose giant cervical schwannoma. Because of the intensity of symptoms, he was subjected to surgery. Multidisciplinary resection involving neurosurgical and oncosurgicalteamn was performed for intravertebral and extravertebral approaches. The second patient is a 55-year-old female that came to the hospital complaining about incapacitating pain and dysesthesia involving T6, T7, and T8 dermatomes, with no other deficits. MRI and CT scan were used to diagnose giant thoracic schwannoma. Because of the intensity of the pain, surgery was decided as the treatment of choice. Video assisted thoracoscopic surgery was performed and total resection was achieved, with improvement of the symptoms. Results: Both patients showed complete remission of the symptoms, with no major deficits after surgery. Conclusion: Although infrequent, giant spinal schwannomas may be the cause of visible lumps in the back with radicular symptoms. When asymptomatic, conservative treatment could be viable, but when they present symptoms, surgery offers good results.

AIM:To evaluate changes in colorectal cancer(CRC) survival over the last 20 ***:We compared two groups of consecutive CRC patients that were prospectively recruited:Group Ⅰincluded 1990 patients diagnosed between 1980 and ***Ⅱincluded 871 patients diagnosed in ***:The average follow up time was 21 mo(1-229)for GroupⅠand 50 mo(1-73.4)for GroupⅡ.Overall median survival was significantly longer in Group Ⅱthan in GroupⅠ(73 mo vs 25 mo,P<0.001)and the difference was significant for all tumor *** surgical mortality was 8% for GroupⅠand 2% for Group Ⅱ(P<0.001).Only 17% of GroupⅠpatients received chemotherapy compared with 50% of GroupⅡpatients(P<0.001).CONCLUSION:Survival in colorectal cancer patients has doubled over the past 20 *** increase seems to be partly due to the generalization in the administration of chemotherapy and to the decrease of post surgical mortality.

Effects of Trifoliate Interstocks on Growth and Yield of Mexican Lime Trees

This study was carried out to determine the effect of 21 Citrus interstocks and the control Citrus macrophylla rootstock budded with Mexican lime （Citrus aurantifolia （Chistm.） Swingle） on growth and yield. Three types of mandarin and 13 trifoliate types including ＂C-35＂ and ＂Benton＂ used as interstocks, developed vigorous trees similar or more productive than the control. ＂Morton＂ and ＂Benecke＂ citranges, ＂Pomeroy＂ trifoliate trifoliate orange and the ＂Severinia buxifolia＂ interstocks showed a moderated effect on tree growth. Flying dragon （FDT） and Hiryu trifoliate oranges performed as semi-dwarfing interstocks. These interstocks reduced the tree height, canopy volume and surface area covered by the foliage on 25%-35%, 20%-30% and 20%-30%, respectively, compared to the control. While most interstocks surpassed the assigned area of growth （24 m2） in the 7th year, ＂FDT＂ and ＂Hiryu＂ did that at the 10th year. These semi-dwarfing interstocks produced up to 80 kg/tree per year. Most treatments recorded the highest production from the 8th to 10th year, except FDT and Hiryu, which showed the highest yield （130 kg/tree） from the 5th to 7th year. This study provides useful results to face the huanglongbing （HLB） problem using moderate or semi-dwarfing interstocks for Mexican lime orchards in high density plantations.
