Approach to Pain in Endometriosis Patients: Systematic Literature Review

Background: Endometriosis is a gynecological, multifactorial condition that affects women’s life quality, especially due to the pain inherent to it. Pain is caused by physical and psychological aspects;therefore, several factors must be analyzed, aiming to treat the whole. Conventional, drug or surgical treatments are the most used;however, the search for alternative therapeutic methods is growing. Objective: To analyze the approach to pain in women with endometriosis, seeking a better understanding of the pathophysiological mechanisms of this disease, its impact on life quality, as well as the means of treatments in use and innovations in this field. Methods: Systematic literature review, conducted in the Virtual Health Library databases between 2010 and 2021, restricted to articles in English and Portuguese with the descriptors: Endometriosis, Pain, and Treatment. After reading the abstracts, 197 articles were found, and, after reading them, 59 articles were selected. Results: This review analyzed 59 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects: author/year of publication, article title, objective, type of study and database. Related factors such as acute or chronic pain, late diagnosis, discrediting symptomatic complaints and trivialization by health professionals have a profound impact on the physical and mental health of women with endometriosis. On the other hand, there are several classes of drugs, surgical procedures, and alternative therapies available for pain management. Conclusion: Given the context addressed, a certain need is suggested for health professionals to better understand the disease and adopt new behaviors or improve existing ones, such as reception, active listening, early diagnosis and comprehensive and individualized treatment. Therapeutic Approaches to Post-Mastectomy Pain Syndrome

Background: Breast cancer is increasingly common in society and post-mastectomy pain is a striking condition associated with surgery, both intra and postoperatively. The post-mastectomy syndrome is characterized by the persistence of pain for more than 3 months after the procedure. It is mostly of neuropathic origin and is highly refractory to treatments, which vary from clinical to surgical measures and alternative techniques. In this context, it is essential to understand the possible approaches to these patients. Objective: Understand alternatives for pain management in patients undergoing mastectomy, especially in those in whom pain persists for more than 3 months. Methods: Systematic literature review, conducted in the Virtual Health Library databases includes: Lilacs, SciELO, Medline, PubMed and Cochrane between 2018 and 2023, restricted to articles in English with the descriptors: Mastectomy, chronic Pain, Nerve Blocks and Breast Cancer. 317 articles were found involving the descriptors presented, all in English. After reading the abstracts, 28 articles were selected. Results: This review analyzed 28 scientific studies that rigorously met the previously established characteristics in the sample selection. The synthesis included the following aspects: author/year of publication, article title, objective, type of study and database. Conclusion: It was concluded that, according to the analyzed studies, post-mastectomy pain is a problem that has increased its incidence and that needs the attention of health professionals. The use of antineuropathic medications, nerve blocks, fat grafting, lymph node transplantation, therapy, physical activity and acupuncture are some examples of therapeutic approaches for these women. Therefore, the team must assist the patients, seeking to provide a better prognosis, quality of life and comfort.

Oxidation of lipids in various products,along with the growth of medically important pathogens,has led to a search for medicinal plants with antioxidant and antimicrobial *** a result,the aim of this study was to evaluate the antioxidant and antibacterial activity of the essential oil of Eucalyptus globulus(EO-Eg).Antioxidant activity was assessed by using the 2,2-diphenyl-1-picrylhydrazyl hydrate(DPPH)assay *** components were identified through gas chromatography-mass spectrometry(GC-MS)*** activity and minimum inhibitory concentration(MIC)were assessed by using the broth microdilution method with standard multidrug-resistant bacterial *** main EO-Eg compounds identified by GC-MS were isopulegol,citronellal,and citronellol,which are primarily used in the industrial ***-Eg demonstrated excellent antioxidant activity with an effective concentration(EC50)of 4.48μL/mL owing to the presence of phenolic *** antibacterial activity,the EO-Eg displayed a broad antimicrobial spectrum of antimicrobial activity across the different resistance phenotypes *** most notable antibacterial activity was observed against Staphylococcus aureus 169 MRSA(MIC=0.0625%).As a result,our findings suggest that EO-Eg has antioxidant and antibacterial potential against hospital-acquired multidrug-resistant pathogens,which may be correlated with its major components.

This study hypothesized that different land-use affect the microbial enzymatic stoichiometry and C-,N-,and P-acquisition in Brazilian semiarid ***,the enzymesβ-glucosidase(C-acquiring enzyme),urease(N-acquiring enzyme),and acid phosphatase(P-acquiring enzyme)were assessed in soil samples collected at 0−5 and 5−10 cm depth from a tropical dry forest,a protected area with Angico,a protected area with Ipê,scrub area,and an agricultural area with *** values of C-,N-,and P-acquiring enzymes were used to calculate the enzymatic C:N,C:P,and N:P *** values of C:P and N:P ratios were higher at 0−5 cm depth,while no significant variation,between soil depth,was observed for C:N *** values of C-and N-acquiring enzymes were higher at 0−5 cm in tropical dry forest areas and Angico forest,*** all land use types,the values of vectors L and A were higher than 1°and 45°,*** study showed that both land-use and soil depth influence the enzymatic stoichiometry,showing higher values of C-and N-acquiring enzymes in native and protected forests at soil surface.

Identification of Sugar,Amino Acids and Minerals from the Pollen of Jandaíra Stingless Bees(Melipona subnitida)

The aim of this investigation was to analyze two samples of pollen from jandaíra stingless bees (Melipona subnitida) in view of their mineral composition, free amino acids and sugars. Palynological analysis showed that the predominant pollen was monofloral from Senna sp. species (94.5%, pollen 2011) and the second pollen sample showed the presence of two primary species, Chamaecrista sp. (39.2% pollen 2009) and Mimosa tenuiflora (43.5%) (pollen 2009). The highest mineral content was potassium. The bee pollen contained 20.8% and 31.0% of mannitol in samples from 2011 and 2009, respectively. Proline and serine are the predominant amino acids. High content of essential amino acids, minerals and the sugar mannitol confirmed high nutritional value of pollen samples from jandaíra bees.

HPLC-DAD analysis and antifungal effect of Hyptis martiusii Benth(Lamiaceae) against Candida strains

Objective: To evaluate the anti Candida activity of Hyptis martiusii decoction and its major compound, caffeic acid alone or in the presence of fluconazole, as well as their cytotoxic effect. Methods: The decoction was characterized using high performance liquid chromatography coupled with diode array detector. For the antifungal activity, the minimum inhibitory concentration(MIC) and the potential effect of the decoction with the fluconazole were evaluated by microdilution method using 96-well microtiter trays. The osmotic fragility test was performed using erythrocytes under saline stress. All tests were performed in triplicate. Results: The chemical characterization of the decoction was performed by high performance liquid chromatography and revealed the presence of seven compounds, including caffeic acid as major constituent. The antifungal tests demonstrated that both decoction(DHm) and caffeic acid obtained from Hyptis martiusii presented MIC and MFC ≥4096 μg/mL against Candida albicans and Candida tropicalis strains. However, in the presence of fluconazole, DHm and caffeic acid presented IC_(50) of 2.60 and 2.53 μg/mL respectively, demonstrating significant synergistic effects against Candida strains. The modulator activity of DHm might be due to the presence of caffeic acid. Moreover, DHm and caffeic acid did not cause significant hemolytic effects, indicating that they present low cytotoxicity. Conclusions: These data indicate that DHm potentiates the activity of the fluconazole, without enhancement of the toxicity, encouraging further toxicological, pharmacological and phytochemical studies to provide consistent evidence of the potential of this plant to be used in drug development.

Feasibility and safety of autologous bone marrow mononuclear cell transplantation in patients with advanced chronic liver disease

AIM： To evaluate the safety and feasibility of bone marrow cell （BMC） transplantation in patients with chronic liver disease on the waiting list for liver transplantation. METHODS： Ten patients （eight males） with chronic liver disease were enrolled to receive infusion of autologous bone marrow-derived cells. Seven patients were classified as Child-Pugh B and three as Child-Pugh C. Baseline assessment included complete clinical and laboratory evaluation and abdominal MRI. Approximately 50 mL of bone marrow aspirate was prepared by centrifugation in a ficoll-hypaque gradient. At least of 100 millions of mononuclear-enriched BMCs were infused into the hepatic artery using the routine technique for arterial chemoembolization for liver tumors. Patients were followed up for adverse events up to 4 mo. RESULTS： The median age of the patients was 52 years （range 24-70 years）. All patients were discharged 48 h after BMC infusion. Two patients complained ofmild pain at the bone marrow needle puncture site. No other complications or specific side effects related to the procedure were observed. Bilirubin levels were lower at 1 （2.19 ± 0.9） and 4 mo （2.10 ± 1.0） after cell transplantation that baseline levels （238 ± 1.2）. Albumin levels 4 mo after BMC infusion （3.73 ± 0.5） were higher than baseline levels （3.47 ± 0.5）. International normalized ratio （INR） decreased from 1.48 （SD = 0.23） to 1.43 （SD = 0.23） one month after cell transplantation. CONCLUSION： BMC infusion into hepatic artery of patients with advanced chronic liver disease is safe and feasible. In addition, a decrease in mean serum bilirubin and INR levels and an increase in albumin levels are observed. Our data warrant further studies in order to evaluate the effect of BMC transplantation in patients with advanced chronic liver disease.

Physiological and Biochemical Characterization of JilóSeeds (Solanum gilo) in Different Harvest Times

The evaluation of seed quality during the development process is important for the definition of the moment of harvest. In this context, the objective in this study was to evaluate the quality of Jiló seeds during development through physiological, biochemical and electrophoretic tests of enzymes. Six genetic materials, JIL white, JIL 001, JIL 005, JIL 006, H1 and H2 were used. Fruits were harvested at 35, 40, 45 days after anthesis (DAA). A total of 50% of all fruits harvested at 45 DAA remained at rest for seven days which were labeled as 45 DAAr. The seeds at each maturation stage were extracted from the fruits and evaluated by water content, weight of one thousand seeds, electric conductivity, germination, germination speed index, seedling emergence, and seedling emergence rate index. The chemical composition of the seeds was also evaluated, as well as the expression of the enzymes esterase, isocitrate lyase, malate dehydrogenase, alcohol dehydrogenase, superoxide dismutase, catalase, peroxidase and endo-β-mannanase. The highest values of germination and vigor were found in seeds harvested at 45 DAA and 45 DAAr, mainly for hybrids H1 and H2. The highest values of lipids and proteins and lower carbohydrates were found in the most immature seeds of Jiló. The enzymes analyzed showed higher expression in the initial stages of development, however reduced with the advancement of the maturation stage. Endo-β-mannanase expression increased with seed development. Jiló seeds harvested at 45 DAAr presented the highest of germination and vigor values. This “being” considered as the ideal harvest point.
