AIM: To characterize the effects of age on the mechanisms underlying the common condition of esophageal dysphagia in older patients, using detailed manometric analysis. METHODS: A retrospective case-control audit was performed on 19 patients aged ≥ 80 years (mean age 85 ± 0.7 year) who underwent a manometric study for dysphagia (2004-2009). Data were compared with 19 younger dysphagic patients (32 ± 1.7 years). Detailed manometric analysis performed prospectively included basal lower esophageal sphincter pressure (BLESP), pre-swallow and nadir LESP, esophageal body pressures and peristaltic duration, during water swallows (5 mL) in right lateral (RL) and upright (UR) postures and with solids. Data are mean ± SE; a P-value < 0.05 was considered significant. RESULTS: Elderly dysphagic patients had higher BLESP than younger patients (23.4 ± 3.8 vs 14.9 ± 1.2 mmHg; P < 0.05). Pre-swallow LESP was elevated in the elderly in both postures (RL: 1 and 4 s P = 0.019 and P = 0.05; UR: P < 0.05 and P = 0.05) and solids (P < 0.01). In older patients, LES nadir pressure was higher with liquids (RL: 2.3 ± 0.6 mmHg vs 0.7 ± 0.6 mmHg, P < 0.05; UR: 3.5 ± 0.9 mmHg vs 1.6 ± 0.5 mmHg, P = 0.01) with shorter relaxation after solids (7.9 ± 1.5 s vs 9.7 ± 0.4 s, P = 0.05). No age-related differences were seen in esophageal body pressures or peristalsis duration. CONCLUSION: Basal LES pressure is elevated and swallow-induced relaxation impaired in elderly dysphagic patients. Its contribution to dysphagia and the effects of healthy ageing require further investigation.

Background:In 2011,Ghana piloted the integration of herbal medicine services into mainstream health care delivery in selected government hospitals across the *** date,however,no single certified herbal drug is generally covered by the national health insurance *** study evaluated the implications of out-of-pocket payment for prescriptions on the patronage of herbal medicine units in 3 selected government hospitals within Kumasi ***:A cross-sectional study on 413 participants was performed using a semi-structured ***:The majority of study respondents were female(54%),and the median age was 35 *** participants(83.1%)were aware that herbal medicine was integrated in the mainstream health care system of the *** the costs of certified herbal drugs,51.5%of respondents considered them very expensive;72.1%of the respondents believed that the costs of certified herbal drugs adversely affected the utilization of herbal units at government hospitals,which produced a positive correlation(r=0.5498).A total of 99.5%of the respondents recommended the inclusion of certified herbal drugs on the national health insurance drug ***:This study revealed that the costs of certified herbal medicines negatively affect utilization of herbal units at government *** improve the utilization of herbal units,certified herbal drugs dispensed at these units must thus be included in the national health insurance drug list.

髌股关节疼痛综合征(patellofemoral pain syn-drome,PFPS)通常由关节的承重异常所致。可调节阻力的膝关节矫正器能够在膝关节屈曲时提供阻力,逐渐发展为一种治疗髌股关节疼痛的有效手段。本文的研究目的是考察膝关节在具有主动阻力的矫正器(Protonics装置)保护下完成不同活动时腿部肌电活动的变化。我们假设在配戴膝关节矫正器后,股四头肌和腘肌的肌电活动会相应发生改变。18例PFPS患者在配戴与不配戴矫正器的两种情况分别下进行测试,10名对照组受试者在不配戴矫正器的情况下进行测试。测试活动包括:上下楼梯、水平地面行走、下蹲以及跨步。活动过程中的膝关节屈曲度由运动分析系统实时测量,3个股四头肌和2个腘肌的肌电活动信号由肌电仪(electromyogra-phy,EMG)同步记录。研究结果显示,在配戴膝关节矫正器后,股四头肌和腘肌的活动有明显变化。这些变化通常具有阶段性而非贯穿于整个运动范围。本研究表明在日常活动以及康复训练时配戴膝关节矫正器对于改善髌股关节疼痛综合征具有良好的作用。

AIM To define a ten-step protocol that reduced the incidence of surgical site infection in the spine surgery practice of the senior author and evaluate the support for each step based on current *** In response to unexplained increased infection rates at our institution following spine surgery,a ten-step protocol was implemented:(1) preoperative glycemic management based on hemoglobin A1 c(HbA1 c);(2) skin site preoperative preparation with 2% chlorhexidine gluconate disposable cloths;(3) limit operating room traffic;(4) cut the number of personnel in the room to the minimum required;(5) absolutely no flash sterilization of equipment;(6) double-gloving with frequent changing of outer gloves;(7) local application of vancomycin powder;(8) re-dosing antibiotic every 4 h for prolonged procedures and extending postoperative coverage to 72 h for high-risk patients;(9) irrigation of subcutaneous tissue with diluted povidone-iodine solution after deep fascial closure; and(10) use of Dura Prep skin preparation at the end of a case before skin closure. Through an extensive literature review,the current data available for each of the ten steps was *** Use of vancomycin powder in surgical wounds,routine irrigation of surgical site,and frequent changing of surgical gloves are strongly supported by the literature. Preoperative skin preparation with chlorhexidine wipes is similarly supported. The majority of current literature supports control of HbA1 c preoperatively to reduce risk of infection. Limiting the use of flash sterilization is supported,but has not been evaluated in spine-specific surgery.
