Aneuploidy in pluripotent stem cells and implications for cancerous transformation

Owing to a unique set of attributes, human pluripotent stem cells （hPSCs） have emerged as a promising cell source for regenerative medicine, disease modeling and drug discovery. Assurance of genetic stability over long term maintenance of hPSCs is pivotal in this endeavor, but hPSCs can adapt to life in culture by acquiring non-random genetic changes that render them more robust and easier to grow. In separate studies between 12.5% and 34% of hPSC lines were found to acquire chromosome abnormalities over time, with the incidence increasing with passage number. The predominant genetic changes found in hPSC lines involve changes in chromosome number and structure （particularly of chromosomes 1, 12, 17 and 20）, remi- niscent of the changes observed in cancer cells. In this review, we summarize current knowledge on the causes and consequences of aneuploidy in hPSCs and highlight the potential links with genetic changes observed in human cancers and early embryos. We point to the need for comprehensive characterization of mechanisms underpinning both the acquisition of chromosomal abnormalities and selection pressures, which allow mutations to persist in hPSC cultures. Elucidation of these mechanisms will help to design culture conditions that minimize the appearance of aneuploid hPSCs. Moreover, aneuploidy in hPSCs may provide a unique platform to analyse the driving for- ces behind the genome evolution that may eventually lead to cancerous transformation.

Towards real-time detection of wastewater in surface waters using fluorescence spectroscopy

The presence of municipal wastewater at the intake of a major drinking water treatment facility located on Lake Ontario was examined using fluorescence data collected during a period of continuous *** addition,controlled mixing of lake water and wastewater sampled from a local treatment facility were conducted using a bench-scale fluorescence system to quantify observed changes in natural organic *** linear regression was applied to components derived from parallel factors *** resulting mean absolute error for predicted wastewater level was 0.22%(V/V,wastewater/lake water),indicating that wastewater detection at below 1.0%(V/V)was *** of sucralose,a wastewater indicator,were conducted for treated wastewater as well as surface water collected at two intake locations on Lake *** suggested minimal wastewater contribution at the drinking water intake.A wastewater detection model using a moving baseline was developed and applied to continuous fluorescence data collected at one of the drinking water intakes,which agreed well with sucralose ***,the simulated addition of 1.0%(V/V)of wastewater/wastewater was detectable in 89%of samples analyzed,demonstrating the utility of fluorescence-based wastewater monitoring.

Fracture of allograft interbody spacer resulting in post-operative radiculopathy: A case report

BACKGROUND Allograft interbody spacers are utilized during transforaminal lumbar interbody fusion(TLIF) to reestablish anterior column support and disc height. While the TLIF technique offers many improvements over previous surgical methods,instrumentation and bone graft-related complications such as spacer misplacement or migration, screw fracture or misplacement, or rod breakage continue to be reported. The objective of this manuscript is to report on a fractured allograft interbody spacer that displaced into the neural foramen and resulted in impingement on the exiting nerve root that required *** SUMMARY A 50-year-old male had two-level TLIF with immediate post-operative right L5 radiculopathy. Computed tomography scan demonstrated a fractured allograft interbody spacer that displaced into the right neural foramen and impinged on the exiting L5 nerve root. Revision surgery was performed to remove the broken allograft fragments from the right L5 foramen and the intact portion of the spacer was left in place. The right leg L5 radicular pain resolved. At the last follow up 12 mo after the index procedure, computed tomography scan confirmed sound interbody and posterolateral *** Displacement of broken allograft interbody spacer following TLIF procedures can result in neurological sequelae that require revision. To avoid such an occurrence,the authors recommend allowing sufficient time for the reconstitution of the graft in saline prior to use to decrease brittleness, to use an impactor size that is as close as possible to the spacer size and meticulous inspection of the cortical allograft spacer for any visible imperfection prior to insertion.

Evaluation of the mechanisms of damage to flexible ureteroscopes and suggestions for ureteroscope preservation

Aim： To investigate the causes and costs of flexible ureteroscope damage, and to develop recommendations to limit damage. Methods： The authors analysed repair figures and possible causes of damage to 35 instruments sent for repair to a leading UK supplier over a 1-year period, and calculated cost figures for maintenance of the instruments as opposed to repair and replacement costs. Results： All damages were handling-induced and therefore did not fall under the manufacturer＇s warranty： 28 % were damaged by misfiring of the laser inside the instrument; 72 %, mainly crushing and stripping of the ureteroscope shaft tube, were likely to have occurred during out-of-surgery handling, washing and disinfection. Seventeen （4 %） instruments were not repaired and consequently taken out of service due to the extensive costs involved. Eighteen （51%） ureteroscopes were repaired at an average cost of 10 833 USD. Conclusion： Damages to flexible ureteroscopes bear considerable costs. Most damages occur during handling between surgical procedures. Thorough adherence to handling procedures, and courses for theater staff and surgeons on handling flexible instruments may help to reduce these damages and prove a cost-saving investment. The authors provide a list of recommended procedural measures that may help to prevent such damages.

Reduced normogastric electrical activity associated with emesis:A telemetric study in ferrets

AIM： To characterize the gastric myoelectric activity （GMA） and intra-abdominal pressure changes induced by emetic stimuli （apomorphine and cisplatin） in the ferret. METHODS： GMA and intra-abdominal pressure were recorded in conscious, unrestrained ferrets surgically implanted with radiotelemetry transmitters. Animals were challenged with apomorphine （0.25 mg/kg sc） and cisplatin （10 mg/kg ip）, and the emetic response was quantified via direct observation and intra-abdominal pressure recording for 1 and 4 h, respectively. The GMA was analyzed by spectral analysis; the parameters used to characterize the GMA were the dominant frequency （DF） and the repartition of spectral power in the bradygastric, normogastric and tachygastric frequency ranges. RESULTS： Retches were identified on the intraabdominal pressure trace as peaks 0.30 ± 1.01 s in duration and 59.57 ± 2.74 mmHg in amplitude, vomit peaks were longer （0.82 ± 0.06 s, P 〈 0.01） and reached a higher pressure （87.73 ± 8.12 mmHg, P 〈 0.001）. The number of retches and vomits quantified via direct observation [apomorphine： 65.5 ± 11.8 retches ± vomits （R＋V）, cisplatin： 202.6 ± 64.1 R＋V] and intra-abdominal pressure （apomorphine： 68.3± 13.7 R＋V, n = 8; cisplatin： 219.0 ± 69.2 R＋V, n = 8） were correlated （r = 0.97, P 〈 0.0001） and the timing of emesis was consistent between the 2 methods. Apomorphine induced a decrease in normogastria from 45.48% ± 4.35% to 36.70 ± 4.34% （n = 8, P 〈 0.05） but the DF of the slow waves was not changed [8.95 ± 0.25 counts/rain （cpm） vs 8.68 ± 0.35 cpm, n = 8, P 〉 0.05]. Cisplatin induced a decrease in normogastria from 55.83% ± 4.30% to 29.22% ± 5.16% and an increase in bradygastria from 14.28% ± 2.32% to 31.19% ± 8.33% （n = 8, P 〈 0.001） but the DF （9.14 ± 0.13 cpm） remained unchanged （P 〉 0.05）. The GMA changes induced by cisplatin preceded the emetic response as normogastria was reduced for 1 h before the onset of emesis （57.61% ± 5.66% to 39.91% ± 5.74%, n = 6, P 〈 0.05）. Peri-emesis

Outcomes and patients' perspectives of transition from paediatric to adult care in inflammatory bowel disease

AIM: To describe the disease and psychosocial outcomes of an inflammatory bowel disease (IBD) transition cohort and their ***: Patients with IBD, aged > 18 years, who had moved from paediatric to adult care within 10 years were identified through IBD databases at three tertiary hospitals. Participants were surveyed regarding demographic and disease specific data and their perspectives on the transition process. Survey response data were compared to contemporaneously recorded information in paediatric service case notes. Data were compared to a similar age cohort who had never received paediatric IBD care and therefore who had not undergone a transition ***: There were 81 returned surveys from 46 transition and 35 non-transition patients. No statistically significant differences were found in disease burden, disease outcomes or adult roles and responsibilities between cohorts. Despite a high prevalence of mood disturbance (35%), there was a very low usage (5%) of psychological services in both cohorts. In the transition cohort, knowledge of their transition plan was reported by only 25/46 patients and the majority (54%) felt they were not strongly prepared. A high rate (78%) of discussion about work/study plans was recorded prior to transition, but a near complete absence of discussion regarding sex (8%), and other adult issues was recorded. Both cohorts agreed that their preferred method of future transition practices (of the options offered) was a shared clinic appointment with all key ***: Transition did not appear to adversely affect disease or psychosocial outcomes. Current transition care processes could be optimised, with better psychosocial preparation and agreed transition plans.
