Additive manufacturing,particularly 3D printing,has revolutionized the manufacturing industry by allowing the production of complex and intricate parts at a lower cost and with greater ***,3D-printed parts frequently require post-processing or integration with other mach.ning technologies to achieve the desired surface finish,accuracy,and mech.nical ***-precision mach.ning(UPM)is a potential mach.ning technology that addresses these challenges by enabling high surface quality,accuracy,and repeatability in 3D-printed *** study provides an overview of the current state of UPM for 3D printing,including the current UPM and 3D printing stages,and the application of UPM to 3D *** the presentation of current stage perspectives,this study presents a detailed discussion of the benefits of combining UPM with 3D printing and the opportunities for leveraging UPM on 3D printing or supporting each *** particular,future opportunities focus on cutting tools manufactured via 3D printing for UPM,UPM of 3D-printed components for real-world applications,and post-mach.ning of 3D-printed ***,future prospects for integrating the two advanced manufacturing technologies into potential industries are *** study concludes that UPM is a promising technology for 3D-printed components,exhibiting the potential to improve the functionality and performance of 3D-printed products in various *** also discusses how UPM and 3D printing can complement each other.

Microcurvature landscapes induce neural stem cell polarity and enhance neural differentiation

Tissue curvature has long been recognized as an important anatomical parameter that affects intracellular behaviors,and there is emerging interest in applying cell-scale curvature as a designer property to drive cell fates for tissue engineering *** neural cells are known to undergo dramatic and terminal morphological ch.nges during development and curvature-limiting behaviors have been demonstrated in neurite outgrowth studies,there are still crucial gaps in understanding neural cell behaviors,particularly in the context of a three-dimensional(3D)curvature landscape similar to an actual tissue engineering *** this study,we fabricated two substrates of microcurvature(curvature-substrates)that present a smooth and repeating landscape with focuses of either a concave or a convex *** these curvature-substrates,we studied the properties of morphological differentiation in n2a neuroblastoma *** contrast to other studies where two-dimensional(2D)curvature was demonstrated to limit neurite outgrowth,we found that both the concave and convex substrates acted as continuous and uniform mech.nical protrusions that significantly enhanced neural polarity and differentiation with few morphological ch.nges in the main cell *** enhanced differentiation was manifested in various properties,including increased neurite length,increased nuclear displacement,and upregulation of various neural *** demonstrating how the micron-scale curvature landscape induces neuronal polarity,we provide further insights into the design of biomaterials utilizing the influence of surface curvature in neural tissue engineering.

APAchE system is better than Ranson system in the prediction of severity of acute pancreatitis

BACKGROUnD: It has been suggested that addition of obesity score to the APAchE-Ⅱ system can lead to more accurate prediction of severity of acute pancreatitis. However there is scanty information on the usefulness of the combined APAchE-O scoring system in Asian patients. This study aimed to compare the accuracy of Ranson, APAchE-Ⅱ and APAchE-O systems in assessing severity of acute pancreatitis in a local ch.nese population. METHODS: One hundred and one consecutive patients with acute pancreatitis were prospectively studied. Body mass index (BMI) was measured on admission. Ranson score, APAchE-Ⅱ and APAchE-O scores were recorded on admission and at 48 hours. By adopting the cut-off levels and definitions advocated in the Atlanta consensus for severe disease, the diagnostic accuracy of the three scoring systems was compared by the area under the curve (AUC) under the receiver operator characteristic curve. RESULTS: Of the 101 patients, 12 (11.9%) patients suffered from severe pancreatitis. Obesity was uncommon and only two patients (2.0%) had BMI >30. Eighty-two (81.2%) patients were normal weight (BMI≤25) whereas 17 (16.8%) were overweight ( BMI 25-30 ). Overweight or obesity (BMI >25) was not associated with severe pancreatitis (P= 0.40). The AUC for admission scores of Ranson, APAchE-Ⅱ, and APAchE-O systems was 0. 549, 0. 904 and 0. 904, respectively. The AUC for 48-hour scores of Ranson, APAchE-Ⅱ and APAchE-O systems was 0.808, 0.955 and 0.951, respectively. COnCLUSIOnS: The APAchE-Ⅱ scoring system is more accurate than the Ranson scoring system of the prediction of severity in acute pancreatitis. Addition of obesity score does not significantly improve the predictive accuracy of the APAchE-Ⅱ system in our local population with a low prevalence of obesity.

Efficacy and safety of tadalafil taken as needed for the treatment of erectile dysfunction in Asian men： results of an integrated analysis

We evaluated the efficacy and safety of as-needed tadalafil in a diverse clinical population （with varying patient demographics, disease severity, and comorbid medical conditions） of Asian men with erectile dysfunction （ED）. An integrated analysis of five double-blind, placebo-controlled trials （n = 1 046） was performed. Patients were randomly assigned to receive 10 mg tadalafil （n = 185）, 20 mg tadalafil （n = 510）, or placebo （n = 351）. Efficacy assessments included the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP） diary and Global Assessment Question （GAQ）. Patients receiving 10 mg or 20 mg tadalafil showed significant improvement from baseline-to-end point on the Erectile Function domain of the International Index of Erectile Function （IIEF-EF） domain score in all clinical sub-populations analyzed, compared with patients receiving placebo （P 〈 0.001）. The 10-mg and 20-mg tadalafil groups showed a mean success rate of 64.1% and 70.5% for sexual intercourse attempts （SEP3, successful intercourse）, respectively, compared with 33.4% in the placebo group （P 〈 0.001）, and 85.5% and 85.4% reported improved erections at end point GAQ, respectively, versus 43.5% in the placebo group （P 〈 0.001）. Tadalafil was well tolerated across all groups studied. Headache and back pain were the most frequently reported adverse events. Overall, tadalafil was effective and well tolerated across a diverse clinical spectrum of Asian men with ED.

Is it safe to perform operation for colorectal malignancy in ch.nese patients without DVT prophylaxis？ An 8-year experience from a regional hospital in Hong Kong

Background Colorectal surgery was regarded as one of the high risk surgery for post-operative deep vein thrombosis （DVT） and pulmonary embolism. This study aimed at investigating the incidence of venous thromboembolism （VTE） after colorectal surgery for *** Data were collected from the prospective database of colorectal malignancy from 2000 to 2008. A total of 1421 colorectal （open and laparoscopic） operations were performed for the colorectal malignancy without DVT *** Only seven patients （0.5%） developed symptomatic DVT and one of them had complication of pulmonary embolism without mortality. Open operation for colorectal malignancy was identified as possible risk factor of DVT,however, risk factors like operative time, low anterior resection, sex, age etc. were not *** Risk of venous thromboembolism after colorectal operation is low in ch.nese of our locality and it might be safe to perform colorectal operation for malignancy without DVT prophylaxis.

Epstein-Barr virus infection and persistence in nasopharyngeal epithelial cells

Epstein-Barr virus(EBV) infection is closely associated with undifferentiated nasopharyngeal carcinoma(nPC), strongly implicating a role for EBV in nPC pathogenesis; conversely, EBV infection is rarely detected in normal nasopharyngeal epithelial tissues. In general, EBV does not show a strong tropism for infecting human epithelial cells, and EBV infection in oropharyngeal epithelial cells is believed to be lytic in nature. To establish life-long infection in humans, EBV has evolved efficient strategies to infect B cells and hijack their cellular mach.nery for latent infection. Lytic EBV infection in oropharyngeal epithelial cells, though an infrequent event, is believed to be a major source of infectious EBV particles for salivary transmission. The biological events associated with nasopharyngeal epithelial cells are only beginning to be understood with the advancement of EBV infection methods and the availability of nasopharyngeal epithelial cell models for EBV infection studies. EBV infection in human epithelial cells is a highly inefficient process compared to that in B cells, which express the complement receptor type 2(CR2) to mediate EBV infection. Although receptor(s) on the epithelial cell surface for EBV infection remain(s) to be identified, EBV infection in epithelial cells could be achieved via the interaction of glycoproteins on the viral envelope with surface integrins on epithelial cells, which might trigger membrane fusion to internalize EBV in cells. normal nasopharyngeal epithelial cells are not permissive for latent EBV infection, and EBV infection in normal nasopharyngeal epithelial cells usually results in growth arrest. However, genetic alterations in premalignant nasopharyngeal epithelial cells, including p16 deletion and cyclin D1 overexpression, could override the growth inhibitory effect of EBV infection to support stable and latent EBV infection in nasopharyngeal epithelial cells. Fulminant ulcerative colitis associated with steroid-resistant minimal ch.nge disease and psoriasis: A case report

A 43-year-old ch.nese patient with a history of psoriasis developed fulminant ulcerative colitis after immunosuppressive therapy for steroid-resistant minimal ch.nge disease was stopped. Minimal ch.nge disease in association with inflammatory bowel disease is a rare condition. We here report a case showing an association between ulcerative colitis, minimal ch.nge disease, and psoriasis. The possible pathological link between 3 diseases is discussed.

Dear Editor, The current standard 12-core systematic transrectal ultrasound （TRUS）-guided biopsy in prostate cancer diagnosis has the disadvantages of overdetecting indolent tumors, while failing to identify clinically significant cancers in up to 35%.1 Advances in imaging techniques, notably the multiparametric magnetic resonance imaging （mpMRI）, allow clinically significant prostate cancers to be detected with more precise localization） Fusion of MRI and real-time TRUS enables targeted biopsy of suspicious lesions, overcoming human error, and inability of biopsy site tracking in cognitive fusion？ Multiple Western series have suggested a higher detection rate, higher specificity, and better risk stratification with fusion biopsy.
