Objective:To investigate levels of awareness of dengue among the inhabitants of Queensland(QLD),a dengue-prevalent state in the north east of ***:A computer-assisted telephone interviewing survey was conducted in mid2014.A total of 1 223 randomly selected respondents( 18 years) across QLD completed a structured questionnaire covering all aspects of ***:97.55% had heard of dengue and participated *** them,54.70% had travelled overseas(48.11% to dengue-risk countries) in the last five years.A total of94.47% said transmission is by mosquito *** addition,84.83% knew of current transmission of dengue in QLD,while 80.97% knew the focus is Far North and North ***,2.35% and 8.97% had experienced an infection in their life or that of their immediate family/partner,respectively.85.03% identified correctly at least one means of prevention.A total of 69.72% advised to use insect repellent,wear covered clothing and avoid visiting mosquito-prone areas while 20.93% advised fumigation and clearing water containers around *** was a significant difference(P 0.05).Conclusions:Although many people throughout QLD have heard of dengue,about 15%appear unaware of local transmission,its symptoms and of methods to reduce risk of infection.A lack of knowledge regarding prevention of mosquito breeding is evident in South East QLD,where dengue is not currently *** study suggests that future dengue awareness campaigns should target communities in both endemic and potentially endemic areas throughout Queensland.

AIM To clarify the effectiveness of scaffold-based therapy for osteochondral lesions of the talus(OLT). METHODS A systematic search of MEDLINE and EMBASE databases was performed during August 2016 and updated in January 2017. Included studies were evaluated with regard to the level of evidence(LOE) and quality of evidence(QOE) using the Modified Coleman Methodology Score. Variable reporting outcome data, clinical outcomes, and the percentage of patients who returned to sport at previous level were also evaluated. RESULTS Twenty-eight studies for a total of 897 ankles were included; 96% were either LOE Ⅲ or Ⅳ. Studies were designated as either of poor or fair quality. There were 30 treatment groups reporting six different scaffold repair techniques: 13 matrix-induced autologous chondrocyte transplantation(MACT), nine bone marrow derived cell transplantation(BMDCT), four autologous matrixinduced chondrogeneis(AMIC), and four studies of other techniques. The categories of general demographics(93%) and patient-reported outcome data(85%) were well reported. Study design(73%), imaging data(73%), clinical variables(49%), and patient history(30%) were also included. The weighted mean American Orthopaedic Foot and Ankle Society(AOFAS) score at final follow-up was: 86.7 in MACT, 88.2 in BMDCT, and 82.3 in AMIC. Eight studies reported that a weighted mean of 68.3% ofpatients returned to a previous level of sport activity. CONCLUSION Scaffold-based therapy for OLT may produce favorable clinical outcomes, but low LOE, poor QOE, and variability of the data have confounded the effectiveness of this treatment.

Palliative radiotherapy(pRT) is primarily employed for palliation of bone pain in patients with castrate-resistant prostate cancer(CRPC). However, evidence that p RT influences prostate-specific antigen response in patients with CRPC on systemic therapy is lacking. We describe three cases of CRPC progressing after treatment with docetaxel(n=2) and abiraterone(n=1), who responded unusually after p RT for bone pain with the development of a significant biochemical response and restoration of response to systemic therapy. The possibility of p RT influencing metastatic disease in CRPC has not been previously reported, and raises the possibility of radiation-induced modulation of anti-tumor immune response mechanisms that may play a role in the restoration of response to systemic treatment.

The genetic alterations associated with cell transformation are in large measure expressed in the metabolic phenotype as cancer cells proliferate and change their local environment,and prepare for ***,the fundamental biochemistry of cancer cells is generally the same as in the untransformed cells,but the cancer cells produce a local environment,the TME,that is hostile to the stromal cells,and compete for *** order to proliferate,cells need sufficient nutrients,either those that cannot be made by the cells themselves,or must be made from simpler ***,in solid tumors,the nutrient supply is often limiting given the potential for rapid proliferation,and the poor quality of the ***,cancer cells may employ a variety of strategies to obtain nutrients for survival,growth and *** much has been learned using established cell lines in standard culture conditions,it is becoming clear from in vivo metabolic studies that this can also be misleading,and which nutrients are used for energy production versus building blocks for synthesis of macromolecules can vary greatly from tumor to tumor,and even within the same *** we review the operation of metabolic networks,and how recent understanding of nutrient supply in the TME and utilization are being revealed using stable isotope tracers in vivo as well as in vitro.

Living with a congenital heart condition can require a daunting and uncertain patient journey during which the support of a specialist nursing service is vital. A descriptive, cross-sectional evaluation completed over two years, utilised a postal questionnaire to investigate patient’s satisfaction with aspects of service. Qualitative responses to open questions on satisfaction with emotional support (n = 103) and contributions to well-being (n = 90) were analysed using a framework approach which identified eight themes. Professional knowledge and expertise, service accessibility, caring attributes, enabling patients to cope with anxiety, depression and meeting changing needs contributed positively to both patient well-being and emotional support. Family support was influential on emotional support alone, whilst mediating medical liaison exerted an impact on well-being by enhancing feelings of security. In conclusion, the specialist nursing service contributed positively to well-being and emotional support of patients and thereby to selective aspects of continuity of care.
