Prognostic model of survival outcomes in non-small cell lung cancer patients initiated on afatinib: pooled analysis of clinical trial data

Objective: Several predictors of survival have been identified in EGFR-positive non-small cell lung cancer(NSCLC) patients treated with first generation EGFR inhibitors. Prognostic models of survival outcomes with afatinib have not been evaluated.Methods: A prognostic tool for overall survival(OS)/progression free survival(PFS) based on pre-treatment clinicopathological factors was developed for EGFR-positive advanced NSCLC patients treated with first-line afatinib using penalised regression of individual-participant data from LUX-Lung 3 and 6(n = 468). Favourable, intermediate and poor risk groups were identified and externally validated using LUX-Lung 1(n = 390) and LUX-Lung 2(n = 129) trials that initiated afatinib following previous chemotherapy or EGFR inhibitor treatment.Results: Discriminative performance was good in the development and validation cohorts. For patients treated with first-line afatinib, the median OS for the favourable, intermediate and poor risk groups were > 47.7, 29.3 and 16.4 months, respectively, and the median PFS were 17.3, 13.2 and 8.3 months, respectively. The improvement in median OS with afatinib use compared to chemotherapy was > 12.4 months for the favourable risk group, whereas no OS benefit was apparent for the poor risk group. The improvement in median PFS with afatinib use compared to chemotherapy was 10.2 months for the favourable risk group and 3.2 months for the poor risk group.Conclusions: A prognostic tool was developed and validated to identify favourable, intermediate and poor risk groups for OS/PFS in EGFR-positive advanced NSCLC patients treated with afatinib. The prognostic groups can inform the likely absolute OS/PFS benefit expected from afatinib compared to chemotherapy in first-line treatment.

Audible noise evaluation for six-phase overhead lines transformed from existing three-phase double circuit infrastructures with uprated voltages

The drive to reduce carbon emissions and meet climate change targets set by governments worldwide has led to the growth of renewable energy sources and electric vehicles in recent years.Resulting intermittent generation and centralised demand require increased transmission capacities when operating associated power systems.Existing transmission lines are not capable of meeting planned needs.In many countries,obtaining a new right of way to build an overhead line(OHL)is increasingly difficult.There is therefore an urgent need to study techniques for transmission line uprating.Simply increasing voltages is not feasible because elevated surface potential gradient at conductor surfaces increases audible noise(AN)levels unacceptably.The feasibility of high phase order,especially six-phase,transmission techniques has been previously demonstrated,and it has been proved to be an effective way to uprate existing three-phase double circuit OHLs with increased voltages.This study analyses two typical overhead line structures,whose conductor surface potential gradient and AN level are calculated.The average and maximum surface potential gradient of the six-phase OHL are 18.5%and 18.6%lower than that of the three-phase ones.The AN of the six-phase single circuit OHL is predicted to be 6.33 and 6.45 dBA lower than that of the three-phase double circuit OHL after the voltage is increased by 45%and 50%for the lines considered.It is shown that six-phase OHLs can increase capacity by allowing elevating phase voltages without increasing acoustic noise levels.

Spatial Distribution of Black Soot and Its Health Effects in Port Harcourt Metropolis, Nigeria

This research presents a novel approach to assessing the health implications of black soot using a MiniVol air sampler.The MiniVol air sampler was used to collect PM from the ambient air at six monitoring sites in Port Harcourt,Nigeria.Sampling was conducted every day for seven days,for a period of 24 hours.PM_(2.5) concentrations at Uniport Junction,GRA Junction,Slaughter Roundabout,Abuloma Jetty,Rumuomasi Roundabout,and New Road Borokiri were 38.6 g/m^(3),28.3 g/m^(3),93.7 g/m^(3),72.9 g/m^(3),30.6 g/m^(3),and 31.3 g/m^(3),respectively.PM_(10) concentrations ranged from 71.2 g/m^(3) to 60.6 g/m^(3),with 103.3 g/m^(3),85.5 g/m^(3),40.1 g/m^(3),and 35.2 g/m^(3) being the highest.The level of PM_(2.5) and PM_(10) pollution in the ambient air was high across the six sampling sites,with mean PM_(2.5) and PM_(10) concentrations exceeding the WHO(2011)guideline.The flame atomic absorption spectrometry(FAAS)technique was used.The presence of heavy metals,such as mean metal concentrations of lead,cadmium,chromium,mercury,and nickel,ranged from 0.009 g/m^(3)-0.532 g/m^(3),0.002 g/m^(3)-0.544 g/m^(3),0.002 g/m^(3)-0.338 g/m^(3),0.001 g/m^(3),and 0.001 g/m^(3)-0.432 g/m^(3),across the six sampling sites.The GC-MS was used to determine the presence of PAHs in particulate matter.Correlation results revealed a strong positive correlation between PM_(2.5) and PM_(10).The findings also revealed a positive relationship between the metals as well as between the metals and PAHs,resulting in asthma,lung cancer,breathing difficulties,and miscarriages among pregnant women,which have affected the health implications of the people living in the environment.

目的:研究水蒸汽蒸馏法(HDE)和无溶剂微波萃取法(SME)对辣木籽精油化学组成的影响,并评估精油的细胞毒性。创新点:比较了不同的提取精油方法对精油的化学成分及其细胞毒性的影响。方法:分别采用HDE和SME两种方法提取辣木籽精油,然后使用气相色谱-质谱法分析其化学成分。通过比较在不同浓度和孵化时间下盐水虾卵的孵化率和幼虾的死亡率来检测精油的细胞毒性。结论:HDE和SME两种方法提取的辣木籽精油分别含有16和26种化学成分,各自占总可识别成分的97.515%和97.816%。因此,SME提取法含有更多的成分。盐水虾卵分别在含有两种精油、海水和氯霉素的样品中孵化24 h后,孵化率分别为60.0%(HDE)、56.7%(SME)、63.3%(海水)和15.0%(氯霉素),精油中的孵化率远高于含有氯霉素的样品的孵化率(P<0.05)。同时,SME和HDE精油对幼虾的半致死量分别为2908.23和3473.63μg/ml(大于1000μg/ml被认为无毒)。因此,两种方法提取的精油可广泛适用于药物、食品防腐剂、抗氧化剂和香料等添加剂。

Peripheral Vascular and Neuromuscular Responses to Ultramarathon Running

Participation in ultramarathon events has grown substantially in the past decade.However,poor understanding of the physiological outcomes associated with participation in this sport prevents athletes and clinicians from adequately addressing performance and recovery tactics.The purpose of this review was to summarize developments in the literature in the last 10 years regarding acute and chronic responses to ultramarathon running with a focus on the peripheral vascular system,neuromuscular outcomes,and running mechanics.Evidence suggests that there are acute impairments in large artery compliance especially following the longer ultramarathon distances.However,most literature indicates that chronic vascular impairments are not evident in ultramarathon runners.Both central and peripheral fatigue mechanisms contribute to declines in muscle force production that may last up to several weeks in some muscles following an ultramarathon.Alterations in gait kinematics and skeletal muscle oxidative capacity increase the metabolic cost of running over prolonged running distances(>4 h).Several factors such as elevation changes and nutritional practices make interpretation of findings challenging.Future studies are needed to better understand the interplay among systems and how external factors contribute to these outcomes to optimize performance and inform recovery strategies in this increasingly popular sport.

Nuclear facilities have a regulatory requirement to measure radiation levels within Post Operational Clean Out(POCO)around nuclear facilities each year,resulting in a trend towards robotic deployments to gain an improved understanding during nuclear decommissioning phases.The UK Nuclear Decommissioning Authority supports the view that human-in-the-loop(HITL)robotic deployments are a solution to improve procedures and reduce risks within radiation characterisation of nuclear sites.The authors present a novel implementation of a Cyber-Physical System(CPS)deployed in an analogue nuclear environment,comprised of a multi-robot(MR)team coordinated by a HITL operator through a digital twin interface.The development of the CPS created efficient partnerships across systems including robots,digital systems and human.This was presented as a multi-staged mission within an inspection scenario for the hetero-geneous Symbiotic Multi-Robot Fleet(SMuRF).Symbiotic interactions were achieved across the SMuRF where robots utilised automated collaborative governance to work together,where a single robot would face challenges in full characterisation of radiation.Key contributions include the demonstration of symbiotic autonomy and query-based learning of an autonomous mission supporting scalable autonomy and autonomy as a service.The coordination of the CPS was a success and displayed further challenges and improvements related to future MR fleets.
