Gastric cancer(GC)represents the fourth leading cause of cancer death worldwide and many factors can influence its development(diet,geographic area,genetic,Helicobacter pylori or Epstein-Barr virus infections).High quality endoscopy represents the modality of choice for GC *** correct morphologic classification during a high-resolution endoscopy is fundamental for oncologic diagnosis,staging and therapeutic *** its initial introduction in clinical practice the endoscopic ultrasound(EUS)has been considered a valuable tool for tumor(T-)and lymph nodes(N-)staging also in GC,in order to establish the best therapeutic strategy for the patient(e.g.,upfront surgery vs neoadjuvant treatments).EUS tools as elastography,Doppler and contrast administration can improve diagnosis mainly in case of malignant lymph node *** has a marginal role in disease staging but has a fundamental role in case of a pre-endoscopic resection management and in the new era of endoscopic mucosal resection or submucosal dissection as minimally invasive *** and locoregional staging of GC with EUS are a method of inarguable value for the assessment of gastric wall involvement and presence of infiltrated paragastric lymph *** can also have a role in disease restaging in those patients who have undergone neoadjuvant *** can also have a role in the advanced phases of the disease,in facilitating palliative,minimallyinvasive treatments,such as gastroenterostomy or biliary *** review intends to discuss the modern role of EUS in GC topic.

Longitudinal bending stiffness does not affect running economy in Nike Vaporfly Shoes

Purpose This study aimed to determine the independent effect of the curved carbon-fiber plate in the Nike Vaporfly 4%shoe on running economy and running *** Fifteen healthy male runners completed a metabolic protocol and a biomechanics *** both protocols participants wore 2 different shoes,an intact Nike Vaporfly 4%(VF_(intact))and a cut Nike Vaporfly 4%(VF_(cut)).The VF_(cut) had 6 medio-lateral cuts through the carbon-fiber plate in the forefoot to reduce the effectiveness of the *** the metabolic protocol,participants ran at 14 km/h for 5 min,twice with each shoe,on a force-measuring treadmill while we measured metabolic *** the biomechanics protocol,participants ran across a runway with embedded force plates at 14 km/*** calculated running economy,kinetics,and lower limb joint *** Running economy did not significantly differ between shoe conditions(on average,0.55%±1.77%(mean±SD))worse in the VF_(cut) compared to the VF_(intact);95%confidence interval(-1.44%to 0.40%).Biomechanical differences were only found in the metatarsophalangeal(MTP)joint with increased MTP dorsiflexion angle,angular velocity,and negative power in the VF_(cut).Contact time was 1%longer in the VF_(intact).Conclusion Cutting the carbon-fiber plate and reducing the longitudinal bending stiffness did not have a significant effect on the energy savings in the Nike Vaporfly 4%.This suggests that the plate's stiffening effect on the MTP joint plays a limited role in the reported energy savings,and instead savings are likely from a combination and interaction of the foam,geometry,and plate.

Use of doppler ultrasound to predict need for transjugular intrahepatic portosystemic shunt revision

BACKGROUND Transjugular intrahepatic portosystemic shunt(TIPS)is used to treat complications of portal hypertension,such as ascites and variceal bleeding(VB).While liver doppler ultrasound(DUS)is used to assess TIPS patency,trans-shunt venography(TSV)is the gold *** To determine the accuracy of DUS to assess TIPS dysfunction and for need for *** Retrospective review of patients referred for TIPS revision from ***,DUS parameters at baseline and at the DUS preceding TIPS revision,TSV data were *** operating characteristics curves,sensitivity,specificity,performance for doppler to predict need for revision were *** and multivariate analyses were used to predict clinical factors associated with need for TIPS *** The cohort consisted of 89 patients with cirrhosis(64%men,76%white,31%alcohol as etiology);median age 59 *** for initial TIPS were VB(41%),refractory ascites(51%),and other(8%).TIPS was revised in 44%.On univariate analysis,factors associated with need for TIPS revision were male(P=0.03),initial indication for TIPS(P=0.05)and indication for revision(P=0.01).Revision of TIPS was associated with lower mortality(26%vs 46%)and significantly lower rates of transplant(13%vs 24%;P=0.1).In predicting need for TIPS revision,DUS has a 40%sensitivity,45%specificity,PPV 78%,and NPV 14%.The most accurate location for shunt velocity measure was distal velocity(Area under the curve:0.79;P=0.0007).CONCLUSION DUS has poor overall sensitivity and specificity in predicting need for TIPS ***-invasive methods of predicting TIPS dysfunction are needed since those needing TIPS revision had better survival.

目的　通过对软骨肉瘤细胞中成骨细胞相关基因的表达进行分析 ,探讨软骨肉瘤中细胞分化的分子表型。　方法　采用RT PCR和印记方法半定量测定 32例软骨肉瘤患者骨钙素 (OC)、碱性磷酸酶 (AP)、骨结合蛋白 (ON)和Cbfa 1的表达 ,分析其与组织学分级和预后的关系。　结果　病理学分级与OC、AP或Cbfa 1表达有明显相关性 ;成骨细胞相关基因在Ⅲ级软骨肉瘤的表达最为多见。骨钙素mRNA表达的检测 ,在 12例Ⅲ级患者有 9例可测到 (75 % ) ,在 2 0例Ⅰ～Ⅱ级患者中有 4例可测到 (2 0 % ) (P =0 .0 0 8)。在 32例患者中 ,2 1个可测到APmRNA ,其中 12例Ⅲ级患者中有 11例(92 % )、2 0例Ⅰ～Ⅱ级患者中有 10例 (5 0 % )可测到AP的表达 (P =0 .0 18)。OC或AP表达常与无病生存率下降相联系 (OC :P =0 .0 16 ;AP :P =0 .0 18)。Cbfa1mRNA只在 4例Ⅲ级软骨肉瘤中测到。所有软骨肉瘤中均可测到ON ,表明其表达与病理分级及无病生存率无相关性。　结论　绝大多数Ⅲ级软骨肉瘤中可测到OC的表达 ,而在Ⅰ～Ⅱ级软骨肉瘤中只有少数表达 ,并且OC的表达常与预后差相关联 ;APmRNA阳性率在Ⅰ～Ⅱ级和Ⅲ级中有明显的差别 ,AP的表达也和无病生存率下降呈正相关 ;在软骨肉瘤中OC或AP的表达可做为独立的预后指标 ;Cbfa

目的采集手术切除未经培养的骨肉瘤标本,研究肿瘤中成骨细胞的分化及其与临床因素的关系。方法观察骨肉瘤中成骨细胞相关基因表达的频率、特异性,以及骨肉瘤成骨细胞表型与生物学行为的关系。应用RT-PCR方法研究48例骨肉瘤标本的碱性磷酸酶(ALP)、骨钙素(OC)和骨桥蛋白(ON)的mRNA转录。磷酸甘油酸激酶(PGK)被用于内参照。结果在大多数骨肉瘤标本中可以发现ALP或OC的mRNA转录(分别为93.75%,79.17%)。在所有的肉瘤检测中均可发现ON的mRNA转录。在骨肉瘤的成骨细胞亚型中可见到ALP和OC的高水平表达。肺转移瘤OC的mRNA表达弱于原发灶。肿瘤细胞有高水平OC表达的病例组有较高的生存率(P=0.045)。结论OC是目前最好的成骨细胞特异性标记物,将来可能用于检测骨肉瘤患者外周血中循环的肿瘤细胞。

Adolescent alcohol abuse is a substantive public health problem that has been the subject of intensive study in recent *** reports of a wide range of effects of adolescent intermittent ethanol(AIE)exposure on brain and behavior,little is known about the mechanisms that may underlie those effects,and even less about treatments that might reverse *** studies from our laboratory have indicated that AIE produced enduring changes in astrocyte function and synaptic activity in the hippocampal formation,suggesting the possibility of an alteration in astrocyte-neuronal connectivity and *** utilized astrocyte-specific,membrane restricted viral labeling paired with immunohistochemistry to perform confocal single cell astrocyte imaging,three-dimensional reconstruction,and quantification of astrocyte morphology in hippocampal area CA1 from adult rats after ***,we assessed the colocalization of astrocyte plasma membrane labeling with immunoreactivity for AMPA-(α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid)glutamate receptor 1,an AMPA receptor subunit and established neuronal marker of excitatory synapses,as a metric of astrocyte-synapse *** significantly reduced the colocalization of the astrocyte plasma membrane with synaptic marker puncta in *** is striking in that it suggests not only an alteration of the physical association of astrocytes with synapses by AIE,but one that lasts into adulthood-well after the termination of alcohol *** even more notable,the AIE-induced reduction of astrocyte-synapse interaction was reversed by sub-chronic treatment with the clinically used agent,gabapentin(Neurontin),in *** suggests that a medication in common clinical use may have the potential to reverse some of the enduring effects of adolescent alcohol exposure on brain *** animal experiments conducted were approved by the Duke University Institutional Animal Care and Use Committee(Protocol

Despite improvements in technique and technology for total knee arthroplasty(TKA),anterior knee pain impacts patient outcomes and *** the prosthetic and surgical technique related causes of pain after TKA,specifically as it relates to anterior knee pain,can aid surgeons in addressing these issues with their *** features of the femoral and patellar components which have been reported as pain generators include: Improper femoral as well as patellar component sizing or designs that result in patellofemoral stuffing; a shortened trochlear groove distance from the flange to the intercondylar box; and then surgical technique related issues resulting in: Lateral patellar facet syndrome; overstuffed patella/flange combination; asymmetric patellar resurfacing,improper transverse plane component rotation resulting in patellar subluxation/*** design consideration that allows impingement of extensor mechanism anatomical elements has the possibility of impacting outcome by becoming a pain *** the number of TKA procedures continues to increase,it is increasingly critical to develop improved,evidence based prostheses that maximize function and patient satisfaction while minimizing pain and other complications.

Remotely sensed data are frequently used for predicting and mapping ecosystem characteristics,and spatially explicit wall-to-wall information is sometimes proposed as the best possible source of information for ***,wall-to-wall information typically relies on model-based prediction,and several features of model-based prediction should be understood before extensively relying on this type of *** such feature is that model-based predictors can be considered both unbiased and biased at the same time,which has important implications in several areas of *** this discussion paper,we first describe the conventional model-unbiasedness paradigm that underpins most prediction techniques using remotely sensed(or other)auxiliary *** this point of view,model-based predictors are typically ***,we show that for specific domains,identified based on their true values,the same model-based predictors can be considered biased,and sometimes severely *** suggest distinguishing between conventional model-bias,defined in the statistical literature as the difference between the expected value of a predictor and the expected value of the quantity being predicted,and design-bias of model-based estimators,defined as the difference between the expected value of a model-based estimator and the true value of the quantity being *** show that model-based estimators(or predictors)are typically design-biased,and that there is a trend in the design-bias from overestimating small true values to underestimating large true ***,we give examples of applications where this is important to acknowledge and to potentially make adjustments to correct for the design-bias *** argue that relying entirely on conventional model-unbiasedness may lead to mistakes in several areas of application that use predictions from remotely sensed data.
