背景与目的我们回顾性评估了程序性死亡配体1（programmed death ligand 1, PD-L1）阳性非小细胞肺癌（non-small cell lung cancer, NSCLC）患者的预后，以比较基线（即入组时）脑转移与帕博利珠单抗及化疗的疗效关系。方法我们汇总分析了KEYNOTE-001（NCT01295827）、KEYNOTE-010（NCT01905657）、KEYNOTE-024（NCT02142738）和KEYNOTE-042（NCT02220894）研究中既往接受过治疗或未接受过的PD-L1阳性[肿瘤细胞阳性比例评分（tumor proportion score, TPS）≥1%]的晚期或转移性NSCLC患者的数据。患者接受帕博利珠单抗（2 mg/kg,10 mg/kg，或每3周200 mg或每2周10 mg/kg）治疗；而化疗是除KEYNOTE-001外所有研究的对照组。所有研究都纳入了既往接受过治疗的稳定脑转移的患者。结果共计纳入3,170例患者，基线时，293例（9.2%）患者伴有脑转移，而2,877例（90.8%）是无脑转移患者；数据截止时，中位（范围）随访时间为12.9（0.1-43.7）个月。与化疗相比，帕博利珠单抗可改善基线伴有脑转移及无脑转移患者的总生存期：PD-L1 TPS≥50%[分别为0.67（95%CI:0.44-1.02）和0.66（95%CI:0.58-0.76）]和PD-L1 TPS≥1%[分别为0.83（95%CI:0.62-1.10）和0.78（95%CI:0.71-0.85）]。无论是否有脑转移，与化疗相比，接受帕博利珠单抗治疗患者的无进展生存期更长，客观缓解率更高，缓解持续时间更长。在脑转移患者中，帕博利珠单抗与化疗的治疗相关不良事件发生率分别为66.3%和84.4%，在无脑转移患者中分别为67.2%和88.3%。结论对于既往未接受治疗或接受过治疗的PD-L1阳性晚期/转移性NSCLC患者，无论基线时是否存在治疗后稳定的脑转移，帕博利珠单抗单药治疗显示出明显的疗效改善，且与化疗相比，不良事件更少。

Healthcare Services Interoperability in Kenya: Challenges and Opportunities

In an attempt to assess the Kenyan healthcare system, this study looks at the current efforts that are already in place, what challenges they face, and what strategies can be put into practice to foster interoperability. By reviewing a variety of literature and using statistics, the paper ascertains notable impediments such as the absence of standard protocols, lack of adequate technological infrastructure, and weak regulatory frameworks. Resultant effects from these challenges regarding health provision target enhanced data sharing and merging for better patient outcomes and allocation of resources. It also highlights several opportunities that include the adoption of emerging technologies, and the establishment of public-private partnerships to strengthen the healthcare framework among others. In this regard, the article provides recommendations based on stakeholder views and global best practices addressed to policymakers, medical practitioners, and IT specialists concerned with achieving effective interoperability within Kenya’s health system. This research is relevant because it adds knowledge to the existing literature on how healthcare quality can be improved to make it more patient-centered especially in Kenya.

Enantioselective iridium-catalyzed allylic substitution with a Reformatsky reagent: direct construction of β-stereogenic homoallylic esters

The Reformatsky reagent derived from ethyl bromoacetate circumvents the challenge associated with the direct regio-and enantioselective iridium-catalyzed allylic alkylation with an unstabilized aliphatic ester enolate. Notably, this work represents the first example of using a Reformatsky reagent in a metal-allyl reaction, which permits the direct construction of enantioenriched β-stereogenic homoallylic esters without preactivation of the pronucleophile or post-reaction modification of the product(e.g., thermal decarboxylation of a 1,3-dicarbonyl). Moreover, the process allows the coupling of a broad range of sterically and electronically diverse electrophiles bearing aryl, heteroaryl and alkenyl derivatives, including the significantly more challenging alkyl-substituted allylic carbonates. The versatility of the ester product is exemplified by a series of functional group manipulations and the development of a concise and efficient enantioselective total synthesis of the natural product,(+)-descurainolide A. Finally, mechanistic studies indicate that the zinc enolate behaves as a soft nucleophile that undergoes outer-sphere alkylation.

Outcomes following minimally invasive dorsal cheilectomy for hallux rigidus:A systematic review

Rice fields are a major source of greenhouse gases,such as nitrous oxide (N_(2)O) and methane (CH_(4)).Organic fertilizers may potentially replace inorganic fertilizers to meet the nitrogen requirement for rice growth;however,the simultaneous effects of organic fertilizers on N_(2)O and CH_(4)emissions and crop yield in paddy fields remain poorly understood and *** this study,experimental plots were established in conventional double-cropping paddy fields in the Pearl River Delta,China,including an unfertilized control and five fertilizer treatments with fresh organic fertilizer (FOF),successively composted organic fertilizer (SOF),chemically composted organic fertilizer (COF),COF supplemented with inorganic fertilizer (COIF),and chemical fertilizers (CFs)(TFOF,TSOF,TCOF,TCOIF,and TCF,respectively).Paddy field soils behaved simultaneously as an N_(2)O sink (cumulative N_(2)O emission:-196 to-381 g N ha^(-1)) and as a CH_(4)source (cumulative CH_(4)emission:719 to 2 178 kg ha^(-1)).Compared to CFs,the effects of organic fertilizers on N_(2)O emission were not *** contrast,total annual CH_(4)emission increased by 157%,132%,125%,and 37%in TFOF,TCOF,TSOF,and TCOIF,respectively,compared to *** TCOIF,rice yield was maintained,while CH_(4)emission was not significantly increased from the paddy fields characterized by a prolonged flood *** important next step is to extend these field-based measurements to larger rice cultivation areas to quantify the regional and national-scale impacts on greenhouse gas emissions and to help determine the optimum practice for fertilizer use.

Implementation of Fuzzy Logic Control into an Equivalent Minimization Strategy for Adaptive Energy Management of A Parallel Hybrid Electric Vehicle

As government agencies continue to tighten emissions regulations due to the continued increase in greenhouse gas production, automotive industries are seeking to produce increasingly efficient vehicle technology. Hybrid electric vehicles (HEVs) have been introduced to mitigate problems while improving fuel economy. HEVs have led to the demand of creating more advanced controls software to consider multiple components for propulsive power in a vehicle. A large section in the software development process is the implementation of an optimal energy management strategy meant to improve the overall fuel efficiency of the vehicle. Optimal strategies can be implemented when driving conditions are known a prior. The Equivalent Consumption Minimization Strategy (ECMS) is an optimal control strategy that uses an equivalence factor to equate electrical to mechanical power when performing torque split determination between the internal combustion engine and electric motor for propulsive and regenerative torque. This equivalence factor is determined from offline vehicle simulations using a sensitivity analysis to provide optimal fuel economy results while maintaining predetermined high voltage battery state of charge (SOC) constraints. When the control hierarchy is modified or different driving styles are applied, the analysis must be redone to update the equivalence factor. The goal of this work is to implement a fuzzy logic controller that dynamically updates the equivalence factor to improve fuel economy, maintain a strict charge sustaining window of operation for the high voltage battery, and reduce computational time required during algorithm development. The adaptive algorithm is validated against global optimum fuel economy and charge sustaining results from a sensitivity analysis performed for multiple drive cycles.
