Transbronchial needle aspiration(TBNA)is a commonly used sampling approach in the diagnosis of hilar and mediastinal lymphadenopathy as well as peripheral *** a very important tool,the continued innovation of TBNA needles is a vital driving force for the development of the *** TBNA plays an important role in interventional pulmonology,there are no clear standards guiding operators to choose an appropriate needle for their *** recent decades,with the advent of endobronchial ultrasound-guided TBNA(EBUS-TBNA),the real-time visualization of TBNA has been *** modern TBNA needles,such as ViziShot2,FLEX 19G,Acquire FNB,and EchoTip ProCore,have made significant progress in specimen collection,convenience,and safety,though still remain grounded in the basic premise and initial upgrades to the original conventional TBNA(cTBNA)*** review introduced the developmental history of WANG cTBNA needles,and summarized the lessons of success and failure and the enlightenments for currently used EBUS-and other emerging TBNA needles,aiming to provide a significant reference for pulmonologists who lived through the cTBNA era and for junior physicians who start working in the EBUS-TBNA *** its long history,TBNA is still playing significant roles in the diagnosis of pulmonary diseases.A deeper understanding from the historical perspectives would facilitate continued innovations in the field of TBNA and beyond.

Transport and degradation of Phenol in Groundwater at Four Ashes

drift deposits and Triassic Sandstone of the Bromsgrove and Wildmoor Formations, 700m thick, form the main aquifers at Four Ashes. The coal tar and products from early plant and tank storage facilities constructed over unprotected ground directly and indirectly caused significant groundwater pollution. The organic pollutants include phenol, cresol, and xylenol. The maximum phenol concentration in the groundwater reached 12000mg/L, with an average of 1300mg/L, which accounts for 40%60% of the total organic contaminants. Three computer codes, Visual Modflow, MT3d, and BioRedox, which solute transport model to groundwater flow were used to simulate and predict the distribution, transport, and degradation of phenol in the polluted groundwater. Over about 46 years, the phenol moved from the pollutant source to the plume front and it will take 220 years to reach the main pumping wells. The BioRebox model was used to simulate the aerobic, sulfidogenic, nitrate reducing, ferrogenic, manganogenic, and methanogenic zones. The residual mass in the groundwater will decrease from 1600 t to 400 t by the year 2080, with 80% of total phenol eventually lost with maximum concentration declining from 15000mg/L to 2000mg/L. 

Objective:To describe a patient with presarcopenia and chronic cough secondary to chronic obstructive pulmonary disease(COPd)successfully treated with supplements and physical *** report:A 75-year-old female patient with a positive past medical history of systemic hypertension,dyslipidemia,heart arrhythmia,and smoking during 20 years evolved with chronic cough due to a chronic obstructive pulmonary disease diagnosed ten years *** came to our private clinic due to low weight,low *** weight was 44.8 kg,her height 1.57 m,body mass index of 18.18 kg/m^(2).Laboratory tests showed SdHEA 76.3,vitamin d of 15.6 ng/ml(nr:>30 ng/ml),C-reactive protein(CRP)of 55 mg/*** tomography showed *** had an skeletal muscle mass index of 5.0 kg/m^(2)(nr:>5.5 kg/m^(2))by dXA.A diagnosis of presarcopenia was determined based on dXA evaluation with low muscle mass but normal gait speed handgrip *** suggested to the patient to increase physical exercise and prescribed a supplement *** five months,she returned asymptomatic,without cough,marked improvement of fatigue,increased energy levels,and weight increased to 50 kg,BMI of 20.28 kg/m^(2).Laboratory tests showed SdHEA to 140,vitamin d3 to 64.5 ng/ml,reduced CRP reduced to 5 mg/dl,and amlodipine was excluded due to better blood pressure ***,two years later,the patient continues without cough and has dyspnea only with high efforts,without fatigue,and her weight is 52 kg and BMI 21.1 kg/m^(2).She also reduced her conventional treatment for COPd,using only a bronchodilator on-demand,without topical ***:This case illustrates an interesting case of a patient with presarcopenia and chronic cough,refractory to conventional approach,successfully treated with a combination of nutraceuticals and physical exercises.

＜正＞The aberrant proteolytic processing of amyloid precursor protein（APP） by b-secretase（B ACE 1） and g-secretase is important for the generation of beta-amyloid protein in the Alzheimer’s disease（Ad） brains.Aβis the major component of senile plaque and may be toxic to brain *** test weather BACE1 expression and beta-amyloid protein production can be induced by transient brain ischemia in a continuous way,and the toxicity of Aβto brain *** use Middle Cerebral Artery Occlusion（MCAO） model,Animals were euthanized at 1d,3d,7d and 14d *** blot analysis investigated that BACE-1 protein began to elevate at 1day and persisted up to 14days after ischemia compared to normal（P＜0.05）.The alterations of BACE1 protein levels were accompanied by corresponding increases of Aβx-42 peptides,which demonstrated by *** sections were also processed for the expression of BACE1 and glial or neuronal markers by double immunofluoresence labeling and evaluated by confocal laser scanning *** was no detectable expression of BACE1 protein by activated microglia,most of BACE1 protein was expressed by reactive astrocytes,and only several neurons were found to express this protein in the *** determine the toxicity of beta-amyloid peptide,we performed Aβx-42 and activated caspase-3 double staining by immunohistochemistry, about 83%Aβx-42 peptides staining cells were apoptotic in the ipsilateral striatum at 7d after *** findings suggested that beta-amyloid peptides were derived from the aberrant processing of APP by BACE1 in the same reactive astrocytes after ischemia and the production of Aβwas correlated with the apoptosis of *** aberrant APP processing and apoptosis in astrocytes after ischemia may imply the corresponding events in Ad.

Sex differences in the susceptibility of SARS-CoV-2 infection and severity have been controversial,and the underlying mechanisms of COVId-19 in a sex-specific manner remain *** we inspected sex differences in SARS-CoV-2 infection,hospitalization,admission to the intensive care unit(ICU),sera inflammatory biomarker profiling,and single-cell RNA-sequencing(scRNA-seq)profiles across nasal,bronchoalveolar lavage fluid(BALF),and peripheral blood mononuclear cells(PBMCs)from COVId-19 patients with varying degrees of disease *** propensity score-matching observations revealed that male individuals have a 29%elevated likelihood of SARS-CoV-2 positivity,with a hazard ratio(HR)1.32(95%confidence interval[Cl]1.18-1.48)for hospitalization and HR 1.51(95%Cl 1.24-1.84)for admission to *** from male patients at hospital admission had elevated neutrophil-lymphocyte ratio and elevated expression of inflammatory markers(C-reactive protein and procalcitonin).We found that SARS-CoV-2 entry factors,including ACE2,TMPRSS2,FURIN,and NRP1,have elevated expression in nasal squamous cells from male individuals with moderate and severe *** observed male-biased transcriptional activation in SARS-CoV-2-infected macrophages from BALF and sputum samples,which offers potential molecular mechanism for sex-biased susceptibility to viral ***-cell interaction network analysis reveals potential epithelium-immune cell interactions and immune vulnerability underlying male-elevated disease severity and mortality in ***,monocyte-elevated expression of Toll-like receptor 7{TLR7)and Bruton tyrosine kinase{BTK)is associated with severe outcomes in males with *** summary,these findings provide basis to decipher immune responses underlying sex differences and designing sex-specific targeted interventions and patient care for COVId-19.
