Objective:To correlate the utility of the Fundamentals of Laparoscopic Surgery(FLS)manual skills program with the Objective Structured Assessment of Technical Skills(OSATS)global rating scale in evaluating operative ***:The Asian Urological Surgery Training and Educational Group(AUSTEG)Laparoscopic Upper Tract Surgery Course implemented and validated the FLS program for its usage in laparoscopic surgical ***’basic laparoscopic skills were assessed using three different training models(peg transfer,precision cutting,and intra-corporeal suturing).They also performed live porcine laparoscopic surgery at the same *** surgery skills were assessed by blinded faculty using the OSATS rating ***:From March 2016 to March 2019,a total of 81 certified urologists participated in the course,with a median of 5 years of post-residency *** differences in task time did not reach statistical significance,those with more surgical experience were visibly faster at completing the peg transfer and intra-corporeal suturing FLS ***,they took longer to complete the precision cutting task than participants with less *** OSATS scores correlated weakly with all three FLS tasks(peg transfer time:r=0.331,r^(2)=0.110;precision cutting time:r=0.240,r^(2)=0.058;suturing with intracorporeal knot time:r=0.451,r^(2)=0.203).Conclusion:FLS task parameters did not correlate strongly with OSATS globing rating scale *** FLS task models demonstrated strong validity,it is important to assimilate the inconsistencies when benchmarking technical proficiency against real-life operative competence,as evaluated by FLS and OSATS,respectively.

The vertical phase distribution of active layers plays a vital role in balancing exciton dissociation and cha rge transport for achieving efficient polymer solar cells(PSCs).The layer-by-layer(LbL)PSCs are commonly prepared by using sequential spin-coating method from donor and acceptor solutions with distinct solvents and solvent *** enhanced exciton dissociation is expected in the LbL PSCs with efficient charge transport in the relatively neat donor or acceptor *** this work,a series of LbL all-polymer solar cells(APSCs)were fabricated with PM6 as donor and PY-DT as acceptor,and triplet material m-Ir(CPmPB)_(3)is deliberately incorporated into PY-DT layer to prolong exciton lifetimes of active *** power conversion efficiency(PCE)of LbL APSCs is improved to 18.24%from 17.32%by incorporating 0.3 wt%m-Ir(CPMPB)_(3)in PY-DT layer,benefiting from the simultaneously enhanced short-circuit current density(Isc)of 25.17 mA cm^(-2)and fill factor(FF)of 74.70%.The enhancement of PCE is attributed to the efficient energy transfer of m-Ir(CPmPB)_(3)to PM6 and PY-DT,resulting in the prolonged exciton lifetime in the active layer and the increased exciton diffusion *** efficient energy transfer from m-Ir(CPmPB)_(3)to PM6 and PY-DT layer can be confirmed by the increased photoluminescence(PL)intensity and the prolonged PL lifetime of PM6 and PY-DT in PM6+m-Ir(CPmPB)_(3)and PY-DT+m-Ir(CPmPB)_(3)*** study indicates that the triplet material as solid additive has great potential in fabricating efficient LbL APSCs by prolonging exciton lifetimes in active layers.

Effects of Saccharomyces cerevisiae or boulardii yeasts on acute stress induced intestinal dysmotility

AIM To investigate the capacity of Saccharomyces cerevisiae(S. cerevisiae) and Saccharomyces boulardii(S. boulardii) yeasts to reverse or to treat acute stress-related intestinal *** Adult Swiss Webster mice were stressed for 1 h in a wire-mesh restraint to induce symptoms of intestinal dysmotility and were subsequently killed by cervical dislocation. Jejunal and colon tissue were excised and placed within a tissue perfusion bath in which S. cerevisiae, S. boulardii, or their supernatants were administered into the lumen. Video recordings of contractility and gut diameter changes were converted to spatiotemporal maps and the velocity, frequency, and amplitude of propagating contractile clusters(PCC) were measured. Motility pre- and post-treatment was compared between stressed animals and unstressed controls. RESULTS S. boulardii and S. cerevisiae helped to mediate the effects of stress on the small and large intestine. Restraint stress reduced jejunal transit velocity(mm/s)from 2.635 ± 0.316 to 1.644 ± 0.238, P < 0.001 and jejunal transit frequency(Hz) from 0.032 ± 0.008 to 0.016 ± 0.005, P < 0.001. Restraint stress increased colonic transit velocity(mm/s) from 0.864 ± 0.183 to 1.432 ± 0.329, P < 0.001 and frequency to a lesser degree. Luminal application of S. boulardii helped to restore jejunal and colonic velocity towards the unstressed controls; 1.833 ± 0.688 to 2.627 ± 0.664, P < 0.001 and 1.516 ± 0.263 to 1.036 ± 0.21, P < 0.001, respectively. S. cerevisiae also had therapeutic effects on the stressed gut, but was most apparent in the jejunum. S. cerevisiae increased PCC velocity in the stressed jejunum from 1.763 ± 0.397 to 2.017 ± 0.48, P = 0.0031 and PCC frequency from 0.016 ± 0.009 to 0.027 ± 0.007, P < 0.001. S. cerevisiae decreased colon PCC velocity from 1.647 ± 0.187 to 1.038 ± 0.222, P < 0.001. Aflibercept for diabetic macular oedema： a Meta-analysis of randomized controlled trials

AIM： To evaluate the relative efficacy and safety of aflibercept for treatment of diabetic macular oedema （DMO）. METHODS： A comprehensive search in MEDLINE, CENTRAL and EMBASE was undertaken for randomized controlled trials （RCTs） comparing intravitreal anti-vascular endothelial growth factor （anti-VEGF） versus another treatment. Primary outcome measures were proportion of patients with at least 15 letters of gain or loss on a logMAR visual acuity chart, and change in best corrected visual acuity （BCVA） and central macular thickness （CMT） from baseline. Safety outcomes were rates of death, thromboembolic events and any systemic or ocular serious adverse events. The final search was performed on November 2017. RESULTS： Four RCTs were included. Only one trial compared efficacy and safety of aflibercept with bevacizumab and ranibizumab over 1 or 2y. Three trials were included for Meta-analysis comprising 661 patients （331 in the aflibercept, and 330 in the photocoagulation group）. Aflibercept was more efficacious compared to photocoagulation in the proportion of patients with at least 15 letters of improvement and worsening, and in improvement of BCVA and reduction in CMT at 1 or 2y. The safety estimates at 1 or 2y did not differ statistically. CONCLUSION： Aflibercept offers superior benefits over photocoagulation in improving and preserving vision, with no differences in safety. Further comparative effectiveness trials between aflibercept and other anti-VEGF agents will aid ophthalmologists in treatment decisions.

Different Asian Monsoon Rainfall Responses to Idealized Orography Sensitivity Experiments in the HadGEM3-GA6 and FGOALS-FAMIL Global Climate Models

Recent work has shown the dominance of the Himalaya in supporting the Indian summer monsoon （ISM）, perhaps by surface sensible heating along its southern slope and by mechanical blocking acting to separate moist tropical flow from drier midlatitnde air. Previous studies have also shown that Indian snmmer rainfall is largely unaffected in sensitivity experiments that remove only the Tibetan Plateau. However, given the large biases in simulating the monsoon in CMIP5 models, such results may be model dependent. This study investigates the impact of orographic forcing from the Tibetan Plateau, Himalaya and Iranian Plateau on the ISM and East Asian snmmer monsoon （EASM） in the UK Met Office＇s HadGEM3-GA6 and China＇s Institute of Atmospheric Physics FGOALS-FAMIL global climate models. The models chosen featnre oppositesigned biases in their simulation of the ISM rainfall and circulation climatology. The changes to ISM and EASM circulation across the sensitivity experiments are similar in both models and consistent with previous studies. However, considerable differences exist in the rainfall responses over India and China, and in the detailed aspects such as onset and retreat dates. In particular, the models show opposing changes in Indian monsoon rainfall when the Himalaya and Tibetan Plateau orography are removed. Our results show that a multi-model approach, as suggested in the forthcoming Global Monsoon Model Intercomparison Project （GMMIP） associated with CMIP6, is needed to clarify the impact of orographic forcing on the Asian monsoon and to fully understand the implications of model systematic error.

Anxiety is a significant mental health issue that substantially affects an individual’s quality of life. Feelings of uneasiness, irritability, and sleep disturbances characterize it. 4-Hydroxyphenyl acetic acid (4-HPAA) is identified in brain cells as a physiological byproduct of tyramine. This study hypothesizes that 4-HPAA may regulate anxiety due to its anxiolytic properties, acting as a modulator of the GABAergic system, which plays a crucial role in the pathophysiology of anxiety disorders. Our study aims to enhance the anxiolytic effects of 4-HPAA through chemical modification to improve its pharmacokinetic properties. Three derivatives, namely Isopropyl-4-hydroxy-[phenyl] acetate (IHPA), Isopropyl-4-hydroxy-[phenyl] acetate (MPAA), and 4-methoxyphenyl acetate (MPHA), have been synthesized from 4-HPAA. This assessment will use well-established animal models, specifically the Elevated Plus-Maze (EPM) and Zero Maze (EZM) tests, selected for their validity in replicating anxiety-like symptoms in animals. Chronic caffeine administration via drinking water (0.3 g/l for 14 days) was employed to induce an anxiety state for testing purposes. IHPA and MPAA demonstrated significant anxiolyticactivity when tested in the EPM and EZM experiments. Molecular docking simulations using AutoDock Vina indicated that 4-HPAA derivatives had docking scores ranging from −5.8 to −4.8 kcal/mol, compared to the standard anxiolytic medication Diazepam, which scored −7.1 kcal/mol. These scores suggest a potential for 4-HPAA derivatives to interact effectively with the Gamma-aminobutyric acid (GABA_A) receptor. In conclusion, our in vivo and in silico analyses indicate a promising anxiolytic potential for 4-HPAA derivatives.

目的用音樂療法作為放鬆的介入方式,以減輕結腸鏡檢查者術中的不適,促進結腸鏡檢查的進程及改善舒適度。方法2021年4月12日~5月18日,於澳門某醫院對104位結腸鏡檢查者進行研究,分為研究組及對照組(每組各52人)。研究組結腸鏡檢查者介入音樂療法,對照組予常規照護,比較兩組疼痛評分(numerical rating scale,NRS)、收縮壓、舒張壓、心率及結腸鏡檢查進鏡時間的差異。結果兩組研究參與者在NRS疼痛評分、術中及術後血壓及心率無差異;兩組結腸鏡檢查的進鏡時間均數沒有顯著差異;但進鏡時間≦10分鐘組(非困難腸鏡組)與>10分鐘組(困難腸鏡組)則存在組間差異,在非困難腸鏡組中,研究組進鏡時間為6.2±1.6分鐘,對照組進鏡時間為7.2±1.6分鐘(T=-2.075,P<0.05);困難腸鏡組則沒有顯著差異。結論音樂放鬆療法對結腸鏡檢查者的疼痛緩解、血壓及心率穩定沒有顯著作用,但仍可縮短“非困難腸鏡”的進鏡時間。
