Anal pruritus is a common anorectal symptom that can significantly impair a patient’s quality of life,including their mental health.It can be one of the most difficult proctological conditions to treat.Patients often delay seeking medical attention,since it is an embarrassing but non-life-threatening situation.Pruritus ani can be associated with idiopathic and secondary causes,such as anorectal diseases,cancer(anal or colorectal),dermatological and sexually transmitted diseases,fungal infections and systemic diseases.If patients are referred for a colonoscopy,this can sometimes provide the first opportunity to evaluate the perianal area.Classifications of anal pruritus are based on the abnormalities of the perianal skin,one of the most commonly used being the Washington classi-fication.A proper digital anorectal examination is important,as well as an anoscopy to help to exclude anorectal diseases or suspicious masses.Endoscopists should be aware of the common etiologies,and classification of the perianal area abnormalities should be provided in the colonoscopy report.Information on treatment possibilities and follow-up can also be provided.The treatment normally consists of a triple approach:proper hygiene,elimination of irritants,and skin care and protection.Several topical therapies have been described as possible treatments,including steroids,capsaicin,tacrolimus and methylene blue intradermal injections.

Testicular prostheses-impact on quality of life and sexual function

Orchiectomy is the recommended treatment for many testicular conditions.However,testicular prosthesis placement is not always performed for different reasons.In this study,we aimed to evaluate patients'opinions and the impact on sexual function and quality of life.This retrospective observational single-center study included patients who underwent orchiectomy between January 2014 and December 2020 at the Department of Urology,Braga's Hospital(Braga,Portugal),where testicular implants were always available and the decision to undergo the procedure was made fully independent of cost.Patients completed four questionnaires that assessed demographic data,satisfaction,self-esteem,and sexual function.Of the 96 patients who underwent orchidectomy,59 replied to the questionnaires,and of these patients,86.4%decided to undergo silicone-based testicular prostheses implantation.The remaining 13.6%refused the implant based on concerns about complications(37.5%),because they felt that it was unnecessary(37.5%),or because it was not offered by the doctor(25.0%).Overall,96.1%of these patients were satisfied with the implant;however,25.5%classified it as"too firm".No statistically significant differences were found in sexual function(all P>0.05).However,it can be observed that there are more patients with prostheses presenting normal sexual activity compared to patients without prostheses(74.0%vs 50.0%),and none of them reported severe erectile dysfunction(0 vs 16.7%).Regarding self-esteem,both patients with and without prostheses present very similar average scores with no statistically significant differences.The present study highlights the highest level of satisfaction among patients who received testicular prostheses.Testicular prostheses implantation is a safe procedure that does not hamper sexual function after orchiectomy.

Combinatorial therapies for spinal cord injury repair

Spinal cord injuries have profound detrimental effects on individuals, regardless of whether they are caused by trauma or non-traumatic events. The compromised regeneration of the spinal cord is primarily attributed to damaged neurons, inhibitory molecules, dysfunctional immune response, and glial scarring. Unfortunately, currently, there are no effective treatments available that can fully repair the spinal cord and improve functional outcomes. Nevertheless, numerous pre-clinical approaches have been studied for spinal cord injury recovery, including using biomaterials, cells, drugs, or technological-based strategies. Combinatorial treatments, which target various aspects of spinal cord injury pathophysiology, have been extensively tested in the last decade. These approaches aim to synergistically enhance repair processes by addressing various obstacles faced during spinal cord regeneration. Thus, this review intends to provide scientists and clinicians with an overview of pre-clinical combinatorial approaches that have been developed toward the solution of spinal cord regeneration as well as update the current knowledge about spinal cord injury pathophysiology with an emphasis on the current clinical management.

Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

Objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the other.Methods: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. Only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November 2022.Results: Of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. One article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent among articles.Conclusion: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

目的旨在通过系统综述,确定针对复杂慢性病患者增强自我管理的干预措施,以改善慢性呼吸困难,为护理人员促进该类人群的自我管理提供依据。方法2023年12月,系统检索Medline、Scopus、Web of Science、CINAHL、Cochrane图书馆和乔安娜布里格斯研究所(Joanna Briggs Institute,JBI)循证实践数据库。对纳入研究进行数据提取和综合,研究人员采用Rayyan文献管理软件筛选标题及摘要,并依据JBI研究质量评估工具进行方法学质量评估。结果共纳入18项研究。这些研究探讨了针对慢性呼吸困难的各种干预措施,包括药物干预(氧气疗法和吸入疗法)和非药物方法(教育计划、呼吸练习、液体摄入管理、身体感知技术、同伴支持、情商培训和网络应用程序的使用)。这些干预措施减轻了患者情绪困扰,可以使患者在自我管理方面变得独立自主。结论促进复杂慢性病患者对慢性呼吸困难的自我管理需要采取综合方法,但目前研究中没有充分考虑到针对患者行为改变的干预措施,需要进一步开展相关研究完善干预措施。

Behavioral and endocrine responses to noninteractive live and video conspecifics in males of the Siamese fighting fish

The physiological mechanisms underlying variation in aggression in fish remain poorly understood.One possibly confounding variable is the lack of standardization in the type of stimuli used to elicit aggression.The presentation of controlled stimuli in videos,a.***.a.video playback,can provide better control of the fight components.However,this technique has produced conflicting results in animal behavior studies and needs to be carefully validated.For this,a similar response to the video and an equivalent live stimulus needs to be demonstrated.Further,different physiological responses may be triggered by live and video stimuli,and it is important to demonstrate that video images elicit appropriate physiological reactions.Here,the behavioral and endocrine responses of male Siamese fighting fish Betta splendens to a matched-for-size conspecific fighting behind a one-way mirror,presented live or through video playback,were compared.The video playback and live stimulus elicited a strong and similar aggressive response by the focal fish,with a fight structure that started with stereotypical threat displays and progressed to overt attacks.Postfight plasma levels of the androgen 11-ketotestosterone were elevated as compared to controls,regardless of the type of stimuli.Cortisol also increased in response to the video images,as previously described for live fights in this species.These results show that the interactive component of a fight and its resolution are not needed to trigger an endocrine response to aggression in this species.The study also demonstrates for the first time in a fish a robust endocrine response to video stimuli and supports the use of this technique for researching aggressive behavior in B.splendens.

Attributes of stand-age-dependent forest determine technosol fertility of Atlantic forest re-growing on mining tailings in Mariana,Brazil

Understanding how soil fertility changes due to environmental conditions and stand-age-dependent forest attributes is important for local-scale forest restoration.We evaluated the effects of stand-age-dependent forest attributes(plant community composition and litter stock)on soil and technosol fertility across two second-growth Atlantic forests(SGF)after the deposition of mining tailings in Mariana,southeastern Brazil.We hypothesized that technosol fertility in the SGF tailings is positively affected by plant community composition variability,stand age,and litter stock.We used total exchangeable bases and organic matter as fertility indicators for technosol and soil,and species composition and litter stock as stand-age-dependent forest attributes.Our results showed significant differences in the stand-age-dependent forest attributes and soil chemical properties between the two forest patches(SGF tailing and SGF non-tailing)evaluated.Thus,there was a marked gradient of litter storage and fertility between soil and technosol that can be important forest recovery indicators for the affected plant communities.Furthermore,according to the tested models,we corroborated the hypothesis that technosol fertility is positively affected by stand age,plant community composition variability,and litter stock,which may contribute considerably to forest recovery on tailings.Our results demonstrate that the fertility predictors analyzed to explain the forest recovery on tailings can also be considered as ecological indicators for assessing forest restoration in areas impacted by mining tailings in Mariana.

Globalization and long-run co-movements in the stock market for the G7: An application of VECM under structural breaks

This paper analyzes the process of long-run co-movements and stock market globalization on the basis of cointegration tests and vector error correction (VEC) models. The cointegration tests used here allow for structural breaks to be explicitly modeled and breakpoints to be computed on a relative-time basis. The data used in our empirical analysis were drawn from Datastream and comprise the natural logarithms of relative stock market indexes since 1973 for the G7 countries. The main results point to the conclusion that significant causal cointegration effects occur in this context and that there is a long-run relationship that governs the worldwide process of market integration. Globalization, however, is a complex adjustment process and in many cases there is only evidence of weak market integration which means that non-proportional price transmission occurs in the market along with proportional changes. The worldwide markets, as expected, appear to be driven in general by the US stock market.

Graphite Nucleation in Cast Iron Melts Based on Solidification Experiments and Microstructure Simulation

Microstructure strongly influences the mechanical properties of cast iron. By inoculating the melt with proper inoculants, foreign substrates are brought into the melt and eventually the graphite can crystallize on them. The elements and substrates that really play a role for nucleation are yet unknown. Until now there is very little knowledge about the fundamentals of nucleation, such as composition and morphology of nuclei. In this work we utilized EN-GJL-200 as a base material and examined several produced specimens. The specimens were cast with and without inoculants and quenched at different solidification states. Specimens were also examined with a high and low oxygen concentration, but the results showed that different oxygen contents have no influence on the nucleation in cast iron melts. Our research was focused on the microscopic examination and phase-field simulations. For studying the samples we applied different analytical methods, where SEM-EDS, -WDS were proved to be most effective. The simulations were conducted by using the software MICRESS, which is based on a multiphase-field model and has been coupled directly to the TCFE3 thermodynamic database from TCAB. On the basis of the experimental investigations a nucleation mechanism is proposed, which claims MnS precipitates as the preferred site for graphite nucleation. This theory is supported by the results of the phase-field simulations.
