研究了超临界CO2夹带乙醇流体从裂殖壶藻Schizochytrium sp.中提取富含DHA油脂的新工艺。基于气液相平衡性质,确定了夹带剂乙醇的添加范围,并对提取过程进行动力学分析,确定了二氧化碳的使用量。通过中心组合设计的响应面优化试验,获得了最佳提取工艺条件:温度52℃、压力38 MPa、乙醇摩尔比0.04。该条件下,微藻油脂的回收率约为88.2%,显著高于纯超临界CO 2萃取的回收率(71.9%)。根据Peng-Robinson方程考察了混合流体的密度变化对油脂溶解度的影响,部分解释了操作条件对油脂回收率的影响。脂肪酸组成分析显示,新工艺得到的油脂中的DHA质量分数约为39.9%,略高于传统工艺。由此可知,超临界CO 2夹带乙醇流体是一种极性范围广、溶解能力强的绿色提取介质,非常适用于中性微藻油脂的提取。

Over the last decade,we have been facing a new aetiology responsible for the development of HCC-the nonalcoholic fatty liver disease(NAFLD).The prevalence of HCC development in this group is higher than that observed in the general population and in non-cirrhotic subjects with other causes of liver *** ultrasound(US)is the first-line tool for HCC surveillance,but,in this population,it has a decreased diagnostic accuracy due to several particular features,including obesity and ***-enhanced ultrasound(CEUS)appeared as a new branch of US due to its ability to depict the vascular architecture of all types of focal lesions(FLs).Nevertheless,CEUS has several limitations besides those inherited from US,which renders this method unreliable as the first-line HCC diagnostic tool and for HCC *** intelligence eliminates operator limitations,which has led to an increased sensitivity and specificity of ***,this approach is still in its early stages and more data are ***,the purpose of the current study is to highlight the strengths and limits of US,along with its alternatives to HCC screening in NAFLD population.

A 39-year-old woman consulted in the postpartum period because of agalactia, polyuria, polydipsia, and headache and decreased visual acuity. The initial analysis was compatible with a panhypopituitarism. The magnetic resonance (MRI) showed an expansive sellar and suprasellar lesion of high density in the basal study and enhancement after contrast administration, in contact with the optic chiasm, so urgent surgery was indicated. While waiting for the surgery, intravenous corticosteroids were initiated and the symptoms rapidly resolved. Most of the pituitary axes were recovered, except the diabetes insipidus. The control MRI showed complete resolution of the sellar mass. The surgery was canceled and the diagnosis of lymphocyte hypohpyisitis was made. Lymphocyte hypophysitis is an uncommon disease in which inflammation of the pituitary gland occurs. This results in a permanent or transient hormonal insufficiency. Lymphocyte hypophysitis is a very rare condition that occurs preferentially in women in the peripartum period. Early initiation of high doses of systemic corticosteroids may avoid unnecessary surgery in some cases.

AIM:To find out if by combining 2 ultrasound based elastographic methods:acoustic radiation force impulse(ARFI)elastography and transient elastography(TE),we can improve the prediction of fibrosis in patients with chronic hepatitis ***:Our study included 197 patients with chronic hepatitis *** each patient,we performed,in the same session,liver stiffness(LS)measurements by means of TE and ARFI,respectively,and liver biopsy(LB),assessed according to the Metavir score.10 LS measurements were performed both by TE and ARFI;median values were calculated and expressed in kilopascals(kPa)and meters/second(m/s),*** TE and ARFI measurements with IQR<30%andSR≥60%were considered ***:On LB 13(6.6%)patients had F0,32(16.2%) had F1,52(26.4%)had F2,47(23.9%)had F3,and 53(26.9%)had F4.A direct,strong correlation was found between TE measurements and fibrosis(r=0.741),between ARFI and fibrosis(r=0.730)and also between TE and ARFI(r=0.675).For predicting significant fibrosis(F≥2),for a cutoff of 6.7 kPa,TE had 77.5% sensitivity(Se)and 86.5%specificity(Sp)[area under the receiver operating characteristic curve(AUROC)0.87] and for a cutoff of 1.2 m/s,ARFI had 76.9%Se and 86.7%Sp(AUROC 0.84).For predicting cirrhosis(F=4),for a cutoff of 12.2 kPa,TE had 96.2%Se and 89.6% Sp(AUROC 0.97)and for a cutoff of 1.8 m/s,ARFI had 90.4%Se and 85.6%Sp(AUROC 0.91).When both elastographic methods were taken into consideration,for predicting significant fibrosis(F≥2),(TE≥6.7 kPa and ARFI≥1.2 m/s)we obtained 60.5%Se,93.3% Sp,96.8%positive predictive value(PPV),41.4%negative predictive value(NPV)and 68%accuracy,while for predicting cirrhosis(TE≥12.2 kPa and ARFI≥1.8 m/s) we obtained 84.9%Se,94.4%Sp,84.9%PPV,94.4% NPV and 91.8%***:TE used in combination with ARFI is highly specific for predicting significant fibrosis;therefore when the two methods are concordant,liver biopsy can be avoided.

Psychiatry Trainees’ Attitudes towards Psychotherapy

Background: Psychotherapy education is one of the essential components of specialist training in psychiatry according to the Royal College of Physicians of Canada. How psychiatry trainees perceive psychotherapy has major implications for how trainees will learn about psychotherapy during training, how they will utilize it in their future practice, and how they will integrate it into their professional identity. Methods: The study involved the distribution of questionnaires to psychiatry residents at an Atlantic Canadian Medical School. The survey consisted of Likert-scaled items which assessed residents’ attitudes towards being a psychotherapist and their future study and practice plans in psychotherapy. Results: Surveys were completed by 18 out of 25 residents (72% response rate). Half of participants indicated that conducting psychotherapy was highly rewarding and the majority of residents planned to incorporate psychotherapy into their future practice. Several mean differences were also noted between those residents who completed their undergraduate medical education in Canada versus at International locations. Conclusion: Despite the recent changes in psychotherapy training requirements and concerns about psychotherapy’s role in psychiatry, many residents perceive psychotherapy skills as being important for competent psychiatry practice and plan to incorporate their psychotherapy training into their future practice. Additional implications for teaching, future research, and practice are discussed.

Is Surgery a Good Choice for Patients over 100 Years with Hip Fracture?

Introduction: Elderly patients represent a difficult category of surgical candidates for orthopedic surgery because they have multiple associated diseases and a high degree of osteoporosis. Presentation of Case: We present the case of a 103 years old woman with a pertrohanteric hip fracture treated successfully using a 135° dynamic hip screw. Discussion: There are many discussions about this kind of patients regarding postoperative complications, duration of hospitalization, stage of recovery and the patient’s status at discharge. Conclusion: We conclude that surgery may benefit patients who are over 100 year-old.
