A Case Report of Lymphocytic Hypophysitis Related to Pregnancy

A 39-year-old woman consulted in the postpartum period because of agalactia, polyuria, polydipsia, and headache and decreased visual acuity. The initial analysis was compatible with a panhypopituitarism. The magnetic resonance (MRI) showed an expansive sellar and suprasellar lesion of high density in the basal study and enhancement after contrast administration, in contact with the optic chiasm, so urgent surgery was indicated. While waiting for the surgery, intravenous corticosteroids were initiated and the symptoms rapidly resolved. Most of the pituitary axes were recovered, except the diabetes insipidus. The control MRI showed complete resolution of the sellar mass. The surgery was canceled and the diagnosis of lymphocyte hypohpyisitis was made. Lymphocyte hypophysitis is an uncommon disease in which inflammation of the pituitary gland occurs. This results in a permanent or transient hormonal insufficiency. Lymphocyte hypophysitis is a very rare condition that occurs preferentially in women in the peripartum period. Early initiation of high doses of systemic corticosteroids may avoid unnecessary surgery in some cases.

To the Editor,Marfan syndrome is an autosomal dominant disorder of connective tissue caused by mutations in the gene encoding a matrix component of microfibrils;fibrillin-1(FBN1),with an incidence of about 1-3 per 10,000 individuals.2.3 Cardinal manifestations include proximal aortic aneurysm,dislocation of the ocular lens,and long-bone *** ectasia,which is defined as a ballooning or significant widening of the dural sac or neural root sleeves,is also a common finding in these patients,which is reported to present in more than 60%of cases and is considered a major criterion for the clinical diagnosis of this syndrome.

Deep learning (DL) has seen an exponential development in recent years, with major impact in many medical fields, especially in the field of medical image. The purpose of the work converges in determining the importance of each component, describing the specificity and correlations of these elements involved in achieving the precision of interpretation of medical images using DL. The major contribution of this work is primarily to the updated characterisation of the characteristics of the constituent elements of the deep learning process, scientific data, methods of knowledge incorporation, DL models according to the objectives for which they were designed and the presentation of medical applications in accordance with these tasks. Secondly, it describes the specific correlations between the quality, type and volume of data, the deep learning patterns used in the interpretation of diagnostic medical images and their applications in medicine. Finally presents problems and directions of future research. Data quality and volume, annotations and labels, identification and automatic extraction of specific medical terms can help deep learning models perform image analysis tasks. Moreover, the development of models capable of extracting unattended features and easily incorporated into the architecture of DL networks and the development of techniques to search for a certain network architecture according to the objectives set lead to performance in the interpretation of medical images.

Objective:Oral cavity squamous cell carcinoma (SCC) may present with early invasion of mandibular *** planning of surgery is essential considering patient's postoperative quality of *** purpose was to evaluate the efficacy of computer tomography scan (CT) and magnetic resonance imaging (MRI) in detecting mandibular bone involvement in oral ***:A retrospective study was conducted on 98 patients with SCC of floor of mouth,lower alveolus and retromolar trigone operated on with curative *** CT and MRI scans were re-reviewed by a consultant radiologist and original histology slides were rereviewed by 3 ***:Forty-five patients were included in the final *** CT and MRI had a sensitivity of 100% and a specificity of 72%.Conclusion:The results suggest that combined CT and MRI have diagnostic utility in detecting mandibular invasion by oral cancer,but with a significant false positive rate.

Objective To examine the relationship between Tpeak-Tendinterval（Tpe） and Tpe/QT ratio with occurrence of ventricular premature beats（VPBs） and left ventricular remodeling in hypertension. Methods A total of 52 patients with mild to moderate essential hypertension were included, undergoing echocardiography and 24-hours Holter monitoring. Ventricular remodeling was assessed by left ventricular mass index（LVMI） using the Devereux formula and diastolic function by transmitral E and A wave velocities and E/A ratio. Tpe was measured in the precordial leads. The end of the T wave was set by the method of the tangent to the steepest descending slope of the T wave. Results Tpe and Tpe/QT in leads V2（r = 0.33, P = 0.01; r = 0.27, P = 0.04 respectively） and V3（r = 0.40, P = 0.002; r = 0.40, P = 0.003, respectively）correlated significantly with LVMI. A significant inverse relationship was observed between E/A ratio and QT（r =-0.33, P = 0.01）, Tpe in V3（r =-0.39,P=0.003） and Tpe/QT in V3（r =-0.31,P= 0.02）. Tpe in V3, V5, mean Tpe and maximum Tpe with cut-off values of 60 ms,59 ms, 62 ms and 71 ms, respectively, associated with the occurrence of ventricular premature beats. Conclusions The repolarization parameters Tpe interval and Tpe/QT ratio correlate with LVMI and indices of left ventricular diastolic function and show better predictive values than traditional parameters such as QT interval and QT dispersion. Lead V3is the best lead for measuring Tpe and Tpe/QT. These ECG indices can therefore be used in clinical practice to monitor LV remodeling and predict occurrence of VPBs.

Deep learning (DL) has experienced an exponential development in recent years, with major impact in many medical fields, especially in the field of medical image and, respectively, as a specific task, in the segmentation of the medical image. We aim to create a computer assisted diagnostic method, optimized by the use of deep learning (DL) and validated by a randomized controlled clinical trial, is a highly automated tool for diagnosing and staging precancerous and cervical cancer and thyroid cancers. We aim to design a high-performance deep learning model, combined from convolutional neural network (U-Net)-based architectures, for segmentation of the medical image that is independent of the type of organs/tissues, dimensions or type of image (2D/3D) and to validate the DL model in a randomized, controlled clinical trial. We used as a methodology primarily the analysis of U-Net-based architectures to identify the key elements that we considered important in the design and optimization of the combined DL model, from the U-Net-based architectures, imagined by us. Secondly, we will validate the performance of the DL model through a randomized controlled clinical trial. The DL model designed by us will be a highly automated tool for diagnosing and staging precancers and cervical cancer and thyroid cancers. The combined model we designed takes into account the key features of each of the architectures Overcomplete Convolutional Network Kite-Net (Kite-Net), Attention gate mechanism is an improvement added on convolutional network architecture for fast and precise segmentation of images (Attention U-Net), Harmony Densely Connected Network-Medical image Segmentation (HarDNet-MSEG). In this regard, we will create a comprehensive computer assisted diagnostic methodology validated by a randomized controlled clinical trial.
